La prevalencia de coledocolitiasis es de un 10 a 20%. 10-20% tienen coledocolitiasis gigante, es decir presencia de cálculos mayores de 15 mm, aumentando la morbimortalidad por complicaciones. El objetivo principal fue determinar la frecuencia de coledocolitiasis gigante, la presencia de factores predictores del éxito o fracaso del manejo endoscópico. El éxito en el manejo endoscópico está entre 80 y 90%, un 20% requieren cirugía de exploración biliar. Se realizó la búsqueda de las variables utilizando el instrumento para la recolección de la información. Se realizó un análisis univariado y bivariado de las variables medidas y se utilizo STATA versión 10. Como principal resultado, se encontró que la frecuencia de coledocolitiasis gigante en nuestra población fue del 10%, el éxito del manejo endoscopio fue del 89.23% y el factor predictor mas fuerte para el éxito fue el diámetro del cálculo, siendo mayor para cálculos de menos de 19.09 mm. Como conclusión, en nuestro estudio, la frecuencia de coledocolitiasis gigante es cercana a la conocida en la literatura mundial. El manejo endoscópico en nuestro estudio es el pilar en estos casos, teniendo probabilidad de éxito en el manejo que es igual a la publicada en los estudios mundiales, que existe la probabilidad que el tamaño del cálculo mayor a 19 mm de diámetro indique mayor tasa de fracaso y requerimiento de técnicas endoscópicas avanzadas para su éxito. Se requieren estudios, con mayor número de pacientes para determinar la validez estadística de estos resultados.
Choledocholithiasis prevalence is 10 to 20%.

The primary objective was to determine the frequency of giant choledocholithiasis, the presence of predictors of success or failure of endoscopic management.
Successful endoscopic management is between 80 and 90%, 20% require surgery biliary exploration.
Search was performed using the instrument for the collection of information. We performed univariate and bivariate analysis of the measured variables and use STATA version 10.
As a main result, we found that the frequency of giant choledocholithiasis in our population was 10%, the endoscopic management success was 89.23% and the strongest predictor for success was the diameter of the stones, stones being higher for less of 19.09 mm.
In conclusion, in our study, the frequency of choledocholithiasis giant is close to the known world literature. Endoscopic treatment in our study is the mainstay in these cases, with probability of success in management that is equal to the one published in global studies, it is likely that the stone size greater than 19 mm in diameter indicates higher rate of failure and advanced endoscopic techniques requirement for success.
Studies are needed with larger numbers of patients to determine the statistical validity of these results.

Fibrolamellar hepatocarcinoma is an infrequent liver tumor, currently considered to be a variant different from hepatocarcinoma. The differences lie in genomic alterations, a greater prevalence of fibrolamellar hepatocarcinoma in young patients, and its lack of association with underlying liver disease. The clinical presentation is unspecific, with symptoms ranging from abdominal pain, malaise, and weight loss to atypical manifestation which include hyperammonemic encephalopathy. We present the case of a 33-year-old woman with no prior medical history who presented with a coma and a diagnosis of inoperable fibrolamellar hepatocarcinoma requiring a cadaver donor transplant. While she was on the waiting list, she received hemofiltration and ammonium benzoate treatment, with progressive improvement in her state of consciousness.

Introducción : En Colombia el cancer colorectal es la cuarta causa de muerte por cancer. La colonoscopia es una estrategia de tamizaje que puede prevenir el desarrollo de lesiones precancerosas. Es importante caracterizar clínica y endoscópicamente la población que es llevada a colonoscopia en nuestro medio, así como el rendimiento y el cumplimiento de los indicadores de calidad de este procedimiento. Objetivo: Caracterizar clínica y endoscópicamente la población de pacientes a quienes se les practicó colonoscopia en el HUS de junio 2017 hasta junio 2018. Materiales y metodos: Estudio observacional descriptivo de pacientes llevados a colonoscopia en el HUS entre Junio 2017 hasta Junio 2018. Resultados : Se incluyeron 554 colonoscopias. La distribución entre hombres y mujeres fue similar y su media de edad fue de 57 años. El 81% de las colonoscopias tenían adecuada preparación. En el 90% de las colonoscopias se logró visualizar el ciego. La tasa de detección de adenomas fue del 8,7%. El tiempo de retirada fue igual o mayor a 6 minutos en el 90% de los casos. La prevalencia de tumores colorrectales en la muestra fue del 6,3%. Conclusiones : La proporción de cumplimiento de los indicadores de calidad de acuerdo con los estándares nacionales fue buena para firma de consentimiento informado y porcentaje de las colonoscopias en las que se visualizó el ciego. La tasa de detección de adenomas fue baja y la prevalencia de tumores colorectales es más baja que la reportada en Colombia.
Introduction: In Colombia the colorectal cancer is the fourth cause of death from cancer. The colonoscopy is a strategy for screening that helps to prevent pre-cancerous lesions. It is important to characterize clinically and endoscopically in our environment the success of this procedure. Objective: Characterize clinically and endoscopically all HUS patients who have received a colonoscopy from June 2017 to June 2018. Materials and Methods: Observational descriptive study of HUS patients which got a colonoscopy procedure at HUS from June 2017 to June 2018. Results: The results include a total of 554 colonoscopies. The gender distribution was similar between men and women and the age average was 57 years. In the 90% of the colonoscopies the Boston scale was reported to be achieved. In the 90% of the colonoscopies the cecum was visualized. The adenomas detection rate was 8,7%. The time needed to remove them was equal or greater 6 minutes in the 90% of the cases. The colorectal tumor rate was 6,3%. Conclusions: The proportion of success of the quality indicators with respect to the national standard was good on the order of frequency with which informed consent was obtained and fully documented and the percentage of the colonoscopies that the cecum was visualized. The adenomas and colorectal tumors rate detection rate was lower compare to Colombia national rate.

