INTRODUCCION: El trasplante cardiaco es una terapia efectiva para los pacientes con falla cardiaca terminal . Al momento no hay registros Colombianos actualizados y publicados de la morbimortalidad del trasplante cardiaco en relación a los episodios de rechazo celular.
MATERIALES Y METODOS: Estudio descriptivo retrospectivo de mayores de 18 años trasplantados en la Clínica Shaio. Se realizaron cálculos de frecuencia, promedios y curvas de Kaplan Meier.
RESULTADOS: La edad promedio fue 46,7 años +/- 13.La indicaciones fueron miocardiopatía dilatada 45%, coronaria 30%, miocarditis 9%, Chagas 9%, valvular 7%. El rechazo celular en los tres primeros años: 57%, 54% y 41% que se reduce 25% entre los tres y cinco años. Las causas de muerte fueron falla del trasplante 46%, infecciones y falla cardiaca 23%, rechazo 8%.El primer año la mortalidad fue 65%, principalmente falla del injerto. La mortalidad a 30 días 29,5%, y supervivencia del 90% al año, 64% a 5 años, 48% a 10 años y 15% a 13 años. La supervivencia media fue 8 años. La vasculopatía fue 33%. La función ventricular 53,5% +/- 12,6 el primer año; 58,4 +/- 5,4% al tercer año; 51,7 +/- 11,9 % al quinto año 46% +/- 15,8 al decimo año.
DISCUSION: Se observo mayor frecuencia de rechazo celular, ocasionando mas falla cardiaca y muerte.
INTRODUCTION: Heart transplantation in an effective therapy for patients with terminal heart failure. There are currently no Colombian records updated and published in morbidity and mortality of heart transplantation in relation to episodes of cellular rejection.
MATERIALS AND METHODS: A descriptive retrospective study of patients older than 18 years transplanted in Shaio Clinic. Frequency calculations were performed, averages, and Kaplan Meier survival.
RESULTS: The average age was 46.7 years + / - 13.The indications were idiopathic cardiomyopathy with 45%, coronary 30%, myocarditis 9%, Chagas 9% valvular 7%. The cellular in the first three years : 57%, 54% 41% decrease to 25% between three and five years . The causes of death were failure of transplantation 46%, infections and heart failure 23%, reject 8 %. The first year the mortality rate was 65%, mainly the graft failure. Mortality at 30 Days 29.5% and survival was 90 % at 1 year, 64% at 5 years, 48% at 10 years and 15% at age of 13. The median survival was 8 years old. The vascular disease was 33%. Left ventricular function 53.5% +/-12.6 the first year; 58.4 +/-5.4% in the third year; 51.7 +/-11.9% the fifth year 46% +/-15.8 to the tenth year.
DISCUSSION: Note increased frequency of cellular rejection, causing more cardiac failure and death.

Introducción: La introducción de terapias percutáneas en el tratamiento de la insuficiencia mitral severa, abrió un nuevo enfoque en el manejo de la falla cardiaca. Describimos las características ecocardiográficas de los pacientes candidatos a implante de Mitraclip en la Clínica Shaio. Métodos: Estudio descriptivo, retrospectivo, en pacientes candidatos a implante de Mitraclip entre enero de 2016 y marzo de 2019. Incluidos adultos con falla cardiaca avanzada e insuficiencia mitral moderada a severa o severa sintomática no candidatos a manejo quirúrgico. Resultados: Se evaluaron 64 pacientes, 23 fueron elegibles para implante de Mitraclip, con edad promedio de 69±7.5 años, 56.5% hombres, el 17% presentaban clase funcional IV. Los hallazgos ecocardiográficos mostraron una fracción de eyección ventricular izquierda promedio del 23% (RIQ 16-28%), insuficiencia mitral severa en el 91.3% (100% funcionales), altura de tienda promedio de 12.7±2.75mm, con superficie de coaptación de 3.74±0.96 mm y longitud de valva posterior de 11.8±2.16mm. El promedio del área valvular y del anillo mitral fue de 6.45cm2 (RIQ 4.3 - 8.6 cm2) y 36mm (RIQ 30 – 40mm), respectivamente. El volumen de fin de diástole del ventrículo izquierdo fue de 112mL/m2SC (RIQ 98 – 165 ml/m2). Posterior al procedimiento, el 86.9% presentó trazas o insuficiencia mitral leve, el 8.7% moderada y 4.35% severa. Conclusiones: Los pacientes candidatos a Mitraclip en la Clínica Shaio presentan severa disfunción ventricular e insuficiencia mitral severa la mayoría de los casos, insuficiencia mitral severa desproporcionada para el grado de dilatación ventricular izquierda, similar a lo reportado en el estudio COAPT, el cual mostró buenos resultados al elegir adecuadamente los pacientes para el procedimiento. Un porcentaje del grupo de pacientes no candidatos a implante del dispositivo presentaban insuficiencia mitral primaria, por el contrario, todos los pacientes elegibles a implante de Mitraclip tenían insuficiencia mitral secundaria, siendo esto la única diferencia ecocardiográfica significativa entre ambos grupos.
Background: The introduction of percutaneous therapies in the management of severe mitral regurgitation has opened a new approach in the treatment of heart failure. We describe the echocardiographic profile of potential patient candidates for Mitraclip implantation at the Shaio Clinic. Methods: Retrospective and descriptive study from January 2016 to March 2019 that included adults with advanced heart failure and moderate to severe or severe symptomatic functional mitral regurgitation that were not deemed good surgical candidates. Results: 64 patients were evaluated, 23 were eligible for Mitraclip implantation. Out of the eligible, the average age was 69±7.5 years, 56.5% were male, 17% of them belonged to the functional class NYHA (New York Heart Association) IV, and the average left ventricular ejection fraction was 23% (RIQ 16-28%). 91.3% had severe mitral regurgitation (100% functional etiology). Average tenting height was 12.7±2.75mm with coaptation surface of 3.74±0.96 mm and posterior leaflet length of 11.8±2.16mm. Average valve area and mitral annulus were 6.45 cm2 (RIQ 4.3 - 8.6 cm2) and 36mm (RIQ 30–40mm) respectively. Left ventricle end diastolic volume was 112/ml/sc (RIQ 98–165ml/m2). After procedure, 86.96% of the patients showed residual mild mitral regurgitation, 8.7% exhibited moderate and 4.35% severe. Conclusion: Patients candidates to Mitraclip implantation at the Shaio Clinic had severe left ventricle dysfunction and severe mitral regurgitation in most cases, with “disproportionate” mitral regurgitation according to the severity of left ventricular dilatation, similar to what was reported in the COAPT study, which showed good results when properly choosing patients for the procedure. A percentage of the group of patients not candidates for implantation of the device presented primary mitral regurgitation; on the other hand, all patients eligible for Mitraclip implantation presented secondary mitral regurgitation, this being the only significant echocardiographic difference between the two patient groups.

