| dc.description | Introducción: Las infecciones de piel y tejidos blandos (IPTB) son uno de los principales motivos de consulta. Reconocer el papel del Staphylococcus aureus es fundamental para determinar el antibiótico empírico, ya que el incremento de cepas de S. aureus resistente a meticilina (SARM) ha generado controversias respecto al uso de betalactámicos como primera opción. Hay evidencia limitada sobre el manejo con estos antibióticos en contextos de alta prevalencia de SARM y de los factores asociados a falla terapéutica en población pediátrica. Objetivo: Describir la evolución clínica e identificar factores asociados a la falla terapéutica a beta-lactámicos en pacientes pediátricos hospitalizados con IPTB. Métodos: Estudio de cohorte retrospectiva realizado en una institución de tercer nivel en Bogotá, Colombia, entre 2020 y 2023, con pacientes pediátricos hospitalizados por IPTB, tratados empíricamente con oxacilina o cefalosporina de primera generación. Se definió falla terapéutica como la persistencia/empeoramiento de signos y síntomas o la necesidad de ampliar el espectro antibiótico en las primeras 48-72 horas de inicio del beta-lactámico. Se analizaron variables demográficas, clínicas y paraclínicas. Se empleó regresión logística para identificar factores asociados. Resultados: Se incluyeron 326 pacientes, el 8,6% (n=28) presentó falla terapéutica. No hubo asociación significativa con variables clínicas o demográficas. La falla terapéutica se asoció con mayor estancia hospitalaria (media 6,8 vs. 3,8 días, p<0.001), mayor duración de tratamiento intravenoso (media 5,6 vs. 3,2 días, p<0.001) y mayor aislamiento de SARM (21,4% vs. 1%, p<0.05). La mayoría de los casos evolucionaron favorablemente. Conclusión: Los beta-lactámicos son efectivos como tratamiento de elección para las IPTB en pediatría. La falla terapéutica es poco frecuente y se asocia principalmente con infecciones por SARM y cuadros más severos. Estos hallazgos respaldan el uso de beta-lactámicos y refuerzan la necesidad de estudios prospectivos para predecir la falla terapéutica. | |
| dc.description.abstract | Background: Skin and soft tissue infections (SSTIs) are one of the main reasons for medical consultation. Recognizing the role of Staphylococcus aureus is essential for choosing empirical antibiotic therapy. The increase in methicillin-resistant S. aureus (MRSA) strains has generated controversy regarding the use of beta-lactams. There is limited evidence on the use of these antibiotics in settings with high MRSA prevalence and on the factors associated with therapeutic failure in the pediatric population. Objective: To describe the clinical course and identify factors associated with therapeutic failure to beta-lactam antibiotics in hospitalized pediatric patients with SSTIs. Methods: A retrospective cohort study was conducted at a tertiary care institution in Bogotá, Colombia, between 2020 and 2023, involving hospitalized pediatric patients with SSTIs who were empirically treated with oxacillin or a first-generation cephalosporin. Therapeutic failure was defined as the persistence/worsening of signs and symptoms or the need to broaden antibiotic coverage within the first 48–72 hours after initiating beta-lactam therapy. Demographic, clinical, and paraclinical variables were analyzed. Logistic regression was used to identify associated factors. Results: A total of 326 patients were included; 8.6% (n=28) experienced therapeutic failure. No significant associations were found with clinical or demographic variables. Therapeutic failure was associated with longer hospital stay (mean 6.8 vs. 3.8 days, p<0.001), longer duration of intravenous treatment (mean 5.6 vs. 3.2 days, p<0.001), and higher MRSA isolation (21.4% vs. 1%, p<0.05). Most cases had a favorable outcome. Conclusions: Beta-lactams are effective as the treatment of choice for SSTIs in pediatrics. Therapeutic failure is uncommon and is mainly associated with MRSA infections and more severe clinical presentations. These findings support the use of beta-lactams and highlight the need for prospective studies to optimize prediction of therapeutic failure. | |