Introducción: La diverticulosis es la condición más frecuentemente encontrada en la colonoscopia, condición asintomática, con un alto costo para el sistema de salud. Diversos factores han demostrado estar en relación con la aparición de la enfermedad. En nuestra población, esta información se desconoce; el objetivo del estudio es la caracterización de la población con diverticulosis y su relación con la frecuencia de la ingesta de fibra.
Materiales y Métodos: Estudio observacional prospectivo de corte transversal con componente analítico. Realizado a todas las personas que asistieron al HUS a realización de colonoscopia entre Noviembre de 2015 y Abril de 2016. Se recolectaron datos de la frecuencia de fibra ingerida a través de entrevistas basadas.
Resultados: Se estudiaron 278 personas, 55.7% mujeres. La prevalencia de diverticulosis fue de 21.58%, siendo más frecuente en mujeres ( 66,7%), 31% entre los 71-80 años, 2,16% tenían antecedente familiar de diverticulosis; principal sitio de afectación fue colon sigmoide en las mujeres y el colon descendente en los hombres. En las personas con diverticulosis el consumo de harina de trigo fue mayor (91,67% vs 86,7%), mientras el consumo de frutas fue mayor en las personas que no presentaron diverticulosis (83,49% vs 78,33%).
Conclusiones: La prevalencia de diverticulosis es similar a lo reportado en la literatura. Así mismo se encontró un mayor consumo de fibra en la población sin divertículos lo que hace pensar que a pesar de que esta condición es multifactorial , el consumo de mayores cantidades de fibra puede prevenir la aparición de la misma
Introduction: Diverticulosis is the condition most often found on colonoscopy, It`s an asymptomatic condition, with a high cost to the health system. Several factors have shown to be related to the onset of disease. In our population, this information is unknown; the aim of the study is the characterization of people with diverticulosis and its relationship with the frequency of fiber intake. Materials and Methods: Prospective observational cross-sectional study with an analytical component. It was made to everyone who attended the HUS for colonoscopy between November 2015 to April 2016 the frequency data of fiber ingested was obteined through interviews. Results: 278 people were studied, 55. 7% were women. The prevalence of diverticulosis was 21. 58%, being more frequent in women (66. 7%), 31% between 71-80 years, 2. 16% had a family history of diverticulosis; the sigmoid colon was the main site of involvement in women and descending colon in men. In people with diverticulosis the consumption of wheat flour was higher (91. 67% vs 86. 7%), while fruit consumption was higher in people who did not have diverticulosis (83. 49% vs 78. 33%) . Conclusions: The prevalence of diverticulosis is similar to that reported in the literature. A pesar del avance en las tecnologías y técnicas quirúrgicas, la morbilidad y mortalidad post trasplante hepático siguen siendo altas y las complicaciones biliares la causa más frecuente con una incidencia global entre el 15% y el 25% y mortalidad asociada del 10%. El objetivo del presente estudio es describir el manejo de las complicaciones biliares y los resultados endoscópicos en los pacientes post trasplante hepático que fueron tratados mediante colangioscopia en un centro referente de trasplante hepático de Colombia en los años 2020 a 2022
Despite the advance in surgical technologies and techniques, post-liver transplant morbidity and mortality remain high and biliary complications are the most frequent cause with an overall incidence between 15% and 25% and associated mortality of 10%. The objective of the present study is to describe the management of biliary complications and endoscopic results in post-liver transplant patients who were treated by cholangioscopy in a leading liver transplant center in Colombia in the years 2020 to 2022.

