dc.description | Introducción: Existen en la actualidad múltiples intervenciones en cirugía vascular que pueden mejorar el pronóstico. Se pretendió describir frecuencia de amputación, infección y necesidad de reintervención en pacientes con isquemia crítica de miembros inferiores sometidos a revascularización, durante el periodo 2018 – 2019 en la Fundación Cardioinfantil Instituto de Cardiología. Materiales y métodos: Estudio de corte transversal, que incluyó todos los pacientes con isquemia crítica de miembros inferiores que de manera electiva fueron llevados a uno de dos procedimientos para revascularización de miembro inferior, sin la intención de ser comparados entre sí. Se seleccionó una muestra por conveniencia, y se obtuvo información de las características sociodemográficas y resultados clínicos de la base de datos del servicio de cirugía vascular y angiología. Resultados: Se evaluaron 87 casos, con un promedio de edad 70 años ±11 años. Se realizó cirugía abierta en 37 pacientes y terapia endovascular en 48 pacientes para revascularización. La decisión de la técnica fue según la clasificación de WIfI y TASC II. Los resultados fueron: amputación (20.6%), infección de herida (18.3%), necesidad de reintervención (8.5%) y muerte en el (8.05%) . No hubo diferencias significativas, en la frecuencia de los desenlaces entre ambas técnicas. Conclusiones: Este estudio demostró desenlaces similares en el manejo con ambas técnicas de revascularización endovascular y cirugía abierta. La tasa de amputación y de infección no tuvo diferencias entre los grupos. Se evidenció una asociación significativa en pacientes con diabetes y/o 3 o más comorbilidades con muerte. dc.description.abstract | Background: Critical lower limb ischemia is defined as a clinical syndrome of ischemic pain at rest and loss of tissue, associated with ulcers that are difficult to heal, and/or gangrene. There are currently multiple interventions in vascular surgery that can improve prognosis. We pretended to describe the clinical outcomes in patients with critical lower limb ischemia undergoing lower limb revascularization in a highly complex institution, during the period 2018-2019. Methods: A cross-sectional study was performed, in which we included all patients with chronic lower limb ischemia who were electively brought to lower limb revascularization from January 2018 to December 2019. A convenience sample was selected, and their medical records were reviewed. Sociodemographic, imaging and clinical data are presented as results in a database of the vascular surgery and angiology service. Results: 87 patients were evaluated, with an average age of 70 years ± 11 years. The technique was endovascular in 48 patients and an open technique in 37 patients. Among the most frequent complications were the need for amputation 20. 6% and wound infection 18. 3%. The factors associated with complications were the presence of DM, greater severity of the disease and a history of AMI. Conclusions: The data analysis showed similar outcomes in the management with both techniques. The rate of amputation and infection of the operative site did not differ between the types of management performed.
