Hallazgos endoscópicos de la vía aérea superior en pacientes despiertos con síndrome de apnea hipopnea obstructiva del sueño y ronquido simple. Revisión sistemática de la literatura

dc.descriptionEl síndrome de apnea hipopnea del sueño es una enfermedad con una alta prevalencia en la población pediátrica y adulta. El diagnóstico y tratamiento temprano de esta condición evita el desarrollo de complicaciones a quienes la padecen, siendo las más comunes las enfermedades cardiovasculares. La evaluación endoscópica de las vía aérea superior de estos pacientes es un examen fundamental para identificar las estructuras que pueden estar contribuyendo al colapso de la vía aérea o que pueden estar dificultando la adaptación del paciente a los dispositivos de presión positiva. Este examen endoscópico puede ser realizado con el paciente despierto o sedado. El examen con paciente despierto tiene como objetivo realizar una evaluación dinámica de la vía aérea que busca reproducir el colapso que se presenta durante el sueño. Se han diseñado un gran número de técnicas y maniobras así como métodos que permitan describir los hallazgos obtenidos de esta evaluación. Hasta la fecha no se ha logrado desarrollar una metodología ni nomenclatura que sea universalmente aceptada por los médicos para describir estos hallazgos. Por esta razón, se ha decido realizar una revisión sistemática de la literatura de los últimos 10 sobre los hallazgos endoscópicos de la vía aérea de pacientes con SAHOS y ronquido simple, buscando recopilar toda la información disponible sobre el tema y brindando al lector los herramientas necesarias para definir cuales quiere adoptar para su práctica diaria según el grado de evidencia o recomendación que se encuentre a cada una de ellas. Adicionalmente su realiza una comparación del rendimiento de esta técnica con la prueba de oro actual (endoscopia de la vía aérea bajo sedación DISE) y se describen las ventajas y desventajas de cada una de estas a la luz de la evidencia actual.spa
dc.description.abstractThe apnea hypopnea syndrome is a disease with a high prevalence in the pediatric and adult population. The diagnosis and early treatment of this condition prevent the development of complications to patients, the most common being cardiovascular diseases. The endoscopic evaluation of the upper airway of these patients is a fundamental exam to identify structures that may be contributing to the collapse of the upper airway or that may hinder the adaptation of the patient to positive pressure devices. This endoscopic examination can be performed with the patient awake or sedated. The purpose is to perform a dynamic evaluation of the airway looking to reproduce the same events that occurs during sleep. A large number of techniques and maneuvers have been designed. To date, it has not been possible to develop a methodology or nomenclature that is universally accepted by physicians to describe these findings. For this reason, a systematic review of the literature of the last 10 years on the endoscopic findings of the airway of patients with OSAHS and simple snoring has been carried out, seeking to collect all available information on the subject and providing the reader with the services for that suits your daily practice, looking the advantages and disadvantages of each of these based in the current evidence.spa
