Caracterización de membranas epiretinianas en oclusiones venosas de la retina mediante angiotomografía de coherencia óptica

Objetivo: Evaluar las características clínicas y angiotomográficas (OCT-A) de los pacientes con membranas epiretinianas (MER) secundarias a oclusiones venosas de la retina (OVR). Métodos: Estudio observacional de corte transversal. Se revisaron retrospectivamente imágenes de OCT-A e historias clínicas de 354 pacientes con diagnóstico de OVR, de los cuales 33 ojos de 33 pacientes cumplieron criterios de inclusión. Se calcularon medidas de tendencia central y medidas de dispersión de las variables cuantitativas según su distribución (kolmogorov-smirnov). Las variables cualitativas fueron descritas en frecuencias y porcentajes. Resultados: Fueron incluidos en el análisis treinta y tres ojos. El promedio de edad fue 68,7 años (DE: 12,4) con un 54.5% de mujeres (n:18). La mediana de agudeza visual (AV) LogMAR 0,70 (RIQ: 1,00). El 57,6% (n:19) presentaron oclusión de vena central, 33,3% (n:11) oclusión de rama venosa y el 9,1% (n:3) oclusión hemicentral. El tiempo entre el diagnóstico de OVR y el de MER fue en promedio de 14,4 meses (RIQ: 26) y previo al diagnóstico de MER habían recibido 3,8 (DE:3,7) inyecciones de antiangiogénico. El grosor retiniano central (GRC) promedio fue de 327 um (DE: 132,3) con disrupción en la integridad de la membrana limitante externa (MLE) y zona elipsoide (ZE) en 60,6% (n:20) y 66.7% (n:22) respectivamente. Hubo presencia de espacios cistoides intraretinianos en la mayoría de pacientes (60,6% n:20) y desorganización de capas retinianas internas (DRIL) en 42,2% (n:14). Se encontraron capas foveales internas ectópicas solo en 12.1% (n:4) de los ojos. La localización de las MER fue en su mayoría extrafoveal (75,7% n: 25) con un patrón de adherencia a la superficie de la retina mixto (global y focal) en un 45,5% (n: 15) de los casos. La zona avascular foveal (ZAF) total fue de 280 um (RIQ: 221um). Se observó que los ojos con MER foveal y disrupción en la MLE y ZE mostraron una tendencia a tener peor AV. Conclusiones: Las características estructurales y vasculares en la retina de pacientes con MER secundarias a OVR corresponden a cambios propios de la patología vascular. Este grupo de pacientes no comparten las características angiotomográficas descritas para MER idiopáticas. La disrupción de la MLE y ZE puede estar asociado con peor agudeza visual.
Purpose: To evaluate clinical and angiotomography (OCT-A) characteristics of patients with epiretinal membranes (ERM) secondary to retinal vein occlusions (RVO). Method: Cross sectional study. OCT-A images and medical records of 354 patients diagnosed with RVO were retrospectively reviewed, of which 33 eyes from 33 patients met inclusion criteria. Measures of central tendency and measures of dispersion of quantitative variables according to their distribution (kolmogorov-smirnov) were calculated. The qualitative variables were described in frequencies and percentages. Results: The mean age was 68.7 years (SD: 12.4) with 54.5% women (n: 18). Median visual acuity (AV) LogMAR 0.70 (IQR: 1.00). 57.6% (n: 19) had a diagnosis of central retinal vein occlusion (CRVO) 33.3% (n: 11) branch retinal vein occlusion (BRVO) and 9.1% (n: 3) hemicentral vein occlusion (HVO). The time between the diagnosis of RVO and ERM averaged 14.4 months (IQR: 26), and previous to the diagnosis of ERM received 3.8 (SD: 3.7) antiangiogenic injections. The mean central retinal thickness (CRT) was 327 um (SD: 132.3) with disruption of the integrity of the external limiting membrane (ELM) and ellipsoid zone (EZ) in 60.6% (n: 20) and 66.7% % (n: 22) respectively. There was presence of intraretinal cystoid spaces in most patients (60.6% n: 20) and disorganization of the inner retinal layer (DRIL) in 42.2% (n: 14). Ectopic inner foveal layers (EIFL) were found only in 12.1% (n: 4) of the eyes. The ERM location was mostly extrafoveal (75.7% n: 25) with a mixed pattern of adherence to the retinal surface (global and focal) in 45.5% (n: 15) of the cases. The mean foveal avascular zone (FAZ) was 280 um (IQR: 221um). Eyes with foveal-involving MER and disruption in MLE and ZE show a tendency to have worse VA. Conclusions: The structural and vascular retinal characteristics of patients with ERM secondary to RVO correspond to inherent changes to vascular pathology. This group of patients does not share the angiotomography characteristics described for idiopathic ERM. Disruption of the MLE and ZE may be associated with worse visual outcome.

