Acretismo placentario: perfil clínico y complicaciones en gestantes del hospital universitario clínica San Rafael, 2009 - 2016

dc.descriptionEl acretismo placentario ha venido en aumento con la realización de mayor número de intervenciones a nivel uterino, como la cesárea, miomectomía hasta afectar al 0.9% de todos los embarazos. Se describe el perfil clínico y complicaciones de una cohorte de pacientes con acretismo placentario. Metodología: Se realizó estudio longitudinal descriptivo con recolección retrospectiva de los datos, incluyendo la totalidad de pacientes con acretismo placentario. El análisis se presenta en términos descriptivos. Resultados: Se encontraron 41 casos de acretismo placentario confirmado en la institución, incidencia de 0.17/1000 pacientes atendidas. La edad gestacional promedio para terminar el embarazo fue 34 semanas, el 100% tuvieron manejo quirúrgico, el 50% requirieron transfusión de hemoderivados, con una mediana de 4 UGRE. El estudio doppler y el ultrasonido se encuentran en baja disponibilidad en nuestro medio, ninguna paciente tuvo acceso a estudios de resonancia magnética. Discusión: A pesar de los avances en medicina el acretismo placentario sigue siendo una entidad con alta morbilidad, difícil de diagnosticar principalmente por la falta de acceso a servicios de salud.spa
dc.description.abstractPlacenta accreta has been increasing worldwide, especially because of the greater number of interventions done at the uterine level, such as caesarean section, myomectomy affecting up to 0.9% of all pregnancies. This study intended to describe the clinical profile and complications of a cohort of patients with abnormal placenta accreta. Methods: A descriptive longitudinal study was performed with retrospective data collection, including all patients with abnormal placenta accreta in the institution during the period between 2009 and 2016. The analysis is presented in descriptive terms. Results: 41 cases of confirmed placenta accreta were found in the institution, the calculated prevalence was 0.17 / 1000 patients attended. The average gestational age to complete the pregnancy was 34 weeks, 100% had surgical management, 50% required transfusion of blood products, with a median of 4 UGRE. The doppler study and the ultrasound had a low availability in our environment, no patient had access to magnetic resonance studies. Discussion: Despite advances in placenta accreta, it still remains an entity with high morbidity that is difficult to diagnose mainly due to lack of access to health services.eng
