Caracterización clínica y paraclíncia de niños de 0 a 18 años con artritis séptica de rodilla atendidos en el Hospital Kennedy durante los años 2014 al 2016

dc.description | Introducción: La artritis séptica de rodilla Se asocia a complicaciones mayores. Un oportuno diagnóstico y tratamiento temprano son fundamentales. Materiales y métodos: Se realiza una revisión de historias clínicas retrospectiva de todos los pacientes con artritis séptica menores de 18 años admitidos en entre Enero de 2014 hasta Junio de 2017. Se analizan las variables de los criterios de Kocher frente a la positividad de cultivos o la visualización macroscópica de pus en liquido sinovial. Las 4 variables descritas por Kocher: Conteo leucocitario mayor a 12000 , VSG mayor de 40 mg/dl , cojera/incapacidad para el apoyo y fiebre dado por temperatura > 38,5 C. Adicionalmente se añade la positividad de la Proteína C Reactiva (mayor a 5). Se realiza un análisis uni y multivariado de cada una de las variables mediante el uso de la herramienta informática STATA. Resultados: No existe una variable por sí sola con un poder de correlación superior frente a las otras, al tomar la positividad de cultivos como standard de referencia (p.0.24-1.) Ningún paciente con cultivos positivos mostró tener positividad en las 4 variables descritas por Kocher. El análisis multivariado evidencia que las características clínicas positivas: cojera y fiebre, fueron las variables que más presentaron diferencia de proporción frente a las combinación de las otras variables y la positividad en cultivos (RP 0.80 y 0.83) respectivamente. Al incorporar positividad de PCR como variable , son: la leucocitosis , fiebre y PCR positiva las variables con mayor diferencia de proporción. Al incluir la visualización macroscópica de signos de infección todas las variables mostraron una diferencia de proporción significativa mayor (p-1). Conclusiones: A pesar de que hay relación directa entre el número de variables positivas y el diagnóstico definitivo de AS, es necesario realizar estudios con mayor número de pacientes y de tipo multicéntrico para emitir conclusiones estadísticamente significativas. | spa |
dc.description.abstract | Background: Septic Arthritis of the knee has an incidence of 2 -29 cases per 100.000. It´s associated with mayor complications such as septic chondrolysis, Early physeal closure and osteoarthritis. And Early diagnosis and treatment are fundamental. Materials and Methods : We conducted a retrospective revision of the clinical record of every patient with less than 18 years of age between 2014 and 2017. We analyzed every variable from the Kocher criteria and compared them to a positive culture or macroscopical finding of articular infection. The 4 variables described by Kocher: leukocytes > 12,000, erythrocyte sedimentation rate (ESR) > 40 mg/dl, limp or impossibility to weight bare, an fiver (temperature >38C). We analyzed the Reactive c protein (>5mg/dl) separately and made a uni an multi variable analysis using a data analysis and statistical software STATA. Results : There was not a single variable which by itself had a grater correlation with septic arthritis than the others when compared with a positive culture (p.0.24-1.) There were no patients with a positive culture that had the 4 Kocher criteria present. The multivariable analysis showed . Clinical variables such as fiver and Impossibility to weight bare where the variables that showed most correlation with positive cultures (RP 0.80 y 0.83). When the Reactive C protein was analyzed it showed a mayor correlation when combined with Fiver and Leucocytes >12,000. When we used cultures and macroscopic findings of infection every variable showed a bigger correlation. Conclusions : Aldo there was a clear relation between the number of positive Kocher Criteria and the final diagnosis of septic arthritis, we need bigger studies with multicentre analysis to produce definitive conclusions with statistical significance | eng |
