Unidad de cuidado intensivo adulto cerrada versus abierta : ansiedad en cuidadores-familiares

Resumen
Objetivo: Estimar la prevalencia de ansiedad estado y la diferencia en el grado de ansiedad en cuidadores-familiares de pacientes en UCIA con y sin restricción de visitas, en la Clínica los Andes Tunja durante abril de 2017 a septiembre de 2017. Metodología: Estudio de tipo cuasi-experimental en el que se identificaron dos grupos de familiares de los pacientes, expuestos a una política de restricción de visitas (política actual de la clínica), y al protocolo de visitas flexibles “UCIA Los Andes”. La selección de las unidades muestrales fue de manera secuencial cada vez que ingresaban los pacientes a la UCIA, se aplicó un cuestionario sociodemográfico y el Inventario de Ansiedad Rasgo-Estado (IDARE), y se diligenció un formato de variables clínicas de cada paciente. Resultados: Se utilizó prueba de Mann-Whitney y se encontró que la prevalencia de ansiedad estado en los cuidadores-familiares de pacientes críticos y la variación en el grado de esta, tiene diferencias estadísticamente significativas en dos condiciones: restricción y no restricción de visitas (p 0,001). Discusión: Si bien la disminución de la prevalencia y el grado de ansiedad estado de los cuidadores-familiares de pacientes en un tipo de UCIA abierta, es demostrada en este estudio y consistente con los resultados de estudios previos, la limitación es que la inferencia de estos hallazgos a otros entornos hospitalarios y de cuidados intensivos, lo limita el pequeño tamaño de muestra; aún así este estudio es un referente frente a las escasas investigaciones en Colombia, que permiten evidenciar el impacto que tiene la visita flexible o restringida, sobre los niveles de ansiedad de los cuidadores familiares y se espera que los resultados obtenidos, contribuyan a reflexionar, considerar e incluso a desarrollar políticas transformadoras en las UCI, que beneficien a los familiares.
Abstract
Objective: To estimate the prevalence of state anxiety and the difference in the level of anxiety in caregivers of patients in ICU with and without restriction of visits, at the Clínica Los Andes Tunja during April 2017 to September 2017. Methodology: Study of a quasi-experimental type in which two groups of family members of the patients were identified, exposed to a policy of restricting visits (current policy of the clinic), and to the protocol of flexible visits "ICU Los Andes". The selection of the sample units was sequential every time the patients were admitted to the ICU, a sociodemographic questionnaire and the Trait-State Anxiety Inventory (TSAI) were applied, and a format of clinical variables of each patient was filled out. Results: Mann-Whitney test and It was found that the prevalence of state anxiety in caregivers of critical patients and the variation in the level of this have statistically significant differences in two conditions: restriction and no restriction of visits (p 0.001). Discussion: Although the decrease in the prevalence and level of anxiety state of patients caregivers-relatives in a type of open ICU, is demonstrated in this study and it is consistent with the results of previous studies, the limitation is that the inference of these findings to other hospital and intensive care settings are limited by the small sample size; Even so, this study is a benchmark against the scarce research in Colombia, which makes it possible to demonstrate the impact of flexible or restricted visits on the anxiety levels of family caregivers and it is expected that the obtained results will contribute to reflect, consider and even to develop transformative policies in the ICUs, that benefit the patients relatives.
