Corneal transplant epidemiology in a reference center in Bogotá, Colombia (2010‑2017)

dc.descriptionPropósito: Describir las características de donante y receptor, las indicaciones, técnicas quirúrgicas y resultados de sobrevida obtenidos en pacientes con trasplante de córnea realizado en la Fundación Oftalmológica Nacional en el periodo comprendido entre 2010 y 2017. Métodos: Estudio observacional descriptivo, donde se revisaron registros médicos de pacientes en quienes se realizó trasplante de córnea en la Fundación Oftalmológica Nacional (FUNDONAL), en Bogotá, Colombia, durante el periodo comprendido entre enero de 2010 y diciembre de 2017. Se evaluaron datos demográficos, indicación de cirugía, técnica de trasplante, transparencia del injerto y presencia de rechazo. Se realizó análisis descriptivo con SPSS versión 21.0. Resultados: Se analizaron 712 ojos de 690 pacientes en quienes se realizó trasplante de córnea. La mediana de edad del receptor fue de 42 años (RIC 21,8 años), 392 sujetos (56,8%) eran hombres. Las ectasias corneales fueron la primera indicación de trasplante. La técnica más realizada fue la Queratoplastia Penetrante (QPP) (89,6%), seguida de la Queratoplastia Lamelar Anterior profunda (DALK) (5,3%), Queratoplastia endotelial (3,5%) y Escleroqueratoplastia (1,5%). El 22,25% de los casos presentó al menos un episodio de rechazo durante el periodo estudiado. La transparencia del injerto fue superior al 80% hasta los 36 meses de seguimiento. Conclusiones: En 712 ojos intervenidos con trasplante de córnea en el periodo de 2010 a 2017, las indicaciones más frecuentes fueron las ectasias corneales, la queratopatía bullosa pseudofáquica/afáquica y la falla del injerto previo. La técnica más utilizada en esta cohorte fue la queratoplastia penetrante, sin embargo, se observa una tendencia a realizar más trasplantes lamelares con el paso de los años. Los resultados de sobrevida de 78% al último control y de rechazo del trasplante de 22,23% son equiparables con los reportados en la literatura internacional. El presente estudio constituye el primer registro de trasplante de córnea en Colombia, con resultados de sobrevida y descripción de variables que permitirán identificar factores de riesgo para desenlaces no deseados.es
dc.description.abstractPurpose: To describe the characteristics of the donor and recipient, indications, surgical techniques, and survival results obtained from patients with corneal transplants performed at Fundación Oftalmológica Nacional (FUNDONAL) between 2010 and 2017. Methods: Descriptive, observational study. Medical records of patients who underwent corneal transplantation were reviewed. Demographic data, surgery indication, transplantation technique, graft transparency, and presence of rejection are evaluated. Statistical analysis was performed with the SPSS software version 21.0. Results: Seven hundred and twelve eyes from 690 patients who underwent corneal transplantation were analyzed. The median age of the recipient was 42 years (interquartile range 21.8 years) and 392 participants (56.8%) were men. Corneal ectasias was the first indication for transplantation. The technique performed most often was penetrating keratoplasty (PK) (89.6%), followed by deep anterior lamellar keratoplasty (5.3%), endothelial keratoplasty (3.5%), and sclerokeratoplasty (1.5%). 22.25% of the cases presented at least one rejection episode. Transparency of the graft is >80% up to 36 months from follow‑up. Conclusions: In 712 eyes that underwent corneal transplantation between 2010 and 2017, the most frequent indications were corneal ectasias, pseudophakic/aphakic bullous keratopathy, and previous graft failure. Although the most common technique is PK, there is a tendency to perform more lamellar transplants over the years. Survival up to the last control was 78% with graft rejection at 22.23%, which are comparable with results reported in the scientific literature. This study constitutes the first corneal transplant registry in Colombia, with survival results and description of variables that will allow for the identification of risk factors for undesirable outcomes.es