Introducción: El síndrome ictérico es una aproximación diagnostica que reúne distintos signos y síntomas. Dependiendo de su etiología tiene como alternativa terapéutica la realización de una colangiopancreatografía retrógrada endoscópica (CPRE), procedimiento invasivo, que tiene riesgos de morbilidad y mortalidad por esta razón su realización debe ser exclusivamente terapéutica y no diagnostica para lo que se requiere estudios diagnósticos previos certeros. Es importante estudiar si la ecografía hepatobiliar adecuada para detectar signos de patología ictérica obstructiva, puesto que en nuestro entorno, el ultrasonido determina la posible realización de una prueba invasiva, como es la CPRE.
El objetivo del estudio es determinar la correlación existente entre los hallazgos de la vía biliar encontrados en la ultrasonografía hepatobiliar y los de la CPRE realizadas en el Hospital Departamental Universitario de la Samaritana (HUS), Bogotá durante el periodo comprendido entre el 01.03.05 y 01.11.07
Materiales y Método: Estudio de concordancia de pruebas diagnósticas retrospectivo. Mediante un cuestionario cerrado se recolecto la información relacionada con las características del paciente, hallazgos del ultrasonido hepatobiliar y reporte de CPRE. Se calcularon los valores de sensibilidad, especificidad y valores predictivos de la ultrasonografía hepatobiliar previa al CPRE. Se utilizó la razón de verosimilitud (RV) como indicador del desempeño del test diagnóstico. El programa para el análisis de datos usado fue EPIDAT versión 3.1. Resultados: Durante el período de estudio, 457 pacientes fueron llevados a CPRE, de los cuales, 271 cumplieron los criterios de inclusión, la ecografía hepatobiliar para el diagnostico de vía biliar dilatada tiene una sensibilidad del 66.5% (IC 95%: 60.2 -72.5) y una especificidad del 65.6% (IC 95%: 46.8 – 81.4), con un cociente de probabilidad negativo del 49% , para el diagnostico de coledocolitiasis una sensibilidad del 25.6% (IC 95%: 18.4 – 33.9) con una especificidad del 87.7% (IC 95%: 81.0 – 92.7). En conclusión, el estudio confirma en nuestro medio que la ecografía hepatobiliar tiene una baja sensibilidad y especificidad para el diagnostico de síndrome biliar obstructivo y coledocolitiasis y existe una baja concordancia entre los hallazgos de la ecografía hepatobiliar y la CPRE.

La CPRE es un procedimiento endoscópico terapéutico para resolver enfermedades obstructivas pancreatobiliares. Son procedimientos con una tasa de complicación menor al 10%. Existe controversia en la literatura en cuanto a los efectos de esta intervención en la población geriátrica mayor de 70 años, en términos de efectividad y complicaciones (propios del procedimiento y derivados de la anestesia). Objetivo: Determinar las diferencias en cuanto a efectividad endoscoópica (tasa de canulación efectiva) y seguridad (complicaciones del procedimiento y de la anestesia intraoperatoria), entre las poblaciones mayores y menores de 70 años a los que se realizó CPRE, en los años 2018 a 2020, en una institución de referencia. Metodología: Se realizó un estudio de corte transversal, evaluando los desenlaces mencionados en pacientes mayores y menores de 70 años. Se realizó también un análisis exploratorio en la población octagenaria. Resultados: Se encontró que la tasa de canulación efectiva fué mayor al 90%, sin diferencias significativas entre todos los grupos explorados. Con respecto a las complicaciones endoscopicas, la distribución fué homogenea, con una mayor tasa, aunque no significativa, de pancreatitis en menores de 70 años. Dentro de las complicaciones relacionadas con la anestesia, los mayores de 70 años presentaron mayor tasa de hipotensión (9,6% vs 3,0%, p<0,05) y mayor requerimiento de soporte vasopresor intraoperatorio (10,6% vs 3,9%, p<0,05), pero no un mayor requerimiento de UCI (7,6% vs 4,5%, p=0,2). En octagenarios, la tasa de complicaciones anestésicas fué similar (hipotensión durante procedimiento: 10,5% vs 4,75, p<0,05; soporte vasopresor intraoperatorio: 13,4% vs 5,0%, p<0,05), pero en este grupo si se encontró un mayor requerimiento de UCI postoperatorio (11,8% vs 3,8%, p<0,05), comparado con los menores de 80 años. Conclusión: El presente estudio sugiere que no hay diferencias en la tasa de canulación efectiva de la vía biliar y no parece ser una variable que se altere por la edad, ya que posiblemente sea más afectada por factores técnicos propios del procedimiento. Sin embargo, existen importantes diferencias entre los grupos explorados en cuanto a la carga de comorbilidades y de complicaciones derivadas de la anestesia, que parece estar relacionada con edad, especialmente en mayores de 80 años. Estos hallazgos invitan a realizar estudios prospectivos con un diseño apropiado, para confirmar estos resultados.
ERCP is a therapeutic endoscopic procedure to resolve obstructive pancreatobiliary diseases. They are procedures with a complication rate of less than 10%. There is controversy in the literature regarding the effects of this intervention in the geriatric population over 70 years of age, in terms of effectiveness and complications (specific to the procedure and derived from anesthesia). Objective: To determine the differences in endoscopic effectiveness (effective cannulation rate) and safety (complications of the procedure and intraoperative anesthesia), between populations older and younger than 70 years who underwent ERCP, in the years 2018 to 2020, in a reference institution. Methodology: A cross-sectional study was carried out, evaluating the mentioned outcomes in patients older and younger than 70 years. An exploratory analysis was also carried out in the eight-year-old population. Results: It was found that the effective cannulation rate was greater than 90%, without significant differences between all the groups explored. With respect to endoscopic complications, the distribution was homogeneous, with a higher, although not significant, rate of pancreatitis in those under 70 years of age. Among the complications related to anesthesia, those over 70 years of age had a higher rate of hypotension (9.6% vs 3.0%, p <0.05) and a higher requirement for intraoperative vasopressor support (10.6% vs 3 , 9%, p <0.05), but not a higher ICU requirement (7.6% vs 4.5%, p = 0.2). However, there are important differences between the groups examined in terms of the burden of comorbidities and complications derived from anesthesia, which seems to be related to age, especially in those older than 80 years. These findings invite prospective studies with an appropriate design to confirm these results.

