dc.description | Introducción La Insuficiencia Renal Aguda (IRA) en los pacientes críticamente enfermos es una condición que se asocia a altas tasas de mortalidad. El estándar de tratamiento para esta condición ha sido la hemodiálisis intermitente (HI). Las Terapias de Reemplazo Renal Continua (TRRC) hemofiltración (HF) y hemodiafiltración (HDF) han surgido recientemente como terapias alternativas para este tipo de pacientes. Actualmente, las ventajas de la TRRC no se encuentran totalmente documentadas y en mucha menor proporción en nuestro medio. Este estudio pretende establecer si existen diferencias en los desenlaces asociados al tratamiento: mortalidad, tiempo de estancia hospitalaria asociada a TRRC y complicaciones entre los pacientes que recibieron HF ó HDF en la Clínica del Country, Bogotá, Colombia. Metodología Estudio tipo cohorte retrospectivo donde se involucraron pacientes con IRA hospitalizados en la UCI de la Clínica del Country entre enero 1ro de 2004 y marzo 30 de 2007. Se recolectaron datos relacionados con: características de los pacientes, condiciones clínicas asociadas y la TRRC recibida. Resultados Veintitrés pacientes fueron incluidos. Los dos grupos de tratamiento fueron similares en cuanto a edad, sexo, etiología de la IRA y puntaje APACHE II. La mortalidad fue mayor entre los pacientes de HF (RR=2,44, p=NS), así como el tiempo de estancia hospitalaria asociada a la TRRC (12,6 vs 5,1 días. p=NS). El RR de hipotermia fue 4,58 (p=NS) en el grupo de HF comparado con el HDF. Conclusiones La HDF podría estar asociada a mejores desenlaces para los pacientes. Se requieren estudios prospectivos que generen mejor nivel de evidencia al respecto. | spa |
dc.description.abstract | Introduction Acute Renal Failure (ARF) in critically ill patients is a condition that is associated with high mortality rates. The standard of treatment for this condition has been intermittent hemodialysis (HI). Hemofiltration (HF) and haemodiafiltration (HDF) Continuous Renal Replacement Therapies (CRRT) have recently emerged as alternative therapies for this type of patient. Currently, the advantages of CRRT are not fully documented and to a much lesser extent in our setting. This study aims to establish whether there are differences in the outcomes associated with treatment: mortality, length of hospital stay associated with CRRT and complications among patients who received HF or HDF at the Clínica del Country, Bogotá, Colombia. Methodology Retrospective cohort study involving patients with ARF hospitalized in the ICU of the Clínica del Country between January 1, 2004 and March 30, 2007. Data related to: patient characteristics, associated clinical conditions and CRRT received were collected. Results Twenty-three patients were included. The two treatment groups were similar in terms of age, sex, etiology of the ARI, and APACHE II score. Mortality was higher among HF patients (RR = 2.44, p = NS), as well as the length of hospital stay associated with CRRT (12.6 vs. 5.1 days. P = NS). The RR of hypothermia was 4.58 (p = NS) in the HF group compared to HDF. Conclusions HDF could be associated with better outcomes for patients. Prospective studies that generate a better level of evidence in this regard are required. | spa |
