Sindrome doloroso post mastectomía: incidencia y características en una población Bogotana: Hospital Universitario Mayor Méderi 2018-2019

dc.descriptionIntroducción: El cáncer de mama se ha constituido como la primera causa de cáncer en las mujeres de nuestro país, se ha descrito una supervivencia del 79,6% a dos años posterior al tratamiento, y una de las principales consecuencias del tratamiento quirúrgico, es el dolor postoperatorio persistente, llamado síndrome doloroso postmastectomía (SPDM); su incidencia a nivel mundial se ha reportado entre el 25-60% y de 55,3% en Colombia. El presente estudio buscó calcular la incidencia y evaluar factores asociados a su presentación en las mujeres llevadas a cirugía por cáncer de mama en el Hospital Universitario Mayor-Mederi en el periodo entre septiembre de 2018 y marzo de 2019. Metodología: Estudio transversal, en el que se evaluaron la totalidad de pacientes tratadas con cirugía oncológica para el cáncer de mama en el Hospital Universitario Mayor Mederi en la ciudad de Bogotá, buscando la presencia de SDPM y sus características. Resultados: Se identificaron 26 pacientes en el periodo de estudio, la incidencia de SDPM fue del 27%. Entre los factores asociados se encontró: índice de masa corporal normal y edad >60 años, realización de mastectomía radical con vaciamiento axilar y adyuvancia con Taxanos, Doxorrubicina y Ciclofosfamida u hormonoterapia con Tamoxifeno. Discusión: La incidencia de dolor es similar a la reportada en otros estudios de la región y los factores asociados no son extrapolables a la población de nuestro país, pero es llamativo que a pesar de los factores descritos en literatura como radioterapia, sobrepeso y menor edad, estas condiciones no presentaron diferencias estadísticamente significativas para nuestra población.spa
dc.description.abstractBackround: Breast cancer has become the first cause of cancer in women in our country, with a two-year survival of 79.6% after treatment, and one of the main consequences has been persistent postoperative pain, called postmastectomy pain syndrome (SPDM); it has been reported an incidence of 25-60% worldwide, and 55.3% in Colombia. The present study aims to calculate the incidence and evaluate associated factors of women with painful post-mastectomy syndrome that has been taken to breast cancer surgery at the University Hospital Mayor-Mederi in the period between september 2018 and march 2019. Methods: A cross-sectional study was performed, in which the totality of patients taken to an oncological surgery for breast cancer were included, at the Mederi University Hospital in Bogotá, and were evaluated for the presence of SDPM. Results: 26 patients met inclusion criteria during the study period were identified, the incidence of SDPM was 27%. The associated characteristics were normal body mass index and age> 60 years, radical mastectomy with axillary emptying and adjuvant with Taxanes, Doxorubicin and Cyclophosphamide or hormone therapy with Tamoxifen Discussion: The incidence of pain is similar to that reported in other studies in the region and the risk factors are not extrapolated to the population of our country, but it is striking that despite being importantly associated with radiotherapy, overweight and younger, these conditions were no statistically different for our population.spa