Introducción: Tradicionalmente la colangiopancreatografía retrógrada endoscópica (CPRE) ha sido el estándar de oro para el diagnóstico y tratamiento de enfermedades biliares. Sin embargo, esta puede fallar entre un 5% a 20% de los pacientes, incluso en los centros de referencia(1); es por esta razón, que en las ultimas décadas se ha venido introduciendo la colangioscopia como una estrategia de intervención, la cual permite abordar casos de vía biliar difícil, que de no ser así, tendrían indicación quirúrgica con la consecuente exposición a un mayor riesgo para el paciente por las complicaciones inherentes a dicho procedimiento. El objetivo de este estudio fue describir las características demográficas, clínicas, paraclínicas y desenlaces, de individuos que fueron llevados a colangioscopía SpyGlass en la Fundación Cardioinfantil entre Febrero de 2019 hasta Febrero de 2021. Metodología Estudio descriptivo, retrospectivo de pacientes consecutivos de todas las edades que fueron llevados a colangioscopia por Spyglass por diferentes indicaciones entre (Febrero de 2019 y Febrero de 202)1; Se evaluaron las características demográficas, las relacionadas con el procedimiento y los resultados obtenidos en términos de éxito técnico y éxito clínico así como complicaciones Resultados: En este estudio se encontraron 60 pacientes llevados a un total de 83 colangioscopias por SpyGlass de estos 54 pacientes (90%) eran adultos y (10%) niños, con una edad media de 51 años [rango entre 1-85 años], la mayoría de los pacientes eran de sexo femenino 33 (54.1%). La indicación mas frecuente fue estenosis indeterminada 27.9%, seguida de estenosis postquirúrgica 23% y calculo difícil 21.3%, éxito técnico se obtuvo en el 100% y el éxito clínico 90% de toda la población. Las complicaciones en nuestra cohorte se presentaron en el 11.6%(n=7) 6 por colangitis y 1 paciente con hemobilia, todos los episodios fueron leves.
Introduction: Endoscopic retrograde cholangiopancreotography (ERCP) has traditionally been the gold standard for diagnosing and treating biliary diseases. However, this may fail in 5 to 20% of patients, even in referral centers (1). Therefore, cholangioscopy has been introduced over the last few decades as an intervention strategy for difficult bile duct cases which would otherwise require a surgical intervention with a consequently greater risk for the patient due to the inherent complications of this procedure. The objective of this study was to describe the demographic, clinical and paraclinical characteristics and outcomes of individuals who underwent SpyGlass cholangioscopy at the Fundación Cardioinfantil between February 2019 and February 2021. Methods: A retrospective, descriptive study of consecutive patients of all ages who underwent SpyGlass cholangioscopy for various reasons between February 2019 and February 2021. Their demographic and procedural characteristics, outcomes achieved in terms of technical and clinical success, and complications were evaluated. Results: This study found 60 patients who underwent a total of 83 SpyGlass cholangioscopies. Of these, 54 patients (90%) were adults and 10% were children, with a mean age of 51 years [range from 1-85 years]. Most of the patients were females (33; 54.1%). The most common indication was indeterminate stenosis (27.9%), followed by postoperative stenosis (23%) and difficult bile duct stones (21.3%). Technical success was achieved in 100% and clinical success in 90% of the total population. In our cohort, 11.6% of the patients had complications (n=7): six patients had cholangitis and one had hemobilia; all episodes were mild.