Factores pronósticos en la agudeza visual final de pacientes con oclusión venosa retiniana tratados con terapia antiangiogénica

Objetivo: determinar los diferentes factores clínicos y de imagen pronósticos para la agudeza visual final a los 3, 6 y 12 meses en los pacientes con oclusiones venosas retinianas tratados con terapia antiangiogénica. Material y métodos: estudio longitudinal de una cohorte de 60 pacientes con oclusión venosa retiniana tratados con terapia antiangiogénica intravitrea con ranibizumab y bevacizumab tomados retrospectivamente entre 2010 y 2012. Posteriormente se realizó un estudio analítico de cohorte retrospectiva, para establecer la asociación y predicción de los cambios de la agudeza visual. Resultados: se encontró una diferencia estadísticamente significativa entre la línea base, 3, 6 y 12 meses post tratamiento. (p < 0.001, prueba de Friedman). En el modelo explicativo de los cambios de la agudeza visual a los 3, 6 y 12 meses con la línea base, se encontró asociación significativa la presencia de IS/OS a los 3 meses con los cambios de la agudeza visual a los 6 meses (r2=.232, p< 0.001) y a los 12 meses (r2=.506, p< 0.001), en este último también se encontró asociado el género masculino (r2=.277 p< 0.001). Tomando los valores de la agudeza visual a los 3,6 y 12 meses y como covariable la agudeza visual inicial se encontró asociación significativa en los tres momentos, 3 meses (r2=0.697, p< 0.001), 6 meses (r2=745, p< 0.001) y 12 meses y (r2=786, p< 0.001), se encontró asociación significativa el presentar oclusión venosa de rama comparada con oclusión central con los cambios de la agudeza visual a los 6 meses (r2=.662, p=0.04) Conclusión: la presencia de IS/OS a los 3 meses y la agudeza visual inicial tienen una alta capacidad predictora de la agudeza visual a los 6 y 12 meses, el género masculino tiene relación con mejoría de la agudeza visual final y la oclusión de rama tiene relación con la mejoría de la agudeza visual a 6 meses.
Objective: To determine the different clinical and imaging factors as prognostic factors for final visual acuity at 3,6 and 12 months in patients with retinal vein occlusion treated with antiangiogenic therapy.
Methods: Longitudinal study of a cohort of patients with retinal vein occlusion treated with intravitreal antiangiogenic therapy taken retrospectively between 2010 and 2012. Subsequently we conducted a retrospective cohort analysis study to establish the association and predicting changes in visual acuity.
Results: We found a statistically significant difference between the baseline, 3, 6 and 12 months post treatment. (p <0.001, Friedman test). In the explanatory model of changes in visual acuity at 3, 6 and 12 months with the baseline, significant association was found between the presence of IS / OS at 3 months with changes in visual acuity at 6 months (r2 = .232, p <0.001) and 12 months (r2= .506, p< 0.001), the latter was also found associated with male gender (r2 = .277 p <0.001). Taking the values of visual acuity at 3,6 and 12 months and as a covariate initial visual acuity; significant association was found in the three moments, at three months (r2 = 0.697, p <0.001), 6 months (r2 = 745, p <0.001) and 12 months (r2 = 786, p <0.001), Significant association was found to present branch vein occlusion central occlusion compared with changes in visual acuity at 6 months (r2 = .662, p = 0.04)
Conclusion: Conclusion: The presence of IS / OS at 3 months and initial visual acuity have a high capacity to predict visual acuity at 6 and 12 months, gender is related to final visual acuity and branch occlusion is related to visual acuity at 6 months.