El objetivo de nuestro estudio fue evaluar la relación entre las mediciones ecocardiográficas del aparato valvular tricuspídeo según el tipo de procedimiento (ecocardiografía transtorácica y ecocardiografía transesofágica tridimensional) en un grupo de pacientes remitidos al Laboratorio de Ecocardiografía de un hospital de alto nivel de complejidad en Bogotá. Se analizó la correlación de las mediciones por ambas técnicas y se ajustaron por las variables de confusión. Se incluyeron 107 participantes. Tanto las mediciones repetidas por el mismo evaluador, como las mediciones entre las dos técnicas ecocardiográficas mostraron correlación utilizando el estadístico de Pearson (p<0,001). El diámetro de la valva posterior fue la medida con mayor correlación entre las dos técnicas (correlación de Pearson: 0,974; IC 95%:0,956-0,986). No se encontraron diferencias entre la población con antecedentes patológicos y quienes no tenían ningún antecedente. Las mediciones del aparato valvular tricuspídeo realizadas con ecocardiografía transtorácica tridimensional se correlacionan con las mediciones realizadas con ecocardiografía transesofágica tridimensional en la población estudiada.
The objective of our study was to evaluate the relationship between echocardiographic measurements of the tricuspid valve apparatus according to the type of procedure (transthoracic echocardiography and three-dimensional transesophageal echocardiography) in a group of patients referred to the Echocardiography Laboratory of a high complexity hospital in Bogotá . The correlation of the measurements by both techniques was analyzed and adjusted for the confounding variables. 107 participants were included. Both the measurements repeated by the same evaluator and the measurements between the two echocardiographic techniques showed correlation using the Pearson statistic (p <0.001). The diameter of the posterior leaflet was the measure with the highest correlation between the two techniques (Pearson's correlation: 0.974; 95% CI: 0.956-0.986). No differences were found between the population with a pathological history and those without no background The measurements of the tricuspid valve apparatus performed with three-dimensional transthoracic echocardiography correlate with the measurements made with three-dimensional transesophageal echocardiography in the population studied.

Introducción: La evaluación de la función miocárdica global y regional juega una papel crítico en el diagnóstico y manejo de los pacientes con enfermedad coronaria con importantes implicaciones pronosticas, las nuevas técnicas ecocardiográficas como la evaluación del STRAIN han sido validadas como una herramienta objetiva, comprehensiva y precisa para evaluar dichos parámetros.
Objetivo: Determinar la capacidad del strain global longitudinal para la detección de estenosis coronaria significativa, número de territorios comprometidos y territorio anatómico del vaso culpable; en pacientes sin antecedentes de enfermedad coronaria previa con infarto agudo del miocardio.
Diseño: estudio de pruebas diagnósticas retrospectivo en el que se utilizó como gold estándar la angiografía coronaria, se seleccionaron 64 pacientes con ecocardiograma transtorácico previo a la angiografía coronaria.
Resultados: Se demostró una exactitud intermedia del strain global longitudinal para detectar estenosis coronaria por análisis de curvas ROC, con un área bajo la curva de 0,78 p= 0,000 (IC 0,6; 1,0), Una sensibilidad de 96.5% (91.7%, 101.3%), especificidad 40.0% (9.6%, 70.4%) y una prevalencia real del enfermedad coronaria de 85.1% (76.5%, 93.6%)
Conclusiones: La medición de la función global y regional por medio del strain global longitudinal identifica pacientes con infarto agudo del miocardio que tienen estenosis coronaria significativa, número de territorios afectados, y la distribución anatómica de los posibles vasos culpables, sin embargo hay que tener precaución en su uso que sólo se limite a escenarios en donde pueda ser interpretado adecuadamente.

Introduction : Evaluation of global and regional myocardial function Playing a critical role in the diagnosis and management of patients with coronary artery disease with important prognostic implications , new echocardiographic techniques such as assessment and GLOBAL STRAIN have been validated as a tool objective , comprehensive and accurate to evaluate these parameters.
Objective: Determine the capacity of global longitudinal strain for the detection of significant coronary stenosis, number of involved territories and anatomical territory of the culprit vessel; in patients with no history of coronary disease with acute myocardial infarction.
Design: Retrospective study of diagnostic tests in which was used as gold standard coronary angiography, 64 patients were selected and took an echocardiography previous to coronary angiography.
Results: an intermediate accuracy of global longitudinal strain was demonstrated to detect coronary stenosis by ROC curve analysis , with an area under the curve of 0.78 p = 0.000 (CI 0.6 , 1.0 ), a sensitivity of 96.5 % ( 91.7 % , 101.3 % ) , Specificity 40.0% ( 9.6 % , 70.4% ) and an actual prevalence of coronary artery disease of 85.1 % ( 76.5 % , 93.6 % )
Conclusions: The measurement of global and regional function by means of global longitudinal strain identifies patients with acute myocardial infarction who have significant coronary stenosis also discriminates the number of affected territories, and the anatomical distribution of possible vessels guilty, however should be careful in his use, using it only limited scenarios where it can be interpreted properly

