Tratamiento de la tormenta ventricular arrítmica revisión sistemática y metaanálisis

Resumen
Introducción: Se define tormenta arrítmica (TA) o tormenta eléctrica (ES) como la ocurrencia de tres o más episodios distintos de TV o FV en un período de tiempo de 24 horas, se considera un factor de riesgo para complicaciones de origen cardíaco y predictor de mortalidad, actualmente no existe consenso para el manejo de esta entidad. Objetivo: Generar recomendaciones para el manejo y el tratamiento farmacológico de mayor impacto en los pacientes con tormenta arrítmica según lo publicado en la literatura mundial. Materiales y métodos: Revisión sistemática en la cual se analizaron estudios analíticos y experimentales publicados entre 1998 a 2015 en los cuales el objetivo principal se enfocó en el manejo de pacientes con tormenta arrítmica, se garantizó la calidad metodológica tomando como base la estrategia Cochrane y la declaración PRISMA. Resultados: se analizaron 33 publicaciones, se evidenció que los factores de predicción de mayor impacto fueron la fracción de eyección por debajo del 30-25%, edad mayor a 65 años y ausencia de control de la patología de base. En el tratamiento se considera la radioablación en modalidad subendocárdica como primera opción de tratamiento en casos de tormenta arrítmica ante fallo en el manejo farmacológico o como terapia de rescate, seguido por el bloqueo simpático, los autores sitúan el manejo farmacológico como terapia adyuvante. Conclusión: la evidencia disponible no es suficiente para la redacción de lineamientos claros en el manejo de dicha patología, nuevas investigaciones que aborden el objetivo de manejo óptimo del paciente con Tormenta Eléctrica son requeridos.
Abstract
Introduction: Arrhythmic storm (AT) or electrical storm (ES) is defined as the occurrence of three or more episodes of VT or VF in a period of 24 hours, it is considered a risk factor for complications of cardiac origin and predictor of mortality, currently there is no consensus for the management of this entity. Objective: Generate recommendations for the management and pharmacological treatment of greater impact in patients with arrhythmic storm as published in the world literature. Materials and methods: Systematic review in which analytical and experimental studies published between 1998 and 2015 were analyzed in which the main objective was focused on the management of patients with arrhythmic storm, methodological quality was guaranteed based on the Cochrane strategy and the PRISM statement. Results: 33 publications were analyzed, it was evidenced that the prediction factors with the highest impact were the ejection fraction below 30-25%, age over 65 years and absence of control of the underlying pathology. In the treatment, radioablation is considered in the subendocardial modality as the first treatment option in cases of arrhythmic storm, before failure in pharmacological management or as rescue therapy, followed by sympathetic blockade, the authors place pharmacological management as adjuvant therapy. Conclusion: The available evidence is not sufficient for the drafting of clear guidelines in the management of this pathology, new investigations that address, the objective of optimal management of the patient with Electrical Storm are required.