La hemorragia de vías digestivas altas (HVDA) es una emergencia frecuente. La etiología más común es la úlcera péptica. La restauración del volumen intravascular y la presión arterial son la prioridad del manejo. La endoscopia (EGD) debe realizarse luego de la reanimación inicial y lograda la estabilización hemodinámica, tratar la lesión subyacente mediante hemostasia. Se realiza un estudio en un hospital de referencia de tercer nivel, Cundinamarca, Colombia
Materiales y métodos: Estudio descriptivo retrospectivo, datos obtenidos de la historia clínica electrónica de pacientes adultos que consultaron por urgencias en el Hospital Universitario de la Samaritana (HUS) por HVDA, diagnosticada por hematemesis, melenas, hematoquezia y/o anemia; a quienes se les realizó EGD durante abril del 2010 - abril del 2011.
Resultados: Se atendieron 385 pacientes en el período de estudio, 100 fueron excluidos debido a hemorragia secundaria a várices esofágicas, historia incompleta y hemorragia digestiva baja, para un total de 285 pacientes analizados. El 69.1% eran mayores de 60 años. El 73.3% presentaba hipertensión arterial, el 55.1% reportaba ingesta de Anti-inflamatorios no esteroideos (AINES) y Aspirina (ASA). El 19.6% reportaba episodios de sangrado previos y 17.9% presentó inestabilidad hemodinámica. Sesenta y tres pacientes (22,1%) requirieron hemostasia endoscópica, treinta y dos (11.2%) presentaron resangrado. La mortalidad reportada fue del 13.1%, del cual 55.3% correspondía a hombres.
Discusión: La mayoría de la población atendida en el HUS por HVDA son adultos mayores de 60 años. La úlcera péptica, continúa siendo el diagnóstico más frecuente asociado al uso de AINES y ASA.
Upper GI bleeding is one of the most common emergencies in the emergency department. The most common etiology is peptic ulcer disease. Restoring intravascular volume and stabilization of blood pressure are the priorities in its management. Endoscopy should be performed after initial resuscitation and hemodynamic stabilization is achieved, and treat the underlying lesion with various techniques for hemostasis. We present a study conducted in a third level referral hospital, in Cundinamarca, ColombiaMaterials and methods: We retrospectively reviewed electronic medical records of all adults patients who presented to the emergency department of Hospital Universitario de la Samaritana (HUS) with upper gastrointestinal bleeding symptoms such as hematemesis, melena, hematochezia and/or anemia; all of whom underwent esophagogastroduodenoscopy between April 2010 -2011. Results: 385 patients were evaluated during the study period, 100 of them were excluded for variceal bleeding, incomplete medical history or lower intestinal bleeding; a total of 285 patients were included. 69. 1% were older than 60 years, 73. 3% had a history of arterial hypertension, 55. 1% chronically used NSAID´s or ASA. 19. 6% had previously experienced at least one episode of intestinal bleeding and 17. 9% of all patients presented hemodynamic instability. Sixty three patients required endoscopic hemostasis and 32 of them showed bleeding recurrence. The mortality for this group was 13. 1% and 55. 3% were men. Discussion. The majority of patients treated at our institution for gastrointestinal bleeding are older than 60 years. Peptic ulcer disease remains the most frequent cause and is strongly associated to NSAID´s or ASA use

El síndrome de Budd-Chiari se define como la obstrucción del flujo venoso hepático. En Colombia hay limitada evidencia respecto a la caracterización de estos pacientes. Este trabajo busca caracterizar la clínica, manejo y sobrevida de estos pacientes en un centro de referencia de hepatología colombiano. Objetivo: Describir las características clínicas, manejo y sobrevida de pacientes con diagnóstico de síndrome de Budd-Chiari en un centro colombiano desde el 2010 al 2021. Resultados: Se incluyeron 31 pacientes con diagnóstico de síndrome de Budd-Chiari. El 58,1% (n=18) fueron mujeres. La mediana de edad fue 27 años [Rango intercuartil (RIC) 23-37]. El principal factor de riesgo fueron las trombofilias adquiridas (48,4%, n=15), principalmente secundarias al síndrome antifosfolípido (73,3%, n=22). El 87,1% (n=27) presentaron ascitis como manifestación clínica principal. Al momento del diagnóstico 48,4% (n=15) se encontraban en fase cirrótica. La principal localización de la obstrucción del flujo fueron las venas hepáticas (73,3%, n=22). El score de Rotterdam fue de clase II (pronóstico intermedio) en el 48,3% (n=14). El 80,6% (n=25) se encontraban anticoagulados. 19,4% (n=6) de los pacientes fueron llevados a derivación portosistémica intrahepática transyugular (TIPS). 8 pacientes murieron (25,8%), con una mediana de sobrevida desde el diagnóstico hasta la muerte de 337,1 días (RIC 46,5-647,5). Conclusiones: El síndrome de Budd-Chiari es una enfermedad pobremente caracterizada en la población colombiana. Este estudio muestra que el principal factor de riesgo asociado a esta condición médica son las trombofilias adquiridas, con clínica y mortalidad similar a la reportada en la literatura.
Budd-Chiari syndrome is defined as the obstruction of the hepatic venous flow. In Colombia, there is limited evidence regarding the characterization of these patients. This study aims to describe the clinical features, management, and survival of these patients in a Colombian hepatology reference center. Objective: To describe the clinical features, management, and survival of patients diagnosed with Budd-Chiari Syndrome at a Colombian Hospital from 2010 to 2021. Results: A total of 31 patients diagnosed with Budd-Chiari syndrome were included. 58.1% (n=18) were women. The median age was 27 years [interquartile range (IQR) 23-27]. Ascites was the main clinical manifestation (87.1%, n=27). At the time diagnosis was made 48.4% (n=15) were cirrhotic. Acquired thrombophilia was the main prothrombotic risk factor (48.4%, n=15), with antiphospholipid syndrome as the most frequent cause (73.3%). The principal location of the outflow obstruction was in the hepatic veins (73.3%, n=22). 48.3% (n=14) had a Class II Rotterdam score (intermediate prognosis). 80.6% (n=25) were on anticoagulation. A transjugular intrahepatic portosystemic shunt (TIPS) was placed in 6 patients (19.4%), and 5 patients received liver transplants (16.1%). 25.8% (n=8) died. The median time from diagnosis to death was 337.1 days [interquartile range (IQR) 46.5-647.5] Conclusions: Budd-Chiari syndrome is a rare disease poorly described in Colombia. This study shows that this population has similar risk factors, clinical features, and mortality as described in other cohorts.

