Prevalencia de hiperglicemia perioperatoria y desenlaces en pacientes sometidos a trasplante hepático adulto

dc.description | INTRODUCCIÓN: La hiperglicemia perioperatoria en pacientes sometidos a trasplante hepático no ha sido evaluada en términos de riesgo en la población colombiana sometida a trasplante hepático y en la literatura mundial la evidencia es escasa y controvertida sin embargo los efectos observados son de relevancia clínica en el pacientes trasplantado. OBJETIVO: Determinar si existe asociación de riesgo con mortalidad, infección, NODAT y rechazo del injerto en los pacientes sometidos a trasplante hepático que presentan hiperglicemia en los años 2006-2017 en la FCI – IC. METODOLOGIA: Estudio observacional con componente analítico de cohorte histórica. Se dividieron dos cohortes que se denominaron Grupo 1 y Grupo 2 según si presentaron o no hiperglicemia perioperatoria respectivamente. Se realizó un análisis univariado y bivariado para descripción de la población y posteriormente un modelo de regresión logística de nuestra variable de agrupación con cada uno de los desenlaces primarios y secundarios. RESULTADOS: Se obtuvo el análisis sobre una población diana de 353 pacientes, la prevalencia observada de hiperglicemia perioperatoria fue de 41.6%, principalmente en pacientes no diabéticos. Con respecto a los desenlaces primarios y secundarios no se observó asociación independiente en cada uno de los modelos construidos, sin embargo se observó que la hiperglicemia durante las primeras 48 horas de manejo en UCI aumenta significativamente más el riesgo de NODAT. CONCLUSIONES: Los resultados demuestran que la hiperglicemia perioperatoria más que una causa es un efecto fisiopatológico y que el adecuado control metabólico atenúa los efectos de riesgo que han sido descritos en la literatura. | spa |