Este es un trabajo observacional descriptivo tipo serie de casos que pretende describir el desenlace del uso de suero autólogo como adyuvante en el cierre anatómico y mejoría de la agudeza visual en pacientes con agujero macular idiopático y persistente, llevados a un segundo tiempo quirúrgico donde se realiza vitrectomía posterior + remoción de membrana limitante interna + suero autólogo + gas en la Fundación Oftalmológica Nacional entre Enero de 2019 y Enero de 2022. Se ha propuesto en la literatura que el uso de suero autólogo como adyuvante de cirugía vitreorretiniana para agujero macular aumenta el porcentaje de cierre anatómico, así como el pronóstico visual.
This is an observational descriptive case series study that aims to describe the outcome of the use of autologous serum as an adjuvant in the anatomical closure and improvement of visual acuity in patients with idiopathic and persistent macular hole, who underwent a second surgical procedure where posterior vitrectomy + internal limiting membrane removal + autologous serum + gas was performed at the National Ophthalmological Foundation from January 2019 to January 2022. In the literature, it has been proposed that the use of autologous serum as an adjuvant to vitreoretinal surgery for macular hole increases the percentage of anatomical closure, as well as the visual prognosis

Los estudios RISE y RIDE son ensayos clínicos aleatorios, paralelos multicéntricos, fase III enfocados en el uso de Ranibizumab para el tratamiento del edema macular diabético. En esta presentación se revisan 10 de las lecciones más importantes para el manejo de esta enfermedad.

Propósito: Analizar las características morfológicas del material hiperreflectivo subretiniano (MHRS) y resultados visuales a corto plazo en pacientes con degeneración macular relacionada con la edad neovascular (DMREn) tratados con antiangiogénicos en la Fundación Oftalmológica Nacional entre el 2011 y 2017. Metodología: Estudio descriptivo longitudinal, en el que se incluyeron 65 pacientes con DMREn no tratada previamente, entre enero del 2011 y diciembre del 2017. Se revisó la tomografía de coherencia óptica (OCT) estructural para determinar las características cuantitativas y cualitativas del MHRS, y la agudeza visual mejor corregida (AVMC) en LogMAR antes y después del tratamiento. Resultados: La mediana de edad fue 76 años (rango 68-84). La mayoría de pacientes fueron mujeres en 56.9%. La lateralidad del ojo más afectado fue el derecho en 50.8%. Aflibercept fue el antiangiogénico más empleado en el 40% de los pacientes. La mediana de AVMC fue de 0.7 (rango 0.5-1.2) logMAR y 0.7 (rango 0.4-1) logMAR de base y después del tratamiento, respectivamente. Los parámetros de base con una correlación positiva por análisis bivariado antes y después del tratamiento fueron la AVMC, reflectividad, bordes y extensión del MHRS. Los parámetros que se asociaron a una mejoría en la AVMC después del tratamiento fueron la presencia de zona elipsoide (ZE) y membrana limitante externa (MLE), la reflectividad, homogeneidad y extensión del MHRS. Conclusión: Este estudio proporciona características tomográficas del MHRS asociadas con resultados visuales a corto plazo en ojos con DMREn tratados con antiangiogénicos.
Purpose: To analyze the morphological characteristics of subretinal hyperreflective material (SHRM) and short-term visual outcomes in patients with neovascular age-related macular degeneration (nAMD) treated with anti-vascular endothelial grown factor (anti-VEGF) at the Fundacion Oftalmologica Nacional between 2011 and 2017. Methodology: Descriptive longitudinal study, including 65 patients with previously untreated nAMD, between January 2011 and December 2017. The structural optical coherence tomography (OCT) was reviewed to determine quantitative and qualitative characteristics of SHRM, and best corrected visual acuity (BCVA) in LogMAR before and after treatment. Results: The median age was 76 years (range 68-84). The majority of patients were women in 56.9%. The laterality of the most affected eye was the right in 50.8%. Aflibercept was the most widely used antiangiogenic in 40% of the patients. The median BCVA was 0.7 (range 0.5-1.2) logMAR and 0.7 (range 0.4-1) logMAR at baseline and after treatment, respectively. The baseline parameters with a positive correlation by bivariate analysis before and after treatment were the BCVA, reflectivity, edges and extension of the SHRM. The parameters that were associated with an improvement in BCVA after treatment were the presence of the ellipsoid zone (EZ) and external limiting membrane (ELM), reflectivity, homogeneity and extension of the SHRM. Conclusion: This study provides tomographic features of the SRHM associated with short-term visual outcomes in eyes with nAMD treated with anti-VEGF.