Introducción: El trasplante hepático es el tratamiento de elección para pacientes con enfermedades hepáticas en estadio terminal de cualquier etiología. El éxito de la intervención depende de la adecuada selección del donante y del receptor. Los determinantes bio-psicosociales influyen en la tasa de complicaciones y mortalidad post trasplante. Objetivo: Identificar intervenciones y desenlaces clínicos en pacientes excluidos de evaluación pre-trasplante por psicología y trabajo social en pacientes con indicación clínica de trasplante hepático entre enero de 2019 y diciembre del 2021 en un centro de referencia de trasplante hepático en Bogotá. Métodos: Estudio de corte transversal en pacientes mayores de edad con indicación clínica de trasplante hepático que no fueron aptos para evaluación pre-trasplante durante valoración de trabajo social y psicología entre enero de 2019 y diciembre de 2021. Resultados: 122 pacientes valorados por psicología y trabajo social que no fueron aptos para evaluación pre-trasplante fueron incluidos. 58,2% (n=71) fueron hombres, 77% (n=94) pertenecían al régimen contributivo, 38,5%, (n=47) tenían primaria como nivel educativo, 34,4% (n=42) eran desempleados, y la mediana de ingresos mensuales era de $1’000.000 COP (RIC $800.000-$1’950.000). El 32,5% (n=37) logró ser incluido a estudio pre-trasplante luego de alguna intervención. La activación de red familiar extensa mostró diferencias en su frecuencia entre el grupo de incluidos y de no incluidos a estudio pre-trasplante (48,6% vs 0,0%). Conclusión: Las intervenciones realizadas por el grupo de soporte multidisciplinario de trasplante permiten aumentar el acceso a la evaluación pre-trasplante, ingreso a lista de espera y trasplante a pacientes inicialmente no aptos por diferentes causales modificables con estas herramientas.
INTRODUCTION: Liver transplantation is the best treatment option for patients with end-stage liver disease of any etiology. The success of the clinical intervention depends on the proper selection of the donor and the recipient. Biopsychosocial determinants influence the rate of post-transplant complications and mortality. OBJECTIVE: To identify interventions and clinical outcomes in patients excluded from pre- liver transplant evaluation with a clinical indication for liver transplantation between January 2019 and December 2021 in a single-center experience in Bogotá. METHODS: A cross-sectional study of patients >18 years old with a clinical indication for liver transplantation that were not suitable during the social work and psychology assessment for pre-liver transplant evaluation between January 2019 to December 2021. RESULTS: Between January 2019 to December 2022, 565 patients were considered candidates for pre-liver transplant evaluation. Of these 122 patients were included in our study because they were excluded from evaluation by psychology and social work. 58.2% (n=71) were men, 77% (n=94) belonged to the private health system, 38.5% (n=47) had primary education level, 34.4% (n=42) were unemployed, and the median monthly income was $250 USD (IQR 200 - 487 USD). 32.5% (n=37) become included in the pre-liver transplant study after some intervention. The activation of the extended family network showed a statistically significant difference in its frequency between the groups included and those not included in the pre-transplant study (p=0.011). CONCLUSION: Interventions by the multidisciplinary liver transplantation support group allow access to pre-transplant evaluation, admission to the waiting list, and transplantation to patients initially excluded for different reasons that can be modified with these tools.

Los pólipos del colon son un importante factor de riesgo para el desarrollo de neoplasia, su identificación precoz, remoción y clasificación histológica es fundamental para determinar curación y sobrevida. OBJETIVO: Describir las características de los pólipos del colon resecados en un hospital de tercer nivel de Cundinamarca. METODOS: Estudio descriptivo retrospectivo tipo serie de casos, de pacientes mayores de 18 años sometidos a colonoscopía en quienes se resecaron pólipos de colon, durante el periodo 2009 a 2011. RESULTADOS: Durante el periodo de estudio de realizaron 3267 colonoscopías, se excluyeron los pacientes que no tenían pólipos, sin reporte de patología o en quienes no se recupero la pieza para histología. Se analizaron 381 pacientes con pólipos los cuales fueron incluidos en el análisis final. La localización principal de los pólipos fueron recto y sigmoide 66%, poliposis múltiple se identifico en el 2,9%. Los pólipos hiperplásicos fueron los mas frecuentemente diagnosticado, seguido por adenomas tubulares. En los pacientes que se le realizó resección de múltiples pólipos se diagnosticó cáncer en 2. Las indicaciones de realización de colonoscopía que no están claramente aprobadas pero en las cuales se identificaron y resecaron pólipos son mas del 30%. CONCLUSIONES: La mayoría de pacientes eran mujeres, sigue siendo el sangrado digestivo la principal indicación de realización de colonoscopía. Los pólipos hiperplásicos seguido de los adenomas son el principal hallazgo de patología.
RATIONALE Colon polyps are an important risks factor for the development of neoplasia, early identification, removal and histologic classification is essential to detect the cure and survival.
OBJETIVES Describe resected colon polyps characteristics in patients in a third level hospital of Cundinamarca. METHODS: Retrospective and descriptive study type number of cases, which contained patients over 18 years who had resected polyps and who were submitted to a colonoscopy during the period 2009 to 2011.
MAIN RESULTS During the study period 3267 colonoscopy procedures were performed. Patients who hadn’t polyps or pathological reports and patients who hadn’t the piece for the histology were excluded. 381 patients with polyps were analyzed, who were included on the final analysis. The main location of the polyps was: rectum and sigmoid that represents 66%. Multiple polyposis were identified in 2,9%. The hyperplasic polyps were the most diagnosed followed by tubular adenomas. The study showed that 2 of the patients undergoing to the multiple resection had cancer. The indications to practice the colonoscopy which aren’t clearly approved but which have been identified and resected the polyps are more than 30%.
CONCLUSIONS Most of patients were women. Rectal bleeding still being the main indication to perform a colonoscopy. The hyperplasic polyps followed by the adenomas are the main pathology discover.

