Cambios radiográficos en fracturas de radio distal extraarticulares tratadas con fijación percutanea seis meses después del trauma

dc.description | A pesar de que no todas las maluniones de fracturas de radio distal son sintomáticas, el fin del tratamiento de estas fracturas es restaurar y mantener la anatomía del radio hasta que este sane, sin embargo con la reducción cerrada y fijación percutánea no es claro si hay cambios radiológicos con el paso del tiempo. La reducción anatómica y la persistencia de esta reducción a largo plazo determina la probabilidad de que se presenten complicaciones. Se busca determinar si se mantienen las medidas radiográficas de estabilidad, en pacientes con fracturas de radio distal fijadas con técnica de Clancey, evaluando radiografías prequirurgicas, quirúrgicas y de control en un seguimiento a seis meses de pacientes tratados en el Hospital Universitario Mayor midiendo las 4 variables radiológicas propuestas por Graham en las fracturas de radio distal, (inclinación radial, altura radial, inclinación volar, varianza ulnar) en 23 pacientes. De esta forma determinar que pacientes son los indicados para realizar este tipo de intervención. Se encontró que la técnica de Clancey restablece los parámetros de normalidad en cuanto a los 4 criterios radiológicos de Graham pero que con el paso del tiempo se observa una pérdida de la reducción inicial. No se encontraron diferencias estadísticamente significativas en cuanto a los diferentes grupos de edad. A pesar de que esta técnica es de bajo costo comparada con la reducción abierta y fijación interna, la reducción que se logra después del manejo inicial se pierde con el paso del tiempo en un alto porcentaje de los pacientes. | spa |
dc.description.abstract | Although not all malunions of distal radius fractures are symptomatic, the end point of the treatment of these fractures is to restore and maintain the anatomy of the radius until it heals, however with the use of closed reduction and percutaneous fixation it is not clear whether or not there are radiological changes over time. The anatomical reduction and the persistence of this reduction in the long term determines the likelihood of complications. The aim is to determine whether or not this treatment maintains radiographic stability measurements in patients with distal radius fractures with Clancey technique, evaluating pre-surgical, surgical and control six-month follow-up of patients treated at the Hospital Universitario Mayor, measuring the four radiological variables proposed by Graham in distal radius fractures (Radial inclination, radial height, volar tilt, ulnar variance) in 23 patients. In this way it can be determined which patients are indicated to perform this type of intervention. It was found that Clancey technique re-establishes the parameters of normality regarding the 4 radiological criteria but with the passage of time a loss of initial reduction is observed. No statistically significant differences were found in the different age groups. Although this technique is inexpensive compared to open reduction and internal fixation, the reduction that is achieved after the initial management is lost with the passage of time in a high percentage of patients. | eng |