RESUMEN
Introducción
El papel de las nuevas técnicas ecocardiográficas para el diagnóstico de infarto agudo del miocardio se encuentra en desarrollo y la realización de mecánica ventricular izquierda podría sugerir la presencia de enfermedad coronaria hemodinámicamente significativa.
Objetivos
Determinar si en pacientes con infarto agudo del miocardio la medición de strain longitudinal global y regional sirve para predecir la presencia de enfermedad coronaria significativa.
Métodos
Es un estudio de pruebas diagnósticas en el que se evaluaron las características operativas de la mecánica ventricular izquierda para la detección de enfermedad coronaria significativa comparado contra el cateterismo cardiaco, considerado el patrón de oro. Se analizaron 54 pacientes con infarto agudo del miocardio llevados a cateterismo cardiaco, a quienes se les realizó un ecocardiograma transtorácico con medición de strain longitudinal global y regional.
Resultados
De los 54 pacientes analizados, el 83% tenía enfermedad coronaria significativa. El hallazgo de un strain longitudinal global < -17.5 tuvo una sensibilidad del 85% y una especificidad del 78% para predecir la presencia de enfermedad coronaria; para la arteria descendente anterior un strain longitudinal regional < – 17.4 tuvo una sensibilidad de 82% y una especificidad de 44%, para la arteria circunfleja una sensibilidad del 87% y una especificidad del 37% y para la arteria coronaria derecha una sensibilidad de 73% y una especificidad de 32%.
Conclusiones
La realización de ecocardiografía con mecánica ventricular en pacientes con infarto agudo del miocardio es útil para predecir la presencia de enfermedad coronaria hemodinámicamente significativa.
SUMARY
Introduction
The role of the new ecocardiographic techniques in patients with acute myocardial infarction is under development, and the measurement of left ventricular mechanics could predict the presence of significant coronary artery disease.
Objectives
To determine if the measure of global longitudinal and regional strain is useful to predict the presence of hemodinamically significant coronary artery disease.
Methods
It is a diagnostic test study, aimed to determine the operative characteristics of left ventricular mechanics compared to cardiac catheterism, which was considered the gold standard. 54 patients with a diagnosis of acute myocardial infarction who underwent cardiac catheterization were analyzed; transthoracic echocardiography was performed with measurement of global and regional longitudinal strains.
Results
Of 54 patients studied, 83% had hemodynamically significant coronary artery disease. The finding of a global longitudinal strain <-17.5 had a sensitivity of 85% and a specificity of 78% for predicting the presence of coronary artery disease; the left anterior descending artery regional longitudinal strain <- 17.4 had a sensitivity of 82% and a specificity of 44%; the circumflex artery strain had a sensitivity of 87% and a specificity of 37%, and for the right coronary artery a sensitivity of 73% and a specificity of 32% was found.
Conclusions
Echocardiography with measurement of ventricular mechanics in patients with acute myocardial infarction is useful for predicting the presence of hemodynamically significant coronary disease.

Primary cardiac tumors are very rare. The reported prevalence ranges from 0.001% to 0.03%. Cardiac paragangliomas are among the most infrequent tumors, making up <1% of all primary cardiac tumors. They are chromaffin cell tumors that produce and secrete catecholamines and are located in the adrenal medulla in 90% of cases and in the mediastinum in <2%.

Endomyocardial fibrosis (EMF) is a condition that was first reported in 1948 in Africa, and its geographical distribution is strongly related to socioeconomic, cultural, and environmental status. More than half of the cases are reported in countries in sub-Saharan Africa; however, this condition has been described in various parts of the world. The associated factors have been previously described and include malnutrition, parasitic infection, noninfectious systemic diseases, and genetic conditions.
The presence of fibrotic thickening of the endocardium secondary to inflammation generates functional alterations, which are detected by noninvasive methods such as echocardiography, with characteristic findings of a restrictive syndrome and an alteration of the ventricular chamber in the presence of dilated atria. We describe the case of a patient with decompensated heart failure and imaging findings consistent with EMF.

El mesotelioma pericárdico primario es tumor maligno poco común, de etiología desconocida, y con diagnostico en etapas avanzadas de la enfermedad. Esto se debe a que clínicamente debuta con signos y síntomas poco específicos para esta patología, todos ellos secundarios a un pericardio engrosado y poco complaciente que progresivamente limita el llenado ventricular produciendo falla cardiaca derecha, disfunción diastólica, disnea, y un cuadro de pericarditis constrictiva, entre otros. Los avanzados estados de la enfermedad en el momento del diagnóstico le confieren alta morbilidad y mortalidad, mala respuesta a las intervenciones medico quirúrgicas, y pronostico malo a corto plazo. Por medio de este reporte de caso, buscamos aumentar la literatura disponible, y con el tiempo, la sensibilidad del personal de la salud a este tipo de patologías poco frecuentes, teniendo como objetivo aportar con nuestra experiencia, y proporcionar herramientas al médico, quien, apoyado por el enfoque multimodal de imágenes, puede lograr un diagnóstico temprano, que brinde a futuro, la oportunidad de mejorar la calidad de vida y eventualmente cambiar el curso de la enfermedad, su deterioro su pronóstico y la sobrevida de este grupo de pacientes.
Primary pericardial mesothelioma is a rare malignant tumor, of unknown etiology, and detected in advanced stages of the disease. This is due to the fact that clinically it debuts with signs and symptoms that are not specific for this pathology, all of them result from a thickened and uncomfortable pericardium that progressively limits ventricular filling producing right heart failure, diastolic dysfunction, dyspnea, and a picture of constrictive pericarditis, among others. The advanced stages of the disease at the time of diagnosis give it high morbidity and mortality, poor response to medical surgical interventions, and poor short-term prognosis. Through this case report, we seek to increase the available literature, and over time, the sensitivity of health personnel to this type of rare diseases, with the objective of contributing with our experience, and providing tools to the doctor, who, Supported by the multimodal image approach, you can achieve an early diagnosis, which provides in the future, the opportunity to improve the quality of life and eventually change the course of the disease, its deterioration, its prognosis and the survival of this group of patients

