Anestesia general versus sedación para colocación de terapia de resincronización cardíaca. Estudio retrospectivo observacional de corte transversal

Resumen
La insuficiencia cardiaca es una de las principales causas de ingresos hospitalarios, a pesar de un tratamiento optimo el riesgo de morbilidad y mortalidad es alto en estos pacientes. La terapia de cardio resincronización ha revolucionado el tratamiento de estos pacientes demostrando reducir síntomas y mejorar su calidad de vida. El manejo anestésico para este procedimiento es un reto para el anestesiólogo dado la complejidad del perfil de estos pacientes. Actualmente no hay recomendaciones claras en la literatura de la técnica anestésica más apropiada, los estudios realizados demuestran que el uso de sedación tiene menor incidencia en el uso de soporte vasopresor intraoperatorio en estos pacientes. El objetivo de este estudio retrospectivo fue comparar el uso de soporte vasopresor con la técnica anestésica en pacientes llevados a terapia de cardio resincronización. Se realizo un análisis estadístico identificando 126 pacientes en el grupo de sedación y 28 pacientes en el de anestesia general, evidenciando en este último grupo una mayor incidencia de uso de soporte vasopresor intraoperatorio con una diferencia estadísticamente significativa.
Abstract
Heart failure is one of the main causes of hospital admissions; despite optimal treatment, the risk of morbidity and mortality is high in these patients. Cardio resynchronization therapy has revolutionized the treatment of these patients, demonstrating to reduce symptoms and improve their quality of life. Anesthetic management for this procedure is a challenge for the anesthesiologist given the complexity of the profile of these patients. Currently there are no clear recommendations in the literature regarding the most appropriate anesthetic technique; the studies carried out demonstrate that the use of sedation has a lower impact on the use of intraoperative vasopressor support in these patients. The objective of this retrospective study was to compare the use of vasopressor support with the anesthetic technique in patients undergoing cardio resynchronization therapy. A statistical analysis was performed identifying 126 patients in the sedation group and 28 patients in the general anesthesia group, showing in the latter group a higher incidence of use of intraoperative vasopressor support with a statistically significant difference.
