Estimación del umbral de costo-efectividad en salud por año de vida ganado para Colombia

Resumen
El propósito de este trabajo fue realizar una estimación empírica del umbral de costo-efectividad por año de vida ganado para el sistema de salud colombiano, desde la perspectiva de la oferta. Esta perspectiva define un marco teórico y empírico, explícito y consistente, que puede ser replicado para diferentes contextos de salud. En el enfoque de la oferta, el umbral se define como el costo de oportunidad de destinar recursos a una nueva tecnología, en términos de los beneficios en salud desplazados o no percibidos como consecuencia de su no disponibilidad, para financiar otras alternativas que compiten por el mismo presupuesto; y su estimación se basa en la cuantificación del efecto causal del gasto sobre los beneficios en salud. Para responder esta pregunta de investigación se desarrolló un modelo econométrico tipo panel no balanceado en tres dimensiones: 1) Entidades Administradoras de Planes de Beneficios en Salud (EAPB) de Colombia; 2) diferentes grupos de condiciones de salud a partir de la Lista Internacional para la Tabulación de la Morbilidad Hospitalaria de la Organización Mundial de la Salud; y 3) período temporal del 2012 al 2016. El desenlace en salud empleado fueron los Años de Vida Ganados (AVG), calculados usando metodologías de tablas de vida, esperanza de vida ajustada por condición de salud y curvas de supervivencia. La endogeneidad esperada se trató de resolver a través del enfoque de variables instrumentales. La estimación se desarrolló utilizando Mínimos Cuadrados Ordinarios en 2 etapas. Para alimentar el modelo se construyó una base de datos novedosa a partir de múltiples fuentes de información institucionales del país para el período 2012-2016: Estudios de Suficiencia, Estadísticas Vitales, Registro Individual de Prestación de Servicios, Base de Datos Única de Afiliados y la información financiera de la Superintendencia de Salud. Por la disponibilidad de información, la estimación se delimitó al universo del régimen contributivo y de tecnologías incluidas en el Plan de Beneficios con Cargo a la Unidad de Pago por Capitación (PBS-UPC). Como variables instrumentales se exploraron los estados financieros de las EAPB para cada año, y el gasto per cápita de nuevas tecnologías incluidas en el PBS-UPC en los años 2012-2016, de cada EAPB, en cada grupo de condiciones de salud, y para cada año. Los resultados el modelo econométrico, sin ajustar por las variables instrumentales, arrojan un coeficiente entre el gasto y los AVG de 0,0933 no estadísticamente significativo, lo que implica un umbral de costo-efectividad de $8.772.177 por AVG. Teniendo en cuenta la variable instrumental de estados financieros, la estimación arrojó un coeficiente de 5,521 estadísticamente significativo a un nivel de confianza del 95% y un umbral de $157.775 por AVG. Por su parte, la estimación con la variable instrumental del gasto per cápita de nuevas tecnologías incluidas en el PBS-UPC, no arrojó resultados lógicos ni estadísticamente significativos. Teniendo en cuenta estos escenarios, se considera que la mejor estimación del umbral de costo-efectividad para el sistema de salud colombiano es de $8.772.177 por AVG. Al conocimiento del autor de este trabajo, esta es la primera estimación empírica del umbral por AVG desde el enfoque de la oferta para Colombia, Latinoamérica, y para cualquier país de ingresos bajos y medios. Las principales limitaciones de este trabajo se presentan a continuación. En primer lugar, la estimación se limita al universo del régimen contributivo y de las tecnologías incluidas en el PBS-UPC, por lo que una línea de investigación futura, si las fuentes de información lo permiten, sería la inclusión del estos dos universos dentro del modelo propuesto. En segundo lugar, en la literatura se recomiendan otras medidas diferentes a los AVG, como los Años de Vida Ajustados por Calidad (AVAC) y Años de Vida Ajustados por Discapacidad (AVAD), que podrían ser tenidas en cuenta para estimar umbrales para diferentes desenlaces en salud. En tercer lugar, una limitación de este trabajo se relaciona en la imposibilidad de identificar y cuantificar un buen instrumento que corrija la endogeneidad esperada entre el gasto y los beneficios en salud. Estimaciones futuras podrían modificar el valor del umbral calculado si se encuentra un instrumento adecuado y con información suficiente.
Abstract
This paper develops an initial estimation of the health cost-effectiveness threshold in the Colombian health system, from the perspective of the supply-side. This approach defines a theoretical and empirical framework, explicit and consistent, that can be replicated in different health settings. In the supply-side approach, the threshold is defined as the opportunity cost of allocating resources to a new technology, in terms of the health benefits displaced or forgone as a consequence of their unavailability to finance other alternatives that compete for the same budget; and its estimate is based on the quantification of the causal effect of the spending on health benefits. To carry out the research, an unbalanced panel-type econometric model with three dimensions was developed: 1) the Colombians insurers Entidades Administradoras de Planes de Beneficios (EAPB); 2) different groups of health conditions, based on the International Shortlist for Hospital Morbidity Tabulation of the World Health Organization; and 3) a temporary period from 2012 to 2016. The health outcome used were the Life-Years Gained (LYG), calculated using life tables, illness adjusted life expectancy and survival curves methodologies. The expected endogeneity was tried to be resolved through an instrumental variables approach. The estimation was carried out using two-stage ordinary last squares. To run the model, a novel database was built from multiple institutional sources available in the country from the period 2012-2016: Estudios de Suficiencia, Estadísticas Vitales, Registro Individual de Prestación de Servicios, Base de Datos Única de Afiliados, and the financial information of the Superintendencia de Salud. Due to the availability of the information, the estimate was delimited to the universe of the contributory regime and the technologies included in the Plan de Beneficios con cargo a la Unidad de Pago por Capitación (PBS-UPC). As instrumental variables, we explore the use of the financial reports of the EAPB each year, and the per capita expenditure of new technologies included in the PBS-UPC between 2012 and 2016 for each EAPB, each health conditions groups, and each year. The results of the econometric model, without adjusting for the instrumental variables, calculate a coefficient between the expenditure and the LYG of 0.0933 not statistically significant, which implies a cost-effectiveness threshold of $8,772,177 per LYG. Taking into account the instrumental variable of financial reports, the estimated coefficient was 5,521 statistically significant at a 95% confidence level, and a threshold of $157,775 per LYG. On the other hand, the estimation with the instrumental variable of the per capita expenditure of new technologies included in the PBS-UPC between 2012 and 2016, did not yield logical and statistically significant results. Taking into account this scenarios, it is considered that the best initial estimation of the cost-effectiveness threshold in the Colombian health system is $8,772,177 per LYG. To our knowledge, this is the first empirical estimate of the threshold per YLG from the supply-side approach for Colombia, Latin America, and for any low-middle income country. The main limitations of this paper are detailed below. First, the estimate is limited to the universe of the contributory regime and the technologies included in the PBS-UPC, so a future research agenda, if the information sources allow it, would be the inclusion of these two universes within the proposed model, Second, other measures beside the LYG are recommended in the literature, such as the Quality-Adjusted Life Years (QALY) and the Disability-Adjusted Life Years (DALY), which could be taken into account to estimate thresholds for different health outcomes. Third, a limitation of this paper is related to the impossibility in identifying and quantifying a good instrument that corrects the expected endogeneity between spending and health benefits. Future research could modify the estimation of the threshold if an appropriate instrument with the sufficient information is found.