Introducción. La auriculilla izquierda es una estructura cardiaca que facilita la generación de trombos en su interior, favoreciendo la aparición de evento embolicos, por lo que su análisis a partir de imágenes bidimensionales y mas recientemente tridimensionales, adquieren cada vez mayor importancia
Objetivo.
Comparar los hallazgos anatómicos de la auriculilla izquierda obtenidos a través de la ecocardiografía tridimensional con respecto a los obtenidos por ecocardiografía bidimensional en un grupo de pacientes con ritmo sinusal y con fibrilación auricular.
Métodos. Se trata de un estudio observacional analítico, transversal, en el que se compararan los resultados en las mediciones anatómicas obtenidas por ecocardiograma bidimensional en pacientes con rimo sinusal y fibrilación auricular, con los resultados de dichas mediciones obtenidas a través del ecocardiograma tridimensional en el mismo grupo de pacientes.
Resultados. Se evaluaron 48 pacientes, 32 pacientes (66%) se encontraron en ritmo sinusal, la edad promedio fue de 58,2 años; 41,7% fueron mujeres y la mayoría, 32 pacientes (66,7%), tenían una o varias comorbilidades de importancia de riesgo cardiovascular, con evidencia de compromiso de la función sistólica en 20 paciente, encontrando una mayor homogeneidad en las variables área y profundidad de la auriculilla izquierda.
Discusión. Los resultados nos permiten apoyar el concepto que las imágenes obtenidas por ecocardiografía tridimensional nos ofrecen una mejor evaluación de la auriculilla izquierda, observando una mayor homogeneidad con la ecocardiografía bidimensional en las variables área y profundidad, existiendo a su vez heterogeneidad en la variable longitud.
Conclusión. El presente estudio demostró que la ecocardiografía tridimensional, es un aporte importante desde el punto de vista diagnostico tanto cualitativo como cuantitativo en el análisis de la auriculilla izquierda, permitiendo una fácil adquisición de imágenes en tiempo real y comparativas con las imágenes bidimensionales.
Introduction The left atrial appendage is a heart structure that facilitates the generation of thrombus within it, which favors the appearance of embolic events. Therefore, its analysis from bidimensional and more recent three-dimensional images acquire more relevance every day.
Objective To compare the anatomical findings of the left atrial appendage obtained through three-dimensional echocardiography with the findings achieved by echocardiography bi-dimensional in a group of patients with sinus rhythm and auricular fibrillation.
Method This is a transversal, analytical and observational study that compares the results in the anatomical measurements obtained by two-dimensional echocardiography in patients with auricular fibrillation and sinus rhythm with the results of such measurements obtained through three-dimensional echocardiography with the same group of patients.
Results 48 patients were evaluated in total of which 32 patients of the group (66%) were evaluated in sinus rhythm. The average age was 58.2 years old. The 41.7% of the group was women (the majority). Besides, 32 patients (66.7%) had one or several ccomorbilities with major cardiovascular risk and evidence of compromise of systolic function in 20 patients. A great homogeneity in the variables area and depth of left atrial appendage was found.
Discussion The results triggered in this study help us to support the concept that three-dimensional images offers a more evaluation of left atrial appendage, obtain homogeneity in the variables area and depth can be comparable to two-dimensional images. Also, there existed heterogeneity in the length variable.
Conclusion This research showed that three-dimensional echocardiography is an important contribution from the diagnostic perspective in both qualitative and quantitative analysis of the left atrial appendage, which permits an easy acquisition of images in real time and comparable with two-dimensional images.

Se realizó un estudio transversal, se incluyeron 3 residentes no cardiólogos y se les dio formación básica en ecocardiografía (horas teóricas 22, horas prácticas 65), con recomendaciones de la Sociedad Americana de Ecocardiografia y aportes del aprendizaje basado en problemas, con el desarrollo de competencia técnicas y diagnósticas necesarias, se realizó el análisis de concordancia entre residentes y ecocardiografistas expertos, se recolectaron 122 pacientes hospitalizados que cumplieran con los criterios de inclusión y exclusión, se les realizo un ecocardiograma convencional por el experto y una valoración ecocardiográfica por el residente, se evaluó la ventana acústica, contractilidad, función del ventrículo izquierdo y derrame pericárdico. La hipótesis planteada fue obtener una concordancia moderada.
Resultados: Se analizó la concordancia entre observadores para la contractilidad miocárdica (Kappa: 0,57 p=0,000), función sistólica del ventrículo izquierdo (Kappa 0,54 p=0.000) siendo esta moderada por estar entre 0,40 – 0,60 y con una alta significancia estadística, para la calidad de la ventana acústica (Kappa: 0,22 p= 0.000) y presencia de derrame pericárdico (Kappa: 0,26 p= 0.000) se encontró una escasa concordancia ubicándose entre 0,20 – 0,40. Se estableció una sensibilidad de 90%, especificidad de 67%, un valor predictivo positivo de 80% y un valor predictivo negativo de 85% para el diagnóstico de disfunción sistólica del ventrículo izquierdo realizado por los residentes.
A cross-sectional study, three residents not included cardiologists and were given basic training in echocardiography (theoretical hours 22 hours 65 practices), with recommendations of the American Society of Echocardiography and contributions of problem-based learning, with the development of technical competence and necessary diagnostic, analysis of agreement between residents and experts echocardiography was performed, 122 hospitalized patients who met the inclusion and exclusion criteria were collected, they were performed by conventional echocardiography and echocardiographic assessment expert by the resident, the acoustic window, contractility and left ventricular function, pericardial effusion was assessed. The hypothesis was to obtain a moderate agreement.
Results: The inter observer agreement for myocardial contractility (Kappa: 0.57 p = 0.000) was analyzed left ventricular systolic function (Kappa 0.54 p = 0.000) where this moderate to be between 0.40 to 0.60 and with high statistical significance for the quality of the acoustic window (Kappa: 0.22 p = 0.000) and presence of pericardial effusion (Kappa: 0.26 p = 0.000) was found poor concordance was reaching between 0.20 - 0.40. A sensitivity of 90%, specificity 67%, positive predictive value of 80% and a negative predictive value of 85% for the diagnosis of left ventricular systolic dysfunction performed by residents was established.