Comportamiento del desprendimiento del vítreo posterior en los pacientes expuestos a cirugía de catarata

Objetivo: Describir el comportamiento del desprendimiento del vítreo posterior (DVP) en pacientes expuestos a cirugía de catarata mediante la biomicroscopia, la ecografía ocular y la tomografía de coherencia óptica macular.
Materiales y métodos: Se realizó un estudio descriptivo, una serie de casos clínicos de 13 pacientes expuestos a cirugía de catarata en la Fundación Oftalmológica Nacional entre febrero a julio de 2015, con seguimiento a 12 meses. Durante 6 visitas se les realizó toma de agudeza visual mejor corregida y biomicroscopía. Tambíen se les realizó ecografia ocular y tomografia de coherencia óptica macular.
Resultados: El porcentaje de DVP por biomicroscopia cambió desde un 7.7% a un 38.4%. El porcentaje de DVP por ecografía en el área nasal cambió de 92.3% a 76.9%. En el área temporal la tasa de DVP cambió de 84.6% y a 76.9%. En al área superior se mantuvo en un 61.5%. En el área inferior varió de un 69.2% a un 76.9%. Y por último, en el área macular de un 53.8% a un 76.9%. El porcentaje de DVP por OCT cambio desde un 69.2% a un 76.9%, en la visita cero y la visita cuatro, respectivamente.
Conclusiones: La cirugía de catarata acelera el proceso del DVP. Hubo una progresión del DVP según la biomicroscopia y el OCT, la ecografía no la consideramos una herramiento eficaz para describir la progresión del DVP.
Purpuse: To describe the behavior the time of posterior vitreous detachment in patients exposed to cataract surgery by biomicroscopy, ocular ultrasound and macular optical coherence tomography.
Materials and methods: A descriptive study of case series of 13 patients was conducted in the Fundacion Oftalmologica Nacional from February to July 2015, 12-month follow-up was performed. For 6 visits they underwent best corrected visual acuity and slitlamp examination. Also they underwent ocular ultrasound and macular optical coherence tomography.
Results: The rate of posterior vitreous detachment by biomicroscopy changed from 7.7% in the visit 0 to 38.4% in visits four. DVP rate by ultrasound in the nasal area changed from 92.3% to 76.9%. In the temporal area changed from 84.6% to 76.9%. In the upper area remained 61.5%. In the lower area it was 69.2% and changed to 76.9%. And finally, in the macular area was 53.8% and in the four visit was 76.9%. The OCT PVD rate change from 69.2% to 76.9%.
Conclussion: Cataract surgery accelerate DVP process. There was an increase of DVP by biomicroscopy and OCT. Ultrasound was not consider an effective tool for diagnosing PVD.

