dc.description | Dada la fisiopatología propia de la acción de la micobacteria causante, generalmente la tuberculosis-TB es diagnosticada de forma tardía por la lenta progresión de la infección y por la demora en las manifestaciones clínicas, lo cual también es muy propio de la tuberculosis de rodilla-TBR, implicando un proceso de destrucción articular, perdida de la estabilidad y movilidad articular y adicionalmente la incidencia no es alta. Por esta razón, se efectuó una revisión de alcance sobre el concepto de la patología, la historia de la patología, las características fisiopatológicas de la enfermedad, las clínicas del adulto con TBR, las formas de diagnóstico de los casos, las formas de tratamiento, los resultados de los métodos diagnósticos y de tratamiento. Metodología: se utilizó la metodología PRISMA-ScR para el estudio acudiendo a las bases de datos PUBMED/MEDLINE, OVIDSp, EMBASE, LILACS y BVS a partir de la pregunta SPIDER (1), aplicando la rigurosidad para una revisión sistemática. Resultados: La TBR presenta una mayor frecuencia en adultos mayores de forma monoarticular. El diagnostico es tardío por la manifestación clínica. Requiere de métodos de diagnostico imagenológicos e histológicos, y es tratada farmacológica y quirúrgicamente. Discusión: La TBR se puede confundir con artritis infecciosa, presenta un curso lento. La baja incidencia hace que no se sospeche esta infección y se diagnostica cuando hay daño articular grave. El tratamiento conjugado junto con el quirúrgico presenta resultados alentadores. Conclusiones: la TBR es un tema que requiere de mayor estudio dados sus efectos clínicos, características patológicas y de diagnóstico. | spa |
dc.description.abstract | Given the pathophysiology of the causative mycobacterium action, tuberculosis-TB is generally diagnosed late due to the slow progression of the infection and the delay in clinical manifestations, which is also very typical of tuberculosis of knee-TBR, involving a process of joint destruction, loss of joint stability and mobility and additionally the incidence is not high. For this reason, a scoping review was carried out on the concept of pathology, the history of the pathology, the pathophysiological characteristics of the disease, the clinics of the adult with TBR, the forms of diagnosis of the cases, the forms of treatment, the results of diagnostic and treatment methods. Methodology: the PRISMA-ScR methodology was used for the study, going to the PUBMED / MEDLINE, OVIDSp, EMBASE, LILACS and BVS databases from the SPIDER question (1), applying rigor for a systematic review. Outcomes: TBR is more frequent in older adults in a monoarticular way. The diagnosis is late due to the clinical manifestation. It requires imaging and histological diagnostic methods, and it is treated pharmacologically and surgically. Discussion: TBR can be confused with infectious arthritis, it has a slow course. The low incidence means that this infection is not suspected and it is diagnosed when there is severe joint damage. Conjugated treatment together with surgical treatment shows encouraging results. Conclusions: TBR is a subject that requires further study given its clinical effects, pathological and diagnostic characteristics. | spa |