Antecedentes: Las guías de tratamiento médico de la Sociedad Europea de Cardiología (ESC) para el tratamiento de los síndromes coronarios agudos en pacientes que se presentan sin elevación persistente del segmento ST (NSTEMI) recomiendan vigilancia en cuidados intensivos (UCI) durante las primeras 24-48 h del evento. Sin embargo, la mortalidad hospitalaria de estos pacientes ha venido en descenso constante, permitiendo a algunos pacientes la opción de ser tratados en las salas generales del hospital. A continuación proponemos la puntuación ACTION ICU para identificar a los pacientes de alto riesgo con NSTEMI y guiar la necesidad selectiva de cuidados en la UCI basada en el riesgo. Objetivo: Evaluar la utilidad del puntaje ACTION ICU para predecir el riesgo de los pacientes de desarrollar complicaciones que requieren atención en la UCI en una cohorte latinoamericana con NSTEMI. Métodos: Se aplicó la puntuación ACTION ICU en una cohorte retrospectiva. La presencia de complicaciones con requerimiento de UCI se definió como el desarrollo agudo de uno o más de los siguientes: paro cardiorrespiratorio, shock, bloqueo auriculoventricular de alto grado, insuficiencia respiratoria, accidente cerebrovascular o muerte. El rendimiento predictivo de este modelo se estimó con un análisis de regresión logística multivariable condicional. Resultados: De 1.062 pacientes con NSTEMI, el desenlace primario estuvo presente en 75 pacientes (7,1%). Del total de la población, 1.019 (96%) ingresaron en UCI. El evento más frecuente fue la insuficiencia respiratoria (4,0%), seguida de falla cardiaca, shock (3,7%) y paro cardíaco (1,7%). La presencia de signos o síntomas de insuficiencia cardíaca tuvo tuvo la mayor asociación el desarrollo de complicaciones (OR: 2,16; IC 95%: 1,61 2,92). El mejor punto de corte para esta población fue 3 (riesgo de complicaciones: 4,0%, SEN:96%, SP:15,4%, VPN:98,1%, VPP:7,9%). Conclusión: El puntaje ACTION ICU puede ser una herramienta prometedora para identificar la necesidad de atención en UCI en pacientes Latinoamericanos con NSTEMI. En necesario continuar con estudios que permitan evaluar la rentabilidad de esta estrategia.
Background: European Society of Cardiology (ESC) guidelines for the management of acute coronary syndromes in patients presenting without persistent ST-segment elevation (NSTEMI) recommend Intensive Care Unit (ICU) surveillance during the first 24 48 h. Interestingly, the in-hospital mortality of NSTEMI patients has consistently decreased, giving some patients the option to be managed in general hospital wards. The ACTION ICU score has been proposed to identify high-risk patients with NSTEMI and guide the selective risk-based need for ICU care. Objective: To evaluate the usefulness of the ACTION ICU score to predict patients’ risk of developing complications requiring ICU care in a Latin-American cohort with NSTEMI. Methods: We applied the ACTION ICU score in a retrospective cohort. A composite primary outcome included: cardiorespiratory arrest, shock, high-grade atrio-ventricular block, respiratory failure, stroke, or death. The predictive performance of this model was estimated with a conditional multivariable logistic regression analysis. Results: Of 1,062 patients with NSTEMI, the primary outcome was present in 75 patients (7.1%), and 1,019 (96%) were admitted to ICU. The most common event was respiratory failure (4.0%), followed by cardiogenic shock (3.7%), and cardiac arrest (1.7%). The presence of heart failure signs or symptoms had the highest association with the primary outcome (OR:2.16; 95%CI:1.61 2.92). The best cut-off point for this population was 3 (complications risk: 4.0%, SEN:96%, SP:15.4%, NPV:98.1%, PPV:7.9%). Conclusion: The ACTION ICU score may be a promising tool to identify the need for ICU care in Latin-American patients with NSTEMI. Furthermore, additional research is needed to evaluate the cost-effectiveness of this strategy.

Se presenta el caso de una paciente femenina de 54 años de edad remitida para la realización de un ecocardiograma transesofágico por sospecharse evento cerebrovascular cardioembólico secundario a fibrilación auricular, en el estudio se documentó una masa de aspecto quístico en aspecto posterior de la raíz aórtica, trabeculada, con flujo dinámico y la presencia de trombos en su interior además esclerosis valvular aórtica con insuficiencia leve sin estenosis, válvula mitral engrosada con aspecto reumático con insuficiencia leve, hipertrofia concéntrica severa del ventrículo izquierdo con hipoquinesia leve generalizada FEVI 48% y derrame pericárdico circunferencial de aprox. 80 ml sin repercusión hemodinámica, no trombos en auriculilla izquierda, se consideró aneurisma de seno de Valvalva, la paciente fue llevada a corrección quirúrgica en donde se confirma hallazgo de seno de Valsalva con flujo a través de un orificio de la comisura hacia seno de Valsalva derecho sin comunicación con la aurícula derecha, 12 días posterior a la cirugía la paciente es egresada sin complicaciones cardiovasculares para continuar con rehabilitación por secuelas del evento cerebro vascular. El aneurisma de senos de Valsalva es un hallazgo raro encontrándose entre el 0.14- 0.35% de pacientes sometidos a cirugía cardíaca con una prevalencia en la población de 0.09%. La presentación clínica puede ser desde asintomática a causar disnea, dolor torácico, simular infarto del miocardio o incluso endocarditis. La corrección quirúrgica cuando se presenta el aneurisma sin otras comorbilidades es baja con sobrevida de 94% en 10 años.
Transesophageal echocardiogram as a diagnostic tool for cardioembolic cerebrovascular event secondary to thrombus into Valsalva sinus aneurysm. This is a case report of a 54-years-old female with a history of systemic arterial hypertension treated with enalapril 20 mg and amlodipine 5 mg both once daily. She was admitted 15 days ago in a Hospital Center of Bogotá for having presented sudden loss of consciousness followed by complex focal seizure pisodes. She presented with motor and sensory aphasia and left hemiparesis, atrial fibrillation was documented in electrocardiogram and an ischemic cardiovascular event was documented in a brain computer tomography. A cardioembolic event due atrial fibrillation was considered and for that reason the patient was referred to the Shaio Clinic Foundation for a transesophageal echocardiography, finding in the aortic root to the posterior aspect a mass with cystic appearance, trabeculated, with dynamic flow and inside it the presence of thrombus, with an aortic valve sclerosis with mild insufficiency, without stenosis, The patient was remitted for a multidisciplinary management by cardiology, neurology and cardiovascular surgery. During the hospitalization, a chest angiography was performed, reporting multilobed dilatation of the non-coronary Valsalva sinus with a mobile thrombus inside. On the 22nd day of admission surgical correction of the aneurysm was performed through aortotomy, locating an orifice that feeds it below the commissure. The coronary and right coronary valves without communication with the right atrium with a Dacron patch. During the postoperative period she presented signs of low cardiac output and myoclonus the patient was managed with inotropes, levosimendan and levetiracetam, 48 hours after surgery she was extubated and 12 days after surgery she is discharged without complications.

