Estudio observacional, descriptivo y retrospectivo cuyo objetivo es caracterizar las complicaciones y desenlaces anestésicos perioperatorios en pacientes llevados a procedimientos de neumología intervencionista, provenientes de 2 instituciones: FCI y FNC, durante el periodo de enero y octubre de 2022.
Abstract
Introduction: Interventional pulmonology procedures are essential for the diagnosis and treatment of airway diseases. Performed under bronchoscopic and endobronchial ultrasound guidance, they trigger pain and autonomic responses by activating the airway protective reflexes; therefore, they require deep sedation or general anesthesia, both of which carry inherent risks. This study aimed to characterize anesthetic complications and outcomes in patients at the Fundación Neumológica Colombiana (FNC) and Fundación CardioInfantil (FCI) between January and October 2022 undergoing these procedures. Methods: This retrospective, descriptive, cross-sectional observational study included patients aged ≥18 years who underwent interventional pulmonology procedures at FNC and FCI during the study period. Variables were analyzed using frequencies or measures of central tendency and dispersion with R software version 4.2.1. Results: A total of 971 procedures were included, with 51.6% performed in women. The median age was 62 years (IQR, 50–72). Most patients were classified as ASA II (48.71%) or ASA III (47.06%). General anesthesia was used in 98.76% of procedures, a laryngeal mask airway in 87.54%, and total intravenous anesthesia (TIVA) with propofol and remifentanil in 99.49% and 97.73% of cases, respectively. The procedure was successfully completed in 98.87% of cases. Anesthetic complications occurred in 1.03% of procedures (mainly bronchospasm), procedural complications in 1.75% (mainly hemoptysis/bleeding), and perioperative mortality was 0.1%. Discussion: This represents one of the largest Colombian case series on anesthesia for interventional pulmonology. Despite the high burden of comorbidities, the cohort demonstrated a favorable safety profile, with a low incidence of complications and perioperative mortality, providing a foundation for future analytical studies.
