Análisis de la pertinencia de escalas de evaluación cognitiva y de funcionamiento ocupacional para el tamizaje y diagnóstico de alteraciones neurocognitivas asociadas al VIH. revisión sistemática de la literatura

dc.descriptionEl Desorden Neurocognitivo Asociado a VIH (HAND) se reconoce como una problemática actual en población que vive con VIH. Los procesos de evaluación para el tamizaje y diagnóstico del HAND son heterogéneos y escasos de consenso. Esto ha generado diferencias en el reporte de prevalencias a nivel mundial y dificultades en la identificación de datos comparativos. Se realizó una revisión sistemática de literatura para determinar la pertinencia de escalas de evaluación cognitiva y de funcionamiento ocupacional para el tamizaje y diagnóstico de HAND analizando dominios cognitivos, tamizaje, participación ocupacional y salud mental. Se incluyeron 114 estudios de corte transversal, casos y controles, cohorte y ensayos clínicos. Se identificaron las escalas Trail Making Test (TMT) y Stroop Color Word Test (SCWT) como las de mayor pertinencia para la evaluación de dominios cognitivos. A nivel de participación ocupacional se requiere mayor uso de instrumentos dirigidos a evaluar el desempeño ocupacional durante la ejecución de actividades, que complementen las escalas de autorreporte como el Índice de Lawton y Brody y el Inventario para la Valoración del Propio Funcionamiento del Paciente (PAOFI). En salud mental se determina como una variable a evaluar de manera transversal por considerarse un factor de confusión en el diagnóstico de HAND.spa
dc.description.abstractHIV-Associated Neurocognitive Disorder (HAND) is recognized as a current issue in people living with HIV (PLWH). Assessment processes for screening and diagnosis of HAND are heterogeneous y lack of consensus. This has triggered differences in prevalence report across the word and difficulties in the identification of comparable data. A systematic review was conducted to determine pertinence of cognitive assessment and occupational functioning scales for HAND screening and diagnosis analyzing cognitive domains, screening, occupational participation and mental health. There were 114 cross-sectional, case and control, cohort, and clinical trials studies included in the systematic review. Trail Making Test (TMT) and Stroop Color Word Test (SCWT) scales were identified as the most pertinent para cognitive domains assessment. As for occupational participation, instruments focusing on evaluation occupational performance during the execution of activities are needed, for complementing self-report scales such as The Patients Assessment of Own Functioning Inventory (PAOFI). Mental health is determined as a variable to evaluate transversally, due to the fact that is considered a confounding in the diagnosis of HAND.spa