La evaluación funcional y estructural del ventrículo derecho (VD) es un aspecto fundamental del estudio ecocardiográfico dado el valor pronóstico de la misma. Hasta el momento, las guías establecen la valoración a partir de parámetros cualitativos, semicuantitativos y cuantitativos con la indicación de reportar la disfunción de manera dicotómica: Presente o ausente, con puntos de corte de normalidad que no son absolutos a un valor ecocardiográfico, sino a la interpretación de los mismos dados por el cardiólogo. Un estudio ha demostrado que cuando se clasifica la disfunción en grados de severidad (leve, moderado, severa) el pronóstico con relación a desenlaces cardiovasculares (MACE) es diferente. Crear un sistema de evaluación a partir de variables de la Ecocardiografía-2D que permita clasificar la función del VD y que sea comparable con la resonancia magnética cardíaca (RMNc) que es el estándar de oro para la evaluación del VD mejorará la evaluación pronóstica de los pacientes. Objetivo: realizar una correlación entre los parámetros de valoración del ventrículo derecho por ecocardiografía y RMNc basados en 3 niveles de estadificación que toman la fracción de eyección del ventrículo derecho (FEVD) por RMNc (leve, moderada y severa). Métodos: Estudio de cohorte histórica de pacientes llevados a ecocardiografía-2D y RMNc durante el periodo 2017–2021. Los estudios fueron revisados de los sistemas de almacenamiento de imagen y post-procesados (por dos cardiólogos entrenados ciegos a los hallazgos) para obtener valores de función del ventrículo derecho en cada una de las dos técnicas de imágenes tomando como valor de referencia la FEVD por RNMc. Se analizaron de manera independiente cada parámetro de Ecocardiografía-2D y se correlacionaron con la fracción de eyección del VD por RMNc. Resultados y conclusiones: Los parámetros clásicos de evaluación cuantitativa y semicuantitativa del ventrículo derecho (TAPSE, TAVSE, área diastólica de la AD, CAF%, TAPSE/PsAP, PsAP) mostraron un comportamiento escalonado (positivo o negativo) con respecto a la fracción de eyección del VD medida por RMNc. La correlación de los parámetros de evaluación del VD (TAPSE, TAVSE, área diastólica de la AD, CAF%, TAPSE/PsAP, PsAP) por ecocardiografía 2D fue moderada con respecto a la FEVD medida por RNM. La relación TAPSE / PsAP mostró ser el parámetro con mejor comportamiento lineal (correlación) con la FEVD medida por RMNc.
The functional and structural evaluation of the right ventricle (RV) is a fundamental aspect of the echocardiographic study given its prognostic value. Objective: to perform a correlation between the evaluation parameters of the right ventricle by echocardiography and cMRI based on 3 staging levels that take the ejection fraction of the right ventricle (FEVD) by cMRI (mild, moderate and severe). Methods: Historical cohort study of patients undergoing 2D-echocardiography and cMRI during the period 2017–2021. The studies were reviewed from the image storage systems and post-processed (by two trained cardiologists blinded to the findings) to obtain right ventricular function values in each of the two imaging techniques, taking FEVD as the reference value. mcRN. Each 2D-Echocardiography parameter was independently analyzed and correlated with the RV ejection fraction by cMRI. Results and conclusions: The classic parameters of quantitative and semiquantitative evaluation of the right ventricle (TAPSE, TAVSE, diastolic area of the RA, CAF%, TAPSE/PsAP, PsAP) showed a staggered behavior (positive or negative) with respect to the fraction of RV ejection measured by cMRI. The correlation of RV evaluation parameters (TAPSE, TAVSE, RA diastolic area, CAF%, TAPSE/PsAP, PsAP) by 2D echocardiography was moderate with respect to FEVD measured by MRI. The TAPSE / PsAP ratio was shown to be the parameter with the best linear behavior (correlation) with the FEVD measured by cMRI

Introducción. El síncope es una patología frecuente en urgencias, afecta adolescentes y adultos mayores, en estos últimos se asocia con mayor frecuencia a cardiopatía estructural y riesgo de desenlace adverso, el ecocardiograma juega un papel importante en su diagnóstico etiológico.
Objetivo. Establecer la frecuencia de cardiopatía estructural detectada por ecocardiografía, en pacientes que consultaron a urgencias por síncope.
Métodos. Es un estudio descriptivo retrospectivo en el cual se revisaron las historias clínicas de pacientes adultos que consultaron a urgencias por síncope y a los cuales se les realizó ecocardiograma, estableciéndose la frecuencia de presentación de las más importantes cardiopatías estructurales.
Resultados. Evaluados 149 pacientes, 60 presentaron un ecocardiograma normal (40%); 89 un ecocardiograma anormal, de estos las cardiopatías más frecuentes fueron: alteraciones contráctiles del ventrículo izquierdo (35,0%), hipertrofia concéntrica severa (27,5%), cardiopatía dilatada (15,0%), fracción de eyección del ventrículo izquierdo <40% (12,5%), hipertrofia excéntrica severa (7,5%), estenosis aórtica severa (2,5%).
Discusión. Los resultados probablemente están relacionados con las características de la población atendida en un centro cardiovascular de referencia, esto podría explicar además la baja frecuencia de cardiopatía típicamente asociada a sincope cardiogénico detectada en el estudio al compararla con estudios hechos sobre población general.
Conclusión. El ecocardiograma es una herramienta diagnóstica útil en la detección de causas cardiogénicas y no cardiogénicas de sincope. Las cardiopatías estructurales más frecuentemente halladas están relacionadas con enfermedades cardiovasculares prevalentes en la población atendida. Mientras la sospecha clínica del médico tratante sea justificada, el estudio debería realizarse.
Introduction. Syncope is a frequent condition in emergency rooms, hospitals, and outpatient facilities. It affects mainly teenagers and mature adults. It can be often associated with structural cardiomyopathy and adverse cardiovascular complications. Echocardiogram plays a very important role in the detection of potentially fatal pathology.
Objective. To establish the frequency of structural cardiomyopathies by means of an echocardiogram in patients who were admitted into the Shaio Clinic’s emergency room with signs of syncope.
Methods. A retrospective, descriptive study. Clinical charts of adult patients with syncope, who consulted the emergency room services at the Shaio Clinic, were reviewed. An echocardiogram was performed to evaluate their illness and to establish the frequency of the most important structural cardiomyopathies.
Results. A total of 149 patients were evaluated, the average age of the patients was 65.93 years old. 60 patients had a normal echocardiogram (40%); 89 patients had an abnormal result and the most frequent structural cardiomyopathies documented were as follows: contractile alterations of the left ventricle (35%), severe concentric hypertrophy (27.5%), dilated cardiomyopathy(15.0%), ejection fraction of the left ventricle < 40% (12.5%) , eccentric severe hypertrophy (7.5%) and severe aortic stenosis (2.5%).
Discussion. The results obtained in this study show that most of the patients diagnosed with syncope, which consulted to the emergency room services at the Shaio Clinic were over the age of 60. In most of the studies conducted a significant structural echocardiographic finding was present, mainly associated with the advanced age of the patient, multiple previous comorbidities. A low frequency of conditions classically accepted as the cause of cardiogenic syncope was found.
Conclusion. An echocardiogram is a useful tool in the detection of the cardiogenic causes of syncope. It should be requested once the patient’s medical history and physical condition have been reviewed, otherwise, it might not reveal any relevant information. The structural cardiomyopathies more frequently found are not necessarily linked to the classic causes of syncope, but are possibly related to the prevailing cardiovascular diseases present in the population that was examined. The study should be conducted just as long as the opinion of the treating physician is justified.