Red neuronal artificial para la detección de edema macular diabético en imágenes de tomografía de coherencia óptica : estudio transversal de prueba diagnóstica

Objetivo: Determinar la exactitud de una red neuronal artificial para el diagnóstico de edema macular diabético en imágenes de tomografía de coherencia óptica de la macula. Diseño: Estudio transversal de prueba diagnóstica. Métodos: Se recolectaron 100 grupos de 5 imágenes obtenidas de ojos con diagnóstico de edema macular diabético y 100 grupos de imágenes sin edema macular diabético. El 70% de las imágenes se usaron para entrenar la red neuronal artificial y el 30% se usaron para el desarrollo de la prueba. Resultados: En el análisis por grupos de 5 imágenes se encontró una sensibilidad del 81.82% (IC 64.54-93.02%) y una especificidad del 88,89% (IC 70.84-97.65%), con una exactitud del 85,00% (IC 73.43-92.90%). El Valor Predictivo Positivo fue de 90.00% (IC 75.37-96.36%) y el Valor Predictivo Negativo del 80.00% (65.71-89.30%). Conclusiones: La RNA mostro un buen rendimiento para la identificación de EMD en imágenes de TCO de pacientes con DM. Se requieren estudios con mayor tamaño de muestra para mejorar la precisión de los resultados.
Purpose: To calculate the accuracy of “OCT-net”, an artificial neural network, to diagnose diabetic macular edema using optical coherence tomography images. Study design: Cross-sectional diagnostic test study. Methods: we collected 100 image volumes from eyes diagnosed with diabetic macular edema and 100 volumes of healthy individuals, each volume contains 5 B-scan images. We split the images randomly in two datasets: 70% of the images were used to train the artificial neural network and 30% for testing its accuracy. Results: sensitivity was found to be 81,82% (CI 64,54-93,02%) and specificity 88,89% (CI 70,84-97,65%) in the image volume analysis. Positive and negative predictive values were 90% (CI 75,37-96,36%) and 80% (CI 65,71-89,30%) respectively. Conclusions: In our study, OCT-net showed a good performance on detecting diabetic macular edema in optic coherence tomography images. More studies with greater sample size are required.

Cambios maculares y de capa de fibras nerviosas ganglionares posterior a la vitrectomía pars plana y sus factores pronósticos en la fundación oftalmológica nacional

