Introducción: La enfermedad aterosclerótica en la arteria carótida con estenosis significativa de su luz en pacientes con síntomas isquémicos cerebrovasculares presume nuevos y repetitivos eventos isquémicos; por lo que el manejo médico óptimo, y quirúrgico tanto abierto como endovascular busca disminuir la severidad e incapacidad que genera en estos pacientes. Se pretende describir las características sociodemográficas y factores de riesgo, así como precisar la técnica quirúrgica y resultados en el posoperatorio mediato en los pacientes intervenidos de endarterectomía carotídea posterior a algunos síntomas isquémicos neurológicos, durante el periodo 2018-2021 en la Fundación Cardioinfantil – laCardio. Materiales y métodos: Estudio transversal observacional de tipo descriptivo retrospectivo, que incluyó todos los pacientes con estenosis carotidea sintomática, candidatos a endarterectomía carotidea temprana (primeros 14 días posterior al evento) se obtuvo información de las características sociodemográficas y resultados clínicos de la base de datos del servicio de cirugía vascular y angiología. Resultados: Se evaluaron 19 pacientes con mediana de edad de 77 años, el 57.89% de género masculino. Entre los factores de riesgo se encuentran 78,95% hipertensión arterial, 63,16% dislipidemia, diabetes mellitus el 31,58% y enfermedad coronaria el 10,53%. El 73,68% de los pacientes presentaron evento cerebrovascular agudo con mediana de 2.5. de puntuación en la escala de NIHSS. El tiempo de intervención posterior al evento cerebrovascular o síntoma isquémico se realizó al séptimo dia. Como desenlaces no se reportó mortalidad durante la hospitalización y se reintervino un paciente en el posoperatorio inmediato por sangrado quirúrgico. Conclusiones: El manejo multidisciplinario de los pacientes con enfermedad carotidea sintomática, con un equipo de expertos en diagnóstico clínico, interpretación de imágenes, manejo médico y tratamiento quirúrgico, demuestra desenlaces favorables en el posoperatorio mediato cuando se interviene antes de 14 dias posteriores al evento cerebro vascular, independientemente del NIHSS de ingreso del paciente.
Introduction: Atherosclerotic disease in the carotid artery with significant stenosis of its lumen in patients with cerebrovascular ischemic symptoms presumes new and repetitive ischemic events; therefore, optimal medical and surgical management, both open and endovascular, seeks to reduce the severity and disability it generates in these patients. The aim is to describe the sociodemographic characteristics and risk factors, as well as to specify the surgical technique and results in the mediate postoperative period in patients undergoing carotid endarterectomy after some neurological ischemic symptoms, during the period 2018-2021 in the Fundación Cardioinfantil - laCardio. Materials and methods: Observational cross-sectional study of retrospective descriptive type, which included all patients with symptomatic carotid stenosis, candidates for early carotid endarterectomy (first 14 days after the event) information on sociodemographic characteristics and clinical outcomes were obtained from the database of the vascular surgery and angiology service. Results: We evaluated 19 patients with a median age of 77 years, 57.89% were male. Risk factors included 78.95% hypertension, 63.16% dyslipidemia, 31.58% diabetes mellitus and 10.53% coronary artery disease. stroke event with a median score of 2.5 on the NIHSS scale occurred in 73.68% of patients. The intervention time after the cerebrovascular event or ischemic symptom was performed on the seventh day. No mortality was reported during hospitalization and one patient was re-intervened in the immediate postoperative period due to surgical bleeding. Conclusions: The multidisciplinary management of patients with symptomatic carotid disease, with a team of experts in clinical diagnosis, image interpretation, medical management and surgical treatment, demonstrates favorable outcomes in the immediate postoperative period when intervention is performed within 14 days after the cerebrovascular event, regardless of the NIHSS at admission.

Accesos vasculares para hemodiálisis, cuál es el más costo efectivo?