Objetivo: Describir un caso de un paciente joven con insuficiencia cardiaca, secundaria a dos malformaciones cardiacas infrecuentes, Síndrome de ALCAPA y válvula mitral asimétrica, parecida al paracaídas, resaltando la utilidad de la ecocardiografía.
Métodos: Se analiza el caso a la luz de la literatura médica. Conclusiones. El origen anómalo de la arteria coronaria izquierda del tronco de la arteria pulmonar y la válvula mitral asimétrica parecida al paracaídas, son malformaciones raras, asociadas a insuficiencia mitral severa e insuficiencia cardiaca. No se encontraron reportes en la literatura de la coexistencia de las dos patologías en un paciente.
Objective: To describe a case of a young patient with heart failure, secondary to two infrequent cardiac malformations, ALCAPA Syndrome and asymmetric mitral valve, similar to the parachute, Highlighting the usefulness of echocardiography. Methods: The case is analyzed in the light of medical literature. Conclusions: The anomalous origin of the left coronary artery from the pulmonary artery trunk and the asymmetric mitral valve similar to the parachute are rare malformations associated with severe mitral regurgitation and heart failure. Presentamos el caso de un joven con síntomas constitucionales y disnea de 9 meses, con evidencia de una masa gigante ubicada en la aurícula izquierda que obstruye el tracto de salida de la válvula mitral que requiere resección. La patología mostró sarcoma pleomórfico poco diferenciado con recurrencia muy agresiva poco después de la cirugía. Esta patología es poco frecuente y tiene un pronóstico ominoso a corto plazo. Su tratamiento es la resección quirúrgica y la quimioterapia adyuvante y la radioterapia aún no está claramente definida
We present the case of a young man with constitutional symptoms and dyspnea lasting 9 months, with evidence of a giant mass located at the left atrium obstructing the outflow tract of the mitral valve requiring resection. Pathology showed poorly differentiated pleomorphic sarcoma with very aggressive recurrence shortly after surgery. This pathology is un-frequent and has an ominous short-term prognosis. Its treatment is surgical resection and adjuvant chemotherapy and radiotherapy is not yet clearly defined.

Introducción: La evaluación de la función ventricular izquierda puede ser valorada mediante la cuantificación de los volúmenes ventriculares y representa un valor importante en el diagnóstico, manejo, seguimiento y pronóstico de pacientes con una variedad de entornos clínicos. Sin embargo, en algunos pacientes se presentan limitaciones técnicas para la evaluación de los volúmenes, por lo que se requieren métodos adicionales que ayuden a su estimación. Objetivo: Evaluar la correlación entre la Integral Velocidad Tiempo del tracto (IVT) de salida del ventrículo izquierdo y la Fracción de Eyección del ventrículo izquierdo (FEVI) de los pacientes que ingresaron a un estudio de ecocardiografía. Métodos: Estudio de pruebas diagnósticas para evaluar la correlación de tipo retrospectivo entre la IVT de tracto de salida del ventrículo izquierdo y la FEVI. Resultados: La correlación entre la IVT del tracto de salida del ventrículo izquierdo y la FEVI mostró una correlación fuerte, estimada por un coeficiente de correlación de Pearson, con valor de 0.671 (IC 95% 0.604-0.729) y una significancia estadística p < 0.0001. Conclusión y discusión: La IVT del tracto de salida del ventrículo izquierdo presenta una buena correlación con la FEVI, determinada por método de Simpson con lo que se propone como una herramienta útil para la aproximación en los servicios de ecocardiografía para estimar un aproximado de la FEVI en los casos donde se encuentren limitaciones para su estimación.
Introduction: The evaluation of the left ventricular function can be assessed by quantifying the ventricular volumes and represents an important value in the diagnosis, management, follow-up, and prognosis of the patients in a variety of clinical settings. However, in some patients could have technical limitations in order to evaluate these volumes, so additional methods may be required in order to help estimate them. Objective: To evaluate the correlation between the left ventricular outflow tract velocity time integral (LVOT VTI) and the left ventricular ejection fraction (LVEF) of patients admitted to an echocardiography study. Methods: Study of diagnostic tests to evaluate the retrospective correlation between LVOT VTI and LVEF. Results: The correlation between the LVOT VTI and LVEF showed in this Study a strong correlation, estimated by a Pearson correlation coefficient of 0.671 (95% CI 0.604-0.729) and statistical significance p < 0.0001. Conclusion and discussion: LVOT IVT presents a good correlation with LVEF determined by the Simpson method, which is proposed as a useful tool for the approximation in the echocardiography services to estimate an approximate of the LVEF in cases where there may be limitations for its estimation.

La fibrilación ventricular asociada al empleo de anestésicos locales ha sido descrita en la literatura como una manifestación adversa poco común. El uso de anestesia local, es una técnica ampliamente usada y sus complicaciones cardiovasculares son raras. Se presenta un caso de fibrilación ventricular secundaria al uso de anestesia local.
Ventricular fibrillation associated with the administration of local anesthetics has been described in medical literature as an uncommon adverse reaction. The use of local anesthesics, is a widely used technique and it is cardiovascular complications are rare. We report a case of ventricular fibrillation secondary to the administration of local anesthesics.