Objetivo: Brindar una guía de práctica clínica basada con la evidencia más reciente para el diagnóstico y tratamiento de la Enfermedad Hepática Grasa No alcohólica teniendo en cuenta la efectividad y seguridad de las intervenciones dirigidas a pacientes, personal asistencial, administrativo y entes gubernamentales de cualquier servicio de atención en Colombia.
Materiales y métodos: Esta guía fue desarrollada por un equipo multidisciplinario con apoyo de la Asociación Colombiana de Gastroenterología, el Grupo Cochrane ITS y el Instituto de Investigaciones Clínicas de la Universidad Nacional de Colombia. Se desarrollaron preguntas clínicas relevantes y se realizó la búsqueda de guías nacionales e internacionales en bases de datos especializadas. Las guías existentes fueron evaluadas en términos de calidad y aplicabilidad. Una guía cumplió los criterios de adaptación, por lo que se decidió adaptar 3 preguntas clínicas. El Grupo Cochrane realizó la búsqueda sistemática de la literatura. Las tablas de evidencia y recomendaciones fueron realizadas con base a la metodología GRADE. Las recomendaciones de la guía fueron socializadas en una reunión de expertos con entes gubernamentales y pacientes.
Resultados: Se desarrolló una guía de práctica clínica basada en la evidencia para el diagnóstico y tratamiento de la Enfermedad Hepática Grasa No alcohólica en Colombia
Conclusiones: El diagnóstico y manejo oportuno de la Enfermedad Hepática Grasa No alcohólica contribuirá a disminuir la carga de la enfermedad en Colombia y las enfermedades asociadas
ABSTRACTObjective: To provide a clinical practice guideline with the latest evidence for diagnosis and treatment of nonalcoholic fatty liver disease for patients, caregivers, administrative and government bodies at all levels of care in Colombia. Materials and Methods: This guide was developed by a multidisciplinary team with the support of the Colombian Association of Gastroenterology, Cochrane STI Group and Clinical Research Institute of the Universidad Nacional de Colombia. Relevant clinical questions were developed and the search for national and international guidelines in databases was performed. Existing guidelines were evaluated quality and applicability. One guidelines met the criteria for adaptation, so the group decided to adapt 3 clinical questions. Systematic literature searches were conducted. The tables of evidence and recommendations were made based on the GRADE methodology. Results: An evidence-based Clinical Practice Guidelines for the diagnosis and treatment of nonalcoholic fatty liver disease was developed for the Colombian context. Conclusions: The opportune detection and appropriate management of nonalcoholic fatty liver disease would contribute to the burden of the disease in Colombia

La coledocolitiasis es una patología que requiere una aproximación adecuada para determinar su manejo dadas las posibles complicaciones por omisión en su diagnostico o la realización de procedimientos terapéuticos. La colangiopancreatografía retrógrada endoscópica (CPRE) es una opción en su manejo, pero es un procedimiento invasivo con riesgos de morbilidad y mortalidad considerables.
El objetivo del estudio es determinar la correlación existente entre los resultados de pruebas serológicas, el diámetro hallado de la vía biliar por ecografía, así como el diagnostico radiológico de coledocolitiasis y las CPRE realizadas en el Hospital Universitario de la Samaritana (HUS) entre el 01/05/2009 y 31/08/2010.
Materiales y Método: Estudio de concordancia de pruebas diagnósticas. Donde a través de la recolección de la información a través de un cuestionario sobre identificación de pacientes con diagnóstico presuntivo de coledocolitiasis, resultados serológicos, hallazgos ecográficos de la vía biliar y el reporte de CPRE se realizo un análisis descriptivo de la población, se calcularon los valores de sensibilidad, especificidad y cocientes de probabilidades, además de determinar el grado de concordancia entre las pruebas utilizando los paquetes estadísticos Stata v. 11 (StataCorp; Tx, USA) y SPSS v. 18 (SPSS Inc.; Ill, USA)
Dada la evidencia actual ningún indicador utilizado de forma única (historia clínica, ecografía, marcadores serológicos) es capaz de determinar el diagnostico de coledocolitiasis con suficiente precisión, sin embargo en pacientes mayores cuya clínica sugiere patología biliar obstructiva, existen algunos puntos de corte que hacen parte de algoritmos en la literatura, los cuales son una guía para determinar la necesidad de CPRE y se pueden utilizar en nuestra institución
Choledocholithiasis is a disease that requires a suitable approach to determine their management given the possible complications of omission in his diagnosis or therapeutic procedures. Endoscopic retrograde cholangiopancreatography (ERCP) is an option in their management, but is an invasive procedure with risks of significant morbidity and mortality.
The aim of this study is to determine the correlation between the results of serological tests, found the diameter of the bile duct by ultrasound, and radiological diagnosis of choledocholithiasis and procedures performed in the University Hospital of the Samaritan woman (HUS) from 01 / 05/2009 and 31/08/2010.
Materials and Methods: concordance of diagnostic tests. Where through the collection of information through a questionnaire to identify patients with a presumptive diagnosis of choledocholithiasis, serological results, sonographic findings of the bile duct and reporting of ERCP was performed a descriptive analysis of the population, were calculated sensitivity, specificity and likelihood ratios, and to determine the degree of concordance between the tests using the statistical software Stata v. 11 (StataCorp, TX, USA) and SPSS v. 18 (SPSS Inc., Ill., USA) Given the current evidence is any indicator used only (medical history, ultrasound, serum markers) is able to determine the diagnosis of choledocholithiasis with sufficient precision, but in older patients whose clinical picture suggests obstructive biliary disease, there are some cut-off points are of algorithms in the literature, which are a guide to determine the need for ERCP and can be used at our institution.