OBJETIVOS: Determinar los factores pronóstico, cambios maculares morfológicos y de capa de fibras nerviosas ganglionares posterior a vitrectomía pars plana, en la Fundación Oftalmológica Nacional.
MATERIALES Y MÉTODOS: Estudio longitudinal de antes y después (3y6 meses) de la vitrectomía pars plana (VPP) en pacientes con membrana epirretiniana, agujero macular, síndrome de tracción vítreo macular y opacidades vítreas no-inflamatorias. Se realizó seguimiento clínico y con tomografía de coherencia óptica.
RESULTADOS: Grupo de 60 pacientes (mujeres 65.0%), edad promedio 65.45+9.49años y tiempo de enfermedad promedio 23+29.79meses. Las indicaciones de VPP (n=60ojos) fueron agujero macular (38.3%) y membrana epirretiniana (36.7%). Se encontró diferencia significativa entre grosor del complejo capa de células ganglionares (CCG)+capa plexiforme interna (CPI) inicial y 3 meses (p=0.039), correlación entre grosor del complejo CCG+CPI al tercer y sexto mes (r=0.704,p<0.001) y grosor del complejo CCG+CPI al tercer mes con grosor foveal central (CFT) al tercer y sexto mes (r=–0.594,p<0.001 y r=–0.595,p=0.001). Mayores de 65años tenían menor grosor de CFNG a 6meses (r=-0.528,p=0.007). El grosor de CFNG promedio y la presencia de la zona elipsoide inicial fueron factores pronósticos de buena agudeza visual al tercer mes de VPP (r2=0.414,p=0.018, y r2=0.414,p=0.010).
CONCLUSIÓN: El grosor de CFNG y la presencia de la zona elipsoide inicial tienen alta capacidad predictiva de buena agudeza visual al tercer mes de VPP, y, correlación inversa entre grosor del complejo CCG+CPI con CFT al tercer y sexto mes de VPP.
PURPOSE: To determine prognostic factors, macular morphology and nerve fiber ganglion layer changes after pars plana vitrectomy, at Fundación Oftalmológica Nacional.
DESIGN AND METHODS: Longitudinal trial, before and after pars plana vitrectomy (3 and 6 months) involving patients with epiretinal membrane, macular hole, macular vitreous traction syndrome and noninflammatory vitreous opacities. Clinical and optic coherence tomography monitoring was done.
RESULTS: Sixty patients (65.0% women), with average age of 65.45+9.49 years, and mean disease duration of 23+29.79months. The indications for pars plana vitrectomy (n=60eyes) were macular hole (38.3%) and epiretinal membrane (36.7%). Significant difference was observed in the thickness of ganglion cell layer (GCL)+inner plexiform layer (IPL) complex at baseline and third months (p=0.039), correlation between GCL + IPL complex at third and sixth months (r=0.704,p<0.001) and GCL + IPL complex at third month with central foveal thickness (CFT) at third and sixth months (r=–0.594,p<0.001 and r=–0.595,p=0.001). Patients older than 65 years have less GCL thickness at six months (r=-0.528,p=0.007). GCFL average thickness and the presence of ellipsoid layer were baseline predictors of good visual acuity after pars plana vitrectomy (r2=0.414,p=0.018, and r2=0.414,p=0.010).
CONCLUSION: GNFL thickness and the presence of the ellipsoid zone at baseline are highly predictive of good visual acuity at the third postoperative month of pars plana vitrectomy and there is an inverse correlation between GCL + IPL complex with CFT at third and sixth months after pars plana vitrectomy.

Hiperautofluorescencia foveal como factor predictor de recuperación visual en pacientes con desprendimiento regmatógeno de retina

Introducción: una de las causas de pobre ganancia visual luego de un tratamiento exitoso de desprendimiento de retina, sin complicaciones, es el daño de los fotoreceptores, reflejada en una disrupción de la capa de la zona elipsoide y membrana limitante externa (MLE). En otras patologías se ha demostrado que la hiperautofluorescencia foveal se correlaciona con la integridad de la zona elipsoide y MLE y una mejor recuperación visual.
Objetivos: evaluar la asociación entre la hiperautofluorescencia foveal, la integridad de la capa de la zona elipsoide y recuperación visual luego de desprendimiento de retina regmatógeno (DRR) exitosamente tratado. Evaluar la concordancia inter-evaluador de estos exámenes.
Metodología: estudio de corte transversal de autofluorescencia foveal y tomografía óptica coherente macular de dominio espectral en 65 pacientes con DRR evaluados por 3 evaluadores independientes. La concordancia inter-evaluador se estudio mediante Kappa de Cohen y la asociación entre las diferentes variables mediante la prueba chi cuadrado y pruebas Z para comparación de proporciones.
Resultados: La concordancia de la autofluorescencia fue razonable y la de la tomografía óptica coherente macular buena a muy buena. Sujetos que presentaron hiperautofluorescencia foveal asociada a integridad de la capa de la zona elipsoide tuvieron 20% más de posibilidad de recuperar agudeza visual final mejor a 20/50 que los que no cumplieron éstas características.
Conclusión: Existe una asociación clínicamente importante entre la hiperautofluorescencia foveal, la integridad de la capa de zona elipsoide y la mejor agudeza visual final, sin embargo ésta no fue estadísticamente significativa (p=0.39)
Introduction: photoreceptor damage, reflected in ellipsoid and external limiting membrane (ELM) layer disruption, is one of the causes of poor visual recovery after successful rhegmatogenous retinal detachment. A correlation between foveal hyperautofluorescence, integrity of ellipsoid and ELM layers and better visual recovery has been demonstrated in other pathologies.
Purpose: to evaluate the association between foveal hyperautofluorescence, the integrity of ellipsoid layer and visual recovery in patients treated successfully for rhegmatogenous retinal detachment (RRD) and inter evaluator concordance for these exams.
Methods: cross-sectional study in which 65 patients with RRD were evaluated by 3 different evaluators with macular spectral-domain optical coherence tomography (SD-OCT) and foveal autofluorescence. Inter evaluator concordance was studied with Cohen´s Kappa and the association between variables was studies with Chi square and Z test for proportions comparison.
Results: the concordance for fundus autofluorescence was fair, and good to very good for SD-OCT. People who had foveal hyperfluorescence associated with ellipsoid layer integrity had 20% more chance to have a final visual acuity ≥ 20/50, compared with those who had note.
Conclusion: there is a clinically relevant association between foveal hyperautofluorescence, ellipsoid layer integrity and better visual results, however this is not statistically significant (p=0.39)

