Resumen
Las fracturas de pelvis inestables con indicación de manejo quirurgico pueden utilizar como guia de procedimiento el fluoroscopio o la Tomografia Axial Computarizada, con este último se considera disminuirían los riesgos de mala fijación, lesión neurovascular y de órganos a nivel de la pelvis y columna sacra. Objetivo. Establecer las complicaciones intra-operatorias y postoperatorias de la fijación interna con tornillos sacro-iliacos guiados por TAC en las fracturas sacroliacas de la pelvis. Materiales y métodos. Estudio de cohorte retrospectiva de 14 pacientes con fractura sacro iliaca a quienes se les realizo fijación percutánea mediante TAC en el hospital universitario Fundación Santa fe de Bogotá y el Hospital Infantil San José durante el periodo 2012 a 2018. Resultados. En el intraoperatorio el tiempo quirúrgico promedio fue de 84.6 minutos, el sangrado intraoperatorio de 7.7 ml, ningún paciente presento mal posición de los tornillos, con un desplazamiento de la fractura < 5 mm, la estancia hospitalaria promedio fue de 8.7 días, no requirieron re intervención durante el tiempo de seguimiento Conclusión. La fijación percutánea de fracturas sacro iliacas guiadas por tomografía, no presento complicaciones intra quirúrgicas y en el seguimiento de los pacientes, con adecuada fijación y desplazamiento mínimo de las fracturas sin haber requerido re intervención quirúrgica. Abstract
Unstable pelvic fractures and especially iliac sacral fractures, can endanger the patient's life, so that an early stabilization of these, using an external fixator and then the final fixation of the same by means of iliac sacral screws Percutaneously, either by fluoroscope or computed tomography, is the standard in the treatment of these patients. Fluoroscope-guided fixation is the most widely used method, since the technical requirement is more feasible, however, it is less precise for the proper position of these screws, unlike CAT, so using the latter, the risks of poor fixation and neurovascular and organ injury at the level of the pelvis and sacral spine. Objective. To establish the intra-operative and postoperative complications of internal fixation with CT-guided sacro-iliac screws in sacroliac pelvic fractures. Patients and methods. Retrospective cohort study of 14 patients with iliac sacral fracture who underwent percutaneous fixation by CT scan at the Fundación Santa Fe de Bogotá University Hospital and the San José Children's Hospital during the period 2012 to 2018. Results. In the intraoperative period, the average surgical time was 81.4 minutes, intraoperative bleeding of 9.61 ml, no patient presented poor screw position, with a fracture displacement of <5 mm, the average hospital stay was 8.72 days, and they did not require re intervention during follow-up time Conclusion. The percutaneous fixation of tomography-guided sacral iliac fractures did not present intra-surgical complications and in the follow-up of the patients, with adequate fixation and minimal displacement of the fractures without requiring surgical intervention.
