Caracterización de membranas epiretinianas en oclusiones venosas de la retina mediante angiotomografía de coherencia óptica

dc.description | Objetivo: Evaluar las características clínicas y angiotomográficas (OCT-A) de los pacientes con membranas epiretinianas (MER) secundarias a oclusiones venosas de la retina (OVR). Métodos: Estudio observacional de corte transversal. Se revisaron retrospectivamente imágenes de OCT-A e historias clínicas de 354 pacientes con diagnóstico de OVR, de los cuales 33 ojos de 33 pacientes cumplieron criterios de inclusión. Se calcularon medidas de tendencia central y medidas de dispersión de las variables cuantitativas según su distribución (kolmogorov-smirnov). Las variables cualitativas fueron descritas en frecuencias y porcentajes. Resultados: Fueron incluidos en el análisis treinta y tres ojos. El promedio de edad fue 68,7 años (DE: 12,4) con un 54.5% de mujeres (n:18). La mediana de agudeza visual (AV) LogMAR 0,70 (RIQ: 1,00). El 57,6% (n:19) presentaron oclusión de vena central, 33,3% (n:11) oclusión de rama venosa y el 9,1% (n:3) oclusión hemicentral. El tiempo entre el diagnóstico de OVR y el de MER fue en promedio de 14,4 meses (RIQ: 26) y previo al diagnóstico de MER habían recibido 3,8 (DE:3,7) inyecciones de antiangiogénico. El grosor retiniano central (GRC) promedio fue de 327 um (DE: 132,3) con disrupción en la integridad de la membrana limitante externa (MLE) y zona elipsoide (ZE) en 60,6% (n:20) y 66.7% (n:22) respectivamente. Hubo presencia de espacios cistoides intraretinianos en la mayoría de pacientes (60,6% n:20) y desorganización de capas retinianas internas (DRIL) en 42,2% (n:14). Se encontraron capas foveales internas ectópicas solo en 12.1% (n:4) de los ojos. La localización de las MER fue en su mayoría extrafoveal (75,7% n: 25) con un patrón de adherencia a la superficie de la retina mixto (global y focal) en un 45,5% (n: 15) de los casos. La zona avascular foveal (ZAF) total fue de 280 um (RIQ: 221um). Se observó que los ojos con MER foveal y disrupción en la MLE y ZE mostraron una tendencia a tener peor AV. Conclusiones: Las características estructurales y vasculares en la retina de pacientes con MER secundarias a OVR corresponden a cambios propios de la patología vascular. Este grupo de pacientes no comparten las características angiotomográficas descritas para MER idiopáticas. La disrupción de la MLE y ZE puede estar asociado con peor agudeza visual. | spa |
dc.description.abstract | Purpose: To evaluate clinical and angiotomography (OCT-A) characteristics of patients with epiretinal membranes (ERM) secondary to retinal vein occlusions (RVO). Method: Cross sectional study. OCT-A images and medical records of 354 patients diagnosed with RVO were retrospectively reviewed, of which 33 eyes from 33 patients met inclusion criteria. Measures of central tendency and measures of dispersion of quantitative variables according to their distribution (kolmogorov-smirnov) were calculated. The qualitative variables were described in frequencies and percentages. Results: The mean age was 68.7 years (SD: 12.4) with 54.5% women (n: 18). Median visual acuity (AV) LogMAR 0.70 (IQR: 1.00). 57.6% (n: 19) had a diagnosis of central retinal vein occlusion (CRVO) 33.3% (n: 11) branch retinal vein occlusion (BRVO) and 9.1% (n: 3) hemicentral vein occlusion (HVO). The time between the diagnosis of RVO and ERM averaged 14.4 months (IQR: 26), and previous to the diagnosis of ERM received 3.8 (SD: 3.7) antiangiogenic injections. The mean central retinal thickness (CRT) was 327 um (SD: 132.3) with disruption of the integrity of the external limiting membrane (ELM) and ellipsoid zone (EZ) in 60.6% (n: 20) and 66.7% % (n: 22) respectively. There was presence of intraretinal cystoid spaces in most patients (60.6% n: 20) and disorganization of the inner retinal layer (DRIL) in 42.2% (n: 14). Ectopic inner foveal layers (EIFL) were found only in 12.1% (n: 4) of the eyes. The ERM location was mostly extrafoveal (75.7% n: 25) with a mixed pattern of adherence to the retinal surface (global and focal) in 45.5% (n: 15) of the cases. The mean foveal avascular zone (FAZ) was 280 um (IQR: 221um). Eyes with foveal-involving MER and disruption in MLE and ZE show a tendency to have worse VA. Conclusions: The structural and vascular retinal characteristics of patients with ERM secondary to RVO correspond to inherent changes to vascular pathology. This group of patients does not share the angiotomography characteristics described for idiopathic ERM. Disruption of the MLE and ZE may be associated with worse visual outcome. | spa |
