dc.description | Objetivo: Determinar la proporción de pacientes y hogares que experimentan costos catastróficos derivados de la tuberculosis. Métodos: Revisión sistemática en tres bases de datos y fuentes adicionales en el periodo comprendido entre enero de 2009 y octubre de 2020. Los costos catastróficos fueron analizados bajo el umbral de ≥20% del ingreso anual del hogar de una persona afectada por tuberculosis. Resultados: Se identificaron 7487 artículos y 19 publicaciones adicionales en otras fuentes. Después de la tamización y revisión de elegibilidad, 40 artículos fueron incluidos en la síntesis cualitativa y ninguno en la síntesis cuantitativa. Los artículos incluidos provenían de 25 países, la mayor parte de Asia y África. Se encontró gran heterogeneidad en los resultados. El porcentaje de costos catastróficos osciló entre 4% y 83%. Sin embargo, este porcentaje llegó a ser hasta del 100% cuando el análisis era específico para tuberculosis resistente. Conclusiones: Independientemente de la heterogeneidad de los estudios incluidos, un porcentaje importante de pacientes con tuberculosis afrontan costos catastróficos. Un mayor porcentaje de pacientes y hogares enfrentan costos catastróficos cuando hay coinfección con VIH o tuberculosis resistente. La detección activa puede tener efectos favorables en los costos que enfrentan los pacientes. | spa |
dc.description.abstract | Objective: Determine the proportion of patients and households that experience catastrophic costs due to tuberculosis. Methods: Systematic review in three databases and additional sources between January 2009 and October 2020. Catastrophic costs were analyzed under the threshold of ≥20% of the annual household income of a person affected by tuberculosis. Results: 7487 articles and 19 additional publications were identified. After screening and eligibility review, 40 articles were included in the qualitative synthesis and none in the quantitative synthesis. The articles included came from 25 countries, most of Asia and Africa. High heterogeneity was found in the results. The percentage of catastrophic costs ranged between 4% and 83%. However, this percentage was up to 100% when the analysis was specific for resistant tuberculosis. Conclusions: Regardless of the heterogeneity of the included studies, a significant percentage of patients with tuberculosis face catastrophic costs. A higher percentage of patients and households face catastrophic costs when there is co-infection with HIV or resistant tuberculosis. Active detection can have a favorable effect on the costs faced by patients. | spa |
