Flacidez del tercio medio e inferior de la cara y uso de hilos de polidioxanona : serie de casos y descripción de una técnica

dc.description | Antecedentes: La flacidez o laxitud de la cara es considerada una de las características que más contribuye a la apariencia de envejecimiento facial. Con el paso del tiempo, las investigaciones han demostrado que la laxitud compromete a todos los tejidos faciales: piel, tejido celular subcutáneo, ligamentos de retención, sistema músculo aponeurótico superficial e inclusive al soporte óseo. La cirugía ha sido considerada el tratamiento “Gold” estándar para la flacidez pero debido a la mayor tendencia de los pacientes a realizase procedimientos mínimamente invasivos con la consecuente disminución de los riesgos quirúrgicos, ha aumentado el uso del levantamiento (“lifting”) facial con hilos como respuesta a esta tendencia. Los hilos absorbibles de Polidioxanona se presentan como una buena opción para el levantamiento de los tejidos faciales flácidos por medio de la mejoría de la piel, a través de la inducción de la síntesis de colágeno y la tracción ejercida por la sutura a nivel del tejido celular subcutáneo. Metodología: serie de casos, descripción de una técnica con hilos de Polidioxanona como tratamiento de la flacidez del tercio medio e inferior de la cara y descripción de los resultados, a través de la estadificación de la flacidez con la escala validada de clasificación de laxitud facial (FLR) al inicio del tratamiento y tres meses posterior a éste. Resultados: El uso de la escala validada de Clasificación de la Flacidez Facial (FLR) como herramienta de comparación permitió tener resultados objetivos. Se observó un cambio estadísticamente significativo en cuanto a la mejoría de la flacidez del tercio medio e inferior de la cara con el tratamiento con hilos de Polidioxanona. Lo cual coincide con un elevado porcentaje de satisfacción (78,5%) entre buena (35,7%), muy buena (35,7%) y excelente (7,1%). Se observó que mas del 50% de los pacientes el grado de dolor experimentado con el procedimiento fue leve a moderado. Solo un paciente (7,1%) reportó el dolor como moderado a severo, haciendo de éste un procedimiento muy tolerable. El reporte de complicaciones en esta serie es similar a la de otros estudios, siendo principalmente complicaciones menores, entre otras: disestesias, edema, eritema. Conclusión: el tratamiento con hilos de Polidioxanona es una opción significativa para el manejo de la flacidez del tercio medio e inferior de la cara. La descripción de nuevas técnicas de aplicación de los hilos y de sus resultados, abre las puertas a nuevas opciones terapéuticas en el campo de la Medicina Estética para el tratamiento de la flacidez facial. Se requerirán estudios con mayores niveles de evidencia para su recomendación. | spa |
dc.description.abstract | Flaccidity or laxity of the face is considered one of the characteristics that most contributes to the appearance of facial aging. Over time, research has shown that laxity involves all facial tissues: skin, subcutaneous cellular tissue, retention ligaments, superficial aponeurotic muscle system and even bone support. Surgery has been considered the Gold standard treatment for flaccidity but due to the greater tendency of patients to perform minimally invasive procedures with the consequent reduction of surgical risks, the use of facial lifting with threads had increased in response to this trend. The absorbable threads of Polidioxanone are presented as a good option for the lifting of the flaccid facial tissues through the improvement of the skin, through the induction of collagen synthesis and by the traction exerted by the suture, at the level of the tissue subcutaneous cellular. Methodology: series of cases, description of a technique with Polydioxanone threads as a treatment of the flaccidity of the middle and lower third of the face and description of the results, through the staging of flaccidity with the validated scale of facial laxity classification (FLR) at the start of treatment and three months after it. Results: The use of the validated scale of Facial Flaccidity Classification (FLR) as a comparison tool allowed to have objective results. A statistically significant change was observed in the improvement of the flaccidity of the middle and lower third of the face with the treatment with Polydioxanone threads. This coincides with a high percentage of satisfaction (78.5%) between good (35.7%), very good (35.7%) and excellent (7.1%). It was observed that more than 50% of the patients the degree of pain experienced with the procedure was mild to moderate. Only one patient (7.1%) reported pain as moderate to severe, making this a very tolerable procedure. The report of complications in this series is similar to that of other studies, being mainly minor complications, among others: dysesthesias, edema, erythema. Conclusion: treatment with Polydioxanone threads is a significant option for the management of sagging of the middle and lower third of the face. The description of new techniques for the application of threads and their results opens the doors to new therapeutic options in the field of Aesthetic Medicine for the treatment of facial sagging. Studies with higher levels of evidence will be required for their recommendation. | spa |