Presentamos el caso de una mujer de 37 años con historia de cirrosis biliar secundaria, en
lista para trasplante hepático, con múltiples episodios de colangitis previos y usuaria de
derivación biliar externa, quien curso con cuadro de bilioptisis y mediante gammagrafía
HIDA se confirmó fistula biliopleurobronquial.
Biliopleurobronchial fistula (FBB) is an abnormal communication between the pathway biliary and bronchial tree. It is an infrequent condition, usually secondary to a local infectious process or a traumatic event. Bilioptisis is pathognomonic.
We present the case of a 37-year-old woman with a history of secondary biliary cirrhosis, in list for liver transplantation, with multiple episodes of previous cholangitis and external biliary derivation, who had a course of bilioptisis and by scintigraphy HIDA confirmed biliopleurobronchial fistula.

Introducción: La hipertensión portal se define como el aumento en el gradiente de presión porto sistémica > a 5 mmHg, es la anomalía hemodinámica asociada a complicaciones más graves en cirrosis. La hemorragia varicosa es una emergencia médica asociada con una mortalidad que a pesar del progreso reciente sigue siendo del orden del 10-20% a las 6 semanas. Diseño: Se realizó un estudio descriptivo de corte transversal.´Métodos: Se hizo un análisis univariado haciendo un análisis descriptivo de las variables numéricas continúas utilizando estadísticas de tendencia central. Se usó el test Shapiro-Wilk en grupos con n<3. Resultados: La prevalencia fue 15% de hipertensión portal en las endoscopias realizadas, la etiología 76% era sinusoidal, de estas la causa autoinmune era la más común, el tipo de varices más prevalente fue GOV 1, encontrándose signos rojos en un 44%. De los que tenían varices GOV1 el mayor porcentaje eran CHILD B 41%. El tratamiento endoscópico que más se realizo fue ligadura endoscópica.
El 10% de los pacientes presentó sangrado digestivo, 50% presento resangrado, requiriendo el 61% colocación de TIPS. Conclusión: La etiología sinusoidal varía con respecto a la epidemiologia local ya que prima las causas autoinmunes. Se observó que a mayor CHILD no se relacionaba con mayores signos de hipertensión portal. El sangrado digestivo no se controló en un 50% lo que obliga a replantear las opciones médicas, endoscópicas, derivadas del paciente y adherencia a guías para disminuir el porcentaje de resangrado.
Introduction: Portal hypertension is defined as the increase in the porto systemic pressure gradient> 5 mmHg, the hemodynamic anomaly associated with more serious complications in cirrhosis. Varicose hemorrhage is a medical emergency associated with a mortality rate that, despite recent progress, continues to be of the order of 10-20% at 6 weeks. Design: A escriptive cross-sectional study was carried out. Methods: A univariate analysis was made by making a descriptive analysis of the continuous numerical variables using central tendency statistics. The Shapiro-Wilk test was used in groups with n <3. Results: The prevalence was 15% of portal hypertension in endoscopies performed, the etiology 76% was sinusoidal, of these the autoimmune cause was the most common, the most prevalent type of varicose veins was GOV 1, with red signs being found in 44%. Of those with varicose veins GOV1, the highest percentage were CHILD B 41%. The endoscopic treatment that was most performed was endoscopic ligation. 10% of the patients presented digestive bleeding, 50% presented with rebleeding, requiring 61% placement of TIPS. Conclusion: The sinusoidal etiology varies with respect to the local epidemiology since it gives priority to autoimmune causes. It was observed that a higher CHILD was not associated with greater signs of portal hypertension. The digestive bleeding was not controlled by 50%, which forces us to reconsider the medical, endoscopic, and patient-derived options and adherence to guidelines to reduce the percentage of rebleeding.
