A 67-year-old woman, with a history of diabetes mellitus, hypertension, deep vein thrombosis in the right lower limb, was admitted to the Mastology Department at HUMM due to a sensation of a mass in the left breast that had been present for a year. On physical examination revealed a poorly defined mass in the left breast, coordinate 10.1. Ultrasound report Bi-rads 4C moderate suspicion of malignancy given by circumscribed oval nodule time 10, 1 cm measuring 24 x 17 x 15 mm and Bi-rads 0 mammography. The pathology of the initial trucut biopsy reported atypical stromal and fusocellular proliferation of difficult classification of nature and biological behavior to be defined by immunoperoxidases, which favored myofibroblastoma, extension studies were negative. Conservative surgery was performed with quadrantectomy with harpoon marking guided by ultrasound. The patient did not receive additional treatment or medication. Clinical and imaging follow-up showed no signs of tumor recurrence. CONCLUSION: Given its presentation as an unusual variant in women (2), this case provides information on the imaging and histological diagnosis and surgical treatment of a case of myofibroblastoma of the breast.

Introduction Alveolar rhabdomyosarcoma (RMS) as a primary breast tumor is a type of cancer with a high morbidity and mortality worldwide. The pathophysiology, clinical evolution and treatment of a case of alveolar rhabdomyosarcoma as a primary tumor of the breast in a patient from Hospital Universitario Mayor - Méderi, 2016 is described. Method This case report describes the picture of a patient who presented as an RMS histopathological report as a primary breast tumor. Due to its rapid progression and the stage in which the disease was found, it was decided to manage with neoadjuvant and surgical treatment, without achieving definitive control of it. A review of the world literature is carried out in the last 10 years, finding less than 40 reports of this type of pathology to date. Results Case report of a 20-year-old female patient who undergoes a diagnosis of RMS as a primary breast tumor, managed with neoadjuvant treatment and surgical management, one month after the treatment performed, she presented a recurrent lesion and management with chemotherapy was proposed (second line) and radiation therapy. Eleven months after her initial management, the patient decided to continue extra-institutional management. Discussion Making a comparison with the world literature in relation to clinical, imaging, histopathological, immunohistochemical and treatment findings, we can determine that our management is comparable to international management, however it is of great importance to promote research on this pathology to obtain a greater volume of data, which would allow to have real incidence and morbidity and mortality figures in our country. conclusion The aggressive behavior of RMS makes an early diagnosis necessary to achieve better results in this type of disease. The multidisciplinary approach, adherence to treatments and the reduction in attention times between diagnosis and specialized consultation are important factors that allow us to reduce the progression of the disease. However, greater awareness about breast health should be encouraged both in citizens and in health professionals. Research on this type of tumor will allow us to measure the prevalence, incidence and mortality of this pathology.

Backround: Breast cancer has become the first cause of cancer in women in our country, with a two-year survival of 79.6% after treatment, and one of the main consequences has been persistent postoperative pain, called postmastectomy pain syndrome (SPDM); it has been reported an incidence of 25-60% worldwide, and 55.3% in Colombia. The present study aims to calculate the incidence and evaluate associated factors of women with painful post-mastectomy syndrome that has been taken to breast cancer surgery at the University Hospital Mayor-Mederi in the period between september 2018 and march 2019. Methods: A cross-sectional study was performed, in which the totality of patients taken to an oncological surgery for breast cancer were included, at the Mederi University Hospital in Bogotá, and were evaluated for the presence of SDPM. Results: 26 patients met inclusion criteria during the study period were identified, the incidence of SDPM was 27%. The associated characteristics were normal body mass index and age> 60 years, radical mastectomy with axillary emptying and adjuvant with Taxanes, Doxorubicin and Cyclophosphamide or hormone therapy with Tamoxifen Discussion: The incidence of pain is similar to that reported in other studies in the region and the risk factors are not extrapolated to the population of our country, but it is striking that despite being importantly associated with radiotherapy, overweight and younger, these conditions were no statistically different for our population.

The clinical presentation of breast cancer is highly variable; most tumors are located in the upper outer quadrant of the breast, followed by the upper inner, and the lower outer. The internal lower quadrant and the central-mammary region are the least frequent sites as reported in the literature (1). A case of breast cancer in the inter-mammary region of a patient treated at the Mayor Méderi University Hospital (HUMM) is presented. Case report: A 63-year-old woman, with no significant history, who was admitted to the HUMM Mastology clinic due to a sensation of a mass located in the inter-mammary region, with progressive growth in the last 6 months. On physical examination, a 2 cm mass was identified in the intermammary region without skin involvement. Report of breast ultrasound and mammography BIRADS 2: Benign findings. The pathology reported hypercellular mucinous carcinoma, 100% positive estrogen receptors, 75% positive progestin receptors, HER2 negative by FISH, KI67: 20%, Luminal B. Magnetic resonance imaging shows an irregular mass with marked enhancement in the inter-mammary valley, without lesions. defined in breasts. The extension studies were negative and it was staged as cT2N0M0, Stage IIA. Conservative surgery with quadrantectomy and bilateral sentinel node was performed due to radiocolloid migration to bilateral lymph node groups after peri-tumoral application. Patient received hormonal therapy with tamoxifen and adjuvant radiotherapy. Clinical and imaging follow-up shows no signs of local relapse. Conclusion: This is the second case of intermammary localized breast cancer reported in the literature (2). Given the strange location, this report provides information as an alternative management in patients with similar characteristics

Backround: Paget's disease extramammary EPEM is an uncommon entity of the skin, with an insidious but aggressive clinical manifestations due to its great locoregional commitment and potential to produce metastasis and even death. HER2 is a biomarker that is currently under study due to its clinical implications in the diagnosis, evolution and prognosis of the disease. The present study proposes to identify the evidence related to the behavior of this biomarker in extramammary Paget's disease. Methods: A systematic review of literature with a meta-analytical component was performed in which the evidence of the behavior of HER2 in patients with EPEM was extracted, according to different articles published in the main database (Medline, Embase, Cochrane, Pubmed, Google Schoolar, Science Direct, tripdatabase) without time limit for the publications. Results: Of 147 articles found on the subject, 38 met criteria for inclusion, (692 patients in total, 51.1% male). HER2 protein was over-expressed in 42.77% of the cases and the amplification of the ERBB2 gene was positive in 11.1%. The dermal invasion occurred in 87.2% of the cases, while the regional metastasis and recurrence after treatment occurred in 76.8% and 93.9% respectively. The association between Her-2 expressoin and the EPEM was significat (p<0.01). Discussion: There is overexpression and/or amplification of HER-2 in EPEM with a target positivity percentage of 42%, therefore therapy with anti-HER2 target therapies should be considered as a possible new therapeutic strategy in selected cases with positivity for this marker.

Breast cancer is the most common type of cancer, with more than 2.2 million cases in 2020. High breast density is a risk factor for breast cancer because malignant lesions are hidden in the dense breast tissue avoiding its early detection. The use of mammography as screening allows the systematic measurement of breast density by manual and computer-assisted methods, classifying it into high and low density. Alcohol consumption and high breast density are factors that increase the risk of breast cancer (BC), but a direct relationship has not yet been established, the measurement of mammographic density was performed with the help of the computer tool (DM -Scan) in 1000 women between the ages of 50 and 69 who attended screening mammography in 2018 and 2019 at the EPS Sanitas / Clínica Colsanitas SA care centers in the city of Bogotá, logistic regression was used. to measure the association between alcohol consumption and mammographic density. Alcohol consumption was not related to higher breast density (95% CI).
