Falla terapéutica de la Penicilina Benzatinica en Sifilis gestacional. Revisión sistemática y metaanálisis

Resumen
Antecedentes: La sífilis es una infección bacteriana que se trasmite por contacto sexual tratable y curable con el uso de antibiótico de primera elección (penicilina benzatínica). En la actualidad no han sido estudiados los casos de falla terapéutica con los regímenes recomendados para la disminución en la incidencia de la sífilis congénita. Objetivo: Estimar la prevalencia de la falla terapéutica del tratamiento con penicilina benzatínica en sífilis gestacional mediante una revisión sistemática y metaanálisis. Estrategias de búsqueda Fuentes de búsqueda: Se buscó en el Registro Cochrane Central de Ensayos Controlados (The Cochrane Library Número 1, 2018), PUBMED y EMBASE, artículos publicados desde 1990 en idioma inglés y español. Criterios de selección: Estudios observacionales de tipo cohortes, ensayos clínicos controlados aleatorizados y no aleatorizados, casos y controles y reporte de casos que evaluaban las fallas terapéuticas en el tratamiento de la penicilina benzatínica en sífilis Gestacional. Resultados principales: En total, 6 estudios fueron incluidos en esta revisión en donde participaron 3435 mujeres con diagnóstico de sífilis gestacional. Se encontró una prevalencia de sífilis congénita del 11% con un IC 95% entre el 8% al 14% con un análisis de efectos aleatorios. Conclusiones de los revisores: La evidencia de los 6 estudios incluidos en esta revisión sugiere que la falla terapéutica de la penicilina benzatínica en la sífilis gestacional tiene una prevalencia importante frente al uso adecuado de los regímenes establecidos en las diferentes guías de práctica clínica contempladas.
Abstract
Background: Syphilis is a bacterial infection that is transmitted by sexual contact treatable and curable with the use of antibiotic of first choice (benzathine penicillin). At present, cases of therapeutic failure have not been studied with the recommended regimens for the decrease in the incidence of congenital syphilis. Objective: Estimate the prevalence of therapeutic failure of benzathine penicillin treatment in gestational syphilis through a systematic review and meta-analysis. Search strategies Search sources: We searched the Cochrane Central Registry of Controlled Trials (The Cochrane Library Number 1, 2018), PUBMED and EMBASE, articles published since 1990 in english and spanish. Selection criteria: Cohort observational studies, randomized and non-randomized controlled clinical trials, cases and controls, and case reports evaluating therapeutic failures in the treatment of benzathine penicillin in gestational syphilis. Main results: In total, 6 studies were included in this review involving 3435 women diagnosed with gestational syphilis. A prevalence of congenital syphilis of 11% was found with a 95% CI between 8% and 14% with a random effect’s analysis. Author´s conclusions: The evidence of the 6 studies included in this review suggests that benzathine penicillin therapeutic failure in gestational syphilis has an important prevalence compared to the appropriate use of the regimens established in the different clinical practice guidelines contemplated.