Marco conceptual: La enfermedad renal crónica es un serio problema de salud pública en nuestro país por la gran cantidad de recursos económicos que requiere su atención. La hemodiálisis es el tratamiento más usado en nuestro medio; el acceso vascular y sus complicaciones derivadas son el principal aspecto que incrementa los costos de atención en éstos pacientes.
Materiales y métodos: Se realizó un estudio económico de los accesos vasculares en pacientes incidentes de hemodiálisis en el año 2012 en la agencia RTS-Fundación Cardio Infantil. Se estableció el costo de creación y mantenimiento del acceso con catéter central, fístula arteriovenosa nativa, fístula arteriovenosa con injerto; y el costo de atención de las complicaciones para cada acceso. Se determinó la probabilidad de ocurrencia de complicaciones. Mediante un árbol de decisiones se trazó el comportamiento de cada acceso en un período de 5 años. Se establecieron los años de vida ajustados por calidad (QALY) en cada acceso y el costo para cada uno de éstos QALY.
Resultados: de 36 pacientes incidentes de hemodiálisis en 2012 el 100% inició con catéter central, 16 pacientes cambiaron a fístula arteriovenosa nativa, 1 a fístula arteriovenosa con injerto que posteriormente pasó a CAPD, 15 continuaron su acceso con catéter y 4 pacientes fallecieron. En 5 años se obtuvieron 2,36 QALY para los pacientes con catéter central que costarían $ 24.813.036,39/QALY y 2,535 QALY para los pacientes con fístula nativa que costarían $ 6.634.870,64/QALY.
Conclusiones: el presente estudio muestra que el acceso vascular mediante fístula arteriovenosa nativa es el más costo-efectivo que mediante catéter
Background: end stage chronic renal disease is a serious public health problem in our country because of the big economic burden that is spent on its management. Hemodialysis is the most used treatment in our setting; vascular access and its derived complications are the most important aspect for raising attention’s costs in this group of patients. Materials and methods: an economical study was undertaken with the hemodialysis incident patients attended in RTS-Fundación Cardio Infantil agency during 2012. The costs of creation, maintenance and complication management of the vascular access via central venous catheter, native arteriovenous fistula and graft arteriovenous fistula were established. The probability of complication occurrence was calculated and a decision tree model was used to establish the behavior or each access in a five years period. The quality adjusted life years (QALYs) were established for each access and the economic ratio for each QALY was calculated also. Results: there were 36 incident hemodialysis patients in 2012, 100% started hemodialysis via central venous catheter, 16 patients change their access to a native arteriovenous fistula, 1 to a graft arteriovenous fistula and latter to CAPD, 15 patients continued with central venous catheter and 4 patients died. In 5 years 2, 36 QALY were obtained for central venous catheter patients that cost $ 24. 813. 036, 39/QALY and 2, 535 QALY for arteriovenous fistula with a ratio of $ 6. 634. 870, 64/QALY. Conclusions: the present study demonstrates that native arteriovenous fistula is a more cost-effective vascular access for hemodialysis than central venous catheters.

Popliteal arteriovenous fistulae (PAF) are anomalous communications between the arterial and venous systems of the lower extremity. They are usually secondary to trauma and are rarely associated with additional vascular defects. The coexistence of a PAF and a venous aneurysm is rare and usually occurs in patients with connective tissue disorders. Evidence regarding the management of this type of anomaly is scarce. However, both open and endovascular approaches seem feasible alternatives for treating this condition. Here, we describe a spontaneous popliteal arteriovenous fistula associated with a venous aneurysm in a 42-year-old male patient who presented with a popliteal mass. Satisfactory endovascular closure of the fistula and exclusion of the venous aneurysm were achieved using an Amplatzer Vascular Plug II.

Introducción: Existen en la actualidad múltiples intervenciones en cirugía vascular que pueden mejorar el pronóstico. Se pretendió describir frecuencia de amputación, infección y necesidad de reintervención en pacientes con isquemia crítica de miembros inferiores sometidos a revascularización, durante el periodo 2018 – 2019 en la Fundación Cardioinfantil Instituto de Cardiología. Materiales y métodos: Estudio de corte transversal, que incluyó todos los pacientes con isquemia crítica de miembros inferiores que de manera electiva fueron llevados a uno de dos procedimientos para revascularización de miembro inferior, sin la intención de ser comparados entre sí. Se seleccionó una muestra por conveniencia, y se obtuvo información de las características sociodemográficas y resultados clínicos de la base de datos del servicio de cirugía vascular y angiología. Resultados: Se evaluaron 87 casos, con un promedio de edad 70 años ±11 años. Se realizó cirugía abierta en 37 pacientes y terapia endovascular en 48 pacientes para revascularización. La decisión de la técnica fue según la clasificación de WIfI y TASC II. Los resultados fueron: amputación (20.6%), infección de herida (18.3%), necesidad de reintervención (8.5%) y muerte en el (8.05%) . No hubo diferencias significativas, en la frecuencia de los desenlaces entre ambas técnicas. Conclusiones: Este estudio demostró desenlaces similares en el manejo con ambas técnicas de revascularización endovascular y cirugía abierta. La tasa de amputación y de infección no tuvo diferencias entre los grupos. Se evidenció una asociación significativa en pacientes con diabetes y/o 3 o más comorbilidades con muerte. Background: Critical lower limb ischemia is defined as a clinical syndrome of ischemic pain at rest and loss of tissue, associated with ulcers that are difficult to heal, and/or gangrene. There are currently multiple interventions in vascular surgery that can improve prognosis. We pretended to describe the clinical outcomes in patients with critical lower limb ischemia undergoing lower limb revascularization in a highly complex institution, during the period 2018-2019. Methods: A cross-sectional study was performed, in which we included all patients with chronic lower limb ischemia who were electively brought to lower limb revascularization from January 2018 to December 2019. A convenience sample was selected, and their medical records were reviewed. Sociodemographic, imaging and clinical data are presented as results in a database of the vascular surgery and angiology service. Results: 87 patients were evaluated, with an average age of 70 years ± 11 years. The technique was endovascular in 48 patients and an open technique in 37 patients. Among the most frequent complications were the need for amputation 20. 6% and wound infection 18. 3%. The factors associated with complications were the presence of DM, greater severity of the disease and a history of AMI. Conclusions: The data analysis showed similar outcomes in the management with both techniques. The rate of amputation and infection of the operative site did not differ between the types of management performed. A significant association was evidenced in patients with diabetes and / or 3 or more comorbidities with death.