OBJETIVOS: Describir y comparar los cambios dinámicos de la geometría del anillo mitral durante todo el ciclo cardiaco en pacientes con insuficiencia mitral isquémica y pacientes con válvula mitral normal.
MATERIALES Y MÉTODOS: Los estudios ecocardiográficos analizados fueron 37, 23 con insuficiencia mitral isquémica y 14 con válvula mitral normal. La reconstrucción del anillo se realizó en la estación de trabajo Xcelera (Philips Medial Systems) mediante la herramienta de análisis mitral (MVQ), en 5 momentos del ciclo cardiaco: Comienzo de Sístole, Mitad de Sístole, Final de Sístole, Mitad de Diástole y Final de Diástole.
RESULTADOS: El anillo del grupo control, fue más dinámico, con sus menores dimensiones al final de la diástole, presentando incremento progresivo durante la sístole. Los cambios en el perímetro y el área, fueron significativos entre el comienzo y mitad de la sístole (p:0.087 y p: 0.055). En el grupo con insuficiencia mitral isquémica, el anillo fue más estático. Todas las dimensiones en este grupo, fueron mayores en los cinco momentos del ciclo cardiaco. (p < 0.1). El anillo también fue más plano, con un índice morfológico anular menor al del grupo control (p:0.087).
DISCUSIÓN Y CONCLUSIONES: En pacientes sin insuficiencia mitral, el anillo es una estructura dinámica. Durante la sístole, las menores dimensiones se produjeron al comienzo de este periodo y la conformación en silla de montar se mantuvo, protegiendo contra la insuficiencia mitral. El anillo del grupo con insuficiencia mitral fue más estático y plano, perdiendo los mecanismos protectores.
OBJECTIVES: To describe and compare the dynamic changes in the geometry of the mitral ring in patients with ischemic mitral regurgitation and normal mitral valve. MATERIALS AND METHODS: 37 studies were analyzed, 23 with ischemic mitral regurgitation and 14 without mitral regurgitation. The mitral ring reconstruction was performed in Xcelera workstation (Philips Medial Systems, Andover, Massachusetts) with a mitral analysis tool (MVQ), in 5 different times of the cardiac cycle: Early systole, middle systole, late systole, middle diastolic and late diastole. RESULTS: The mitral ring of the control group, was more dynamic, with its smaller size in the late diastole, with a progressive increase during systole. Changes in the perimeter and area were significant between the early and middle of systole (p: 0. 087 and p: 0. 055, respectively). In the group with ischemic mitral regurgitation, the ring was more static. All dimensions were larger in this group, with a significant difference in every dimension (p < 0. 1). The annular morphological index was lower is this group, than in the control group. DISCUSSION AND CONCLUSIONS: The mitral ring is a dynamic structure in patients without mitral regurgitation. During systole, the smaller dimensions occurred at the beginning of this period and the saddle shape remained during the whole cardiac cycle, protecting against mitral regurgitation. The mitral ring was static and flat in patients with ischemic mitral regurgitation, losing the protective mechanisms described in the control group.

Introducción: La ecocardiografía es actualmente la técnica de imagen diagnóstica más utilizada para la evaluación de la anatomía y la función cardiovascular. En la actualidad se está utilizando la ecocardiografía por speckle tracking la cual permite una evaluación mas objetiva y confiable de la función ventricular, sin embargo se requieren valores de referencia que hagan que los valores obtenidos sean válidos y útiles para determinar en forma mas oportuna conductas previas al deterioro de su función.
Objetivo general: Determinar los valores de referencia para mecánica ventricular izquierda mediante ecocardiografía bidimensional por speckle tracking con equipo Toshiba Artida con transductor multifrecuencia de 3 megahertzios en pacientes sin patología cardiaca conocida en la Fundación Clínica Shaio en el año 2014.
Metodología: Análisis de una cohorte prospectiva de todos los pacientes que ingresaron a la Fundación Clínica Shaio para evaluación ecocardiográfica sin patología cardiaca conocida entre los meses Agosto y Diciembre del 2014.
Resultados: Se presenta este estudio de la evaluación de la mecánica ventricular izquierda en adultos sanos, los resultados son similares a los obtenidos en estudios de referencia, sin embargo se consideran de gran importancia ya que de acuerdo a la guía actual de evaluación de la mecánica ventricular por strain rate es importante que cada equipo se encuentre estandarizado con el fin de tener resultados válidos de acuerdo a las diferentes patologías en las que se puede aplicar y a nuestra población.
Introduction: Echocardiography is currently the diagnostic imaging technique more often used to evaluate cardiovascular anatomy and function. Nowadays speckle tracking echocardiography is being used, which allows a more accurate and objective evaluation of ventricular function, but nevertheless, reference values are needed to make obtained measurements more valid and useful, to determine in an opportunistic way which decisions are needed to make, prior to function deterioration. Main Objective: To determine reference values for left ventricular mechanics, using Speckle Tracking Two-Dimensional Echocardiography with Toshiba Artida equipment and a 3-megahertz multi-frequency transducer, in patients without any known cardiac pathology at Fundacion Clinica Shaio in 2014. Methods: Prospective cohort analysis of every patient admitted at Fundacion Clinica Shaio for echocardiographic evaluation without any known cardiac pathology from August to December 2014. Results: This study about left ventricular mechanics evaluation in healthy adults is presented; the results are similar to those obtained in reference trials, however these are considered of great importance since current guidelines for Strain Rate ventricular mechanics evaluation state that every equipment needs to be standardized to obtain valid results according to our population and all of the different pathologies where this technique can be applied.

La válvula tricuspídea es tradicionalmente conocida como la válvula olvidada por la menor importancia en cuanto a literatura publicada y ensayos clínicos disponibles hasta hace unos años. La ecocardiografía con sus diferentes modalidades de imagen se mantiene como el patrón de oro para determinar el mecanismo y la severidad de la insuficiencia tricuspídea. En los últimos años, la investigación ha permitido implementar la técnica 3D tanto en el examen transtorácico (TT) como en el transesofágico (TE), para evaluar la anatomía valvular tricúspide desde su aspecto auricular o ventricular, permitiendo analizar la estructura de sus valvas, sus variaciones anatómicas y los diferentes mecanismos o lesiones que pueden afectar su adecuado funcionamiento generando regurgitación tricuspídea con las consecuencias en morbimortalidad que esto conlleva.
The tricuspid valve is traditionally known as the forgotten valve because of the least importance in the published literature and clinical trials available until a few years ago. Echocardiography with its different imaging modalities remains the gold standard to determine the mechanism and severity of tricuspid regurgitation. In recent years, research permits 3D technique to be implemented both in the transthoracic (TT) and transesophageal (TE) echocardiogram, to evaluate the tricuspid valve anatomy from its atrial or ventricular aspect, allowing the structure of its leaflets to be analyzed, its anatomical variations and the different mechanisms or lesions that can affect its proper functioning generating tricuspid regurgitation with the consequences in morbidity and mortality.
