Relación entre nivel educativo y edad de aparición de la enfermedad de Alzheimer, revisión sistemática

dc.descriptionIntroducción: La enfermedad de Alzheimer (EA) es una patología grave e insidiosa, que involucra un proceso neurodegenerativo catastrófico irreversible que termina con la muerte. El origen de la EA no obedece a una única razón etiológica, sino que obedece a diversas naturalezas que pueden ir desde las alteraciones a nivel genético, la influencia ambiental, los modos de vida y condiciones demográficas como el sexo y la escolaridad. Objetivo: Describir la relación entre el nivel educativo y la edad de inicio de la enfermedad de Alzheimer. Métodos: Revisión sistemática de literatura. Se realizó una búsqueda de estudios observacionales analíticos y literatura gris. El riesgo de sesgo se evaluó aplicando los instrumentos de evaluación crítica desarrolladas por el Instituto Joanna Briggs. Resultados: Se seleccionaron siete estudios, seis emplearon diseño transversal y un estudio de casos y controles anidado en una cohorte. Ninguno de los estudios reporta herramientas de cálculo de muestra. El diagnóstico de enfermedad de Alzheimer se basó en los criterios del NINCDS, DSM – IV, y NIA-AA. El 70% de los sujetos de estudios fueron mujeres. El nivel educativo de los sujetos varió entre los 0 y 19 años de escolarización formal, siendo las mujeres, afroamericanos e hispanos los grupos poblacionales con menor nivel académico. La edad promedio de aparición de la demencia producida por enfermedad de Alzheimer osciló entre 67,3 y 86,5 años. No se encontraron diferencias significativas entre la edad de aparición de demencia entre hombres y mujeres. Conclusiones: No hay consenso ni evidencia suficiente que permita asegurar una asociación entre el nivel educativo y la edad de aparición de la enfermedad de Alzheimer esporádica. Se hace necesario un mayor nivel de evidencia para establecer la asociación entre estas variables.spa
dc.description.abstractIntroduction: Alzheimer's disease (AD) is a serious and insidious pathology, which involves an irreversible catastrophic neurodegenerative process that ends with death. The origin of AD is not due to a single etiological reason but is due to various natures that can range from genetic alterations, environmental influence, lifestyles, and demographic conditions such as sex and education. Objective: To describe the relationship between educational level and age of onset of Alzheimer's disease. Methods: Systematic literature review. A search for analytical observational studies and a gray literature search was performed. The risk of bias was assessed by applying the critical appraisal instruments developed by the Joanna Briggs Institute. Results: Seven studies were selected, six used a cross-sectional design and one a case-control study nested in a cohort. The educational level of the subjects varied between 0 and 19 years of formal schooling. The average age of onset of dementia caused by Alzheimer's disease ranged from 67.3 to 86.5 years. Seven studies were selected, six used a cross-sectional design and a nested case-control study in a cohort. None of the studies report sample calculation. The diagnosis of Alzheimer's disease was based on the NINCDS, DSM-IV, and NIA-AA criteria. 70% of the study subjects were women. The educational level of the subjects ranged from 0 to 19 years of formal schooling. The women, African Americans and Hispanics being the population groups with the lowest academic level. The average age of onset of dementia caused by Alzheimer's disease ranged from 67.3 to 86.5 years. No significant differences were found between the age of onset of dementia between men and women. Conclusions: There is no consensus or sufficient evidence to ensure an association between educational level and the age of onset of sporadic Alzheimer's disease. A higher level of evidence is necessary to establish the association between these variables.spa