Comparación de grosor macular y grado de miopía en grupo de pacientes de la Fundación Oftalmológica Nacional

OBJETIVOS: Comparar las medidas de grosor macular, medido por HD-OCT Cirrus®, en pacientes de la Fundación Oftalmológica Nacional con miopía baja y alta.
MÉTODOS: Estudio transversal analítico, en el que se incluyeron pacientes con miopía alta, definida como defecto esférico mayor a 6.0 dioptrías (D), y pacientes con miopía baja, definida como defecto esférico entre -1.00 D y -6.00 D; a los que se le realizó mapa de grosores maculares con el HD-OCT Cirrus®, y se compararon las medias de los grosores (1, 3 y 6 mm).
RESULTADOS: Un total de 128 ojos de pacientes entre 12 y 80 años fueron incluidos; 64 ojos en grupo de miopes bajos y 64 ojos en grupo de miopes altos. La media de edad fue: 37.3 y 38.4 años, y el defecto esférico promedio fue de 2.2 D y 11.5 D, respectivamente. El promedio de grosores en mácula interna (319.9 vs. 307.87 micras, P=0.002), y mácula externa (276.08 vs. 270.23 micras, P=0.047) fue significativamente mayor en el grupo de miopía baja que en el de miopía alta. Sin embargo la media del grosor macular central (1 mm) no tuvo diferencia (261.25 vs. 262 micras P=0.891).
CONCLUSIÓN: Este estudio muestra que el grosor macular de 3 y 6 mm centrales es más delgado en miopes altos, pero en 1 mm central es similar que en miopes bajos. En los pacientes estudiados no se encontró diferencia en grosores maculares según género o subgrupos de edad.
PURPOSE: To compare the macular retinal thickness by HD-OCT Cirrrus®, between patients with low and high myopia, from the Fundación Oftalmológica Nacional.
METHODS: This prospective comparative study recruited asintomatic subjects with high myopia, defined as a spherical defect over 6 dioptres(D), and subjects with low myopia, defined as a spherical defect between -1.0 D and 6.0 D. Optical coherence tomography eith HD-OCT Cirrus® was performed.
RESULTS: A total of 128 eyes between 12 and 80 years were selected as test eyes. 64 high myopic eyes and 64 low non- myopic eyes were included. The mean age of the high myopic group and low myopic group was 37.3 and 38.4 years old, respectively. The mean refraction was 11.5 D in the high myopia group and 2.2 in the non-myopia group. The low myopia group had significantly greater mean retinal thickness in the inner (319.9 vs 307.87 micras, P=0.002) and outer (276.08 vs 270.23 micras, P=0.047) macular area than the hign myopia group. However, the mean retinal thickness in 1 mm area was similar in both groups.
CONCLUSION: This study demostrated that the retinal thickness in individuals with high myopia is thinner in the inner (3 mm) and the outer (6 mm) macular region, but similar in 1 mm area than those with low myopia. There is not difference in macular thickness relative with age or gender.
