dc.description | La “tríada terrible del codo” (luxación con fractura de la cúpula radial y apófisis coronoides) presenta alto grado de dificultad en el manejo con resultados variables1, debido a las lesiones ligamentarias asociadas que producen inestabilidad articular y perdida funcional. No hay descripción de estas en la literatura médica, ni estudios suficientes sobre la tríada misma, que incluyan la resonancia magnética nuclear (RMN) como ayuda diagnóstica. El propósito del estudio es determinar las lesiones óseas y ligamentarias presentes en los individuos con tríada terrible del codo, mediante los resultados de imágenes diagnósticas incluyendo la RMN, permitiendo orientar el tratamiento idóneo con los beneficios adecuados posteriores. Metodología: La población objeto de estudio fueron 39 pacientes con tríada terrible del codo a quienes se realizó como protocolo: evaluación clínica, radiografía convencional previa y posterior a la reducción, tomografía axial computarizada (TAC) y RMN. Las imágenes fueron valoradas por ortopedistas y por radiólogo especializado en sistema osteomuscular. Resultados: Se identificaron lesiones del ligamento colateral lateral radial (71,8%) principalmente y del complejo ligamentario interno (26,9%), siendo concomitantes en un 20,5% de los casos, y en 8 de los individuos presentaron fractura de olécranon concomitantemente. Adecuada concordancia entre la Radiografía, TAC y RMN y la relación de los hallazgos con la conducta de tratamiento fue predominantemente quirúrgica. Conclusiones: La radiografía simple y TAC identifican las lesiones óseas, pero no las ligamentarias, que se determinaron mediante RNM, orientando una adecuada decisión terapéutica para evitar inestabilidades o complicaciones posteriores. | spa |
dc.description.abstract | The "terrible triad of the elbow" (dislocation with radial head fracture and coronoid process fracture) presents a high degree of difficulty in management with variable results, due to the associated ligament injuries that produce joint instability and functional limitation. There is no description of these in the medical literature, also there isn’t sufficient studies on the triad itself, including magnetic resonance imaging (MRI) as diagnostic aid. The purpose of the study is to determine the bone and ligament injuries in individuals with a terrible triad of the elbow, through the results of diagnostic imaging including MRI, allowing the pertinent treatment with the appropriate posterior benefits. Methods: The study population consisted of 39 patients with a terrible triad of the elbow who underwent this protocol: clinical evaluation, pre and post-reduction conventional x-Ray, computed tomography (CT) and MRI. The images were evaluated by orthopedists and by a specialized radiologist in the musculoskeletal system. Results: The injuries of the radial lateral collateral ligament (71.8%) and internal ligament complex (26.9%) were identified, being 20.5% concomitant, and 8 of the individuals present olecranon fracture concomitantly. Adequate concordance between x-Ray, CT and MRI and the relationship of the findings to the treatment behavior was predominantly surgical. Conclusions: Simple x-Ray and CT scan identify bone injuries, but not the ligament ones, which were determined by MRI, orienting an appropriate therapeutic decision to avoid subsequent instabilities or complications. | eng |
