Evaluación de la artrosis de rodilla por compartimentos con la escala de Kellgren y Lawrence modificada

dc.descriptionIntroducción: La clasificación de Kellgren y Lawrence (KL) se utiliza para evaluar la artrosis de rodilla, no hay en la literatura consenso de las características a evaluar en cada categoría, ni la opción de evaluar por compartimentos. Se propone la modificación de KL para el análisis individual de los compartimentos de la rodilla. Metodología: La modificación a la escala se realizó por consenso informal. Se evaluó la concordancia intraobservador e interobservador utilizando el coeficiente Krippendorff`s alpha. 210 radiografías de rodilla fueron evaluadas por 6 observadores, en dos instancias con 4 semanas de diferencia asignándoles un puntaje. Resultados: Se construyo un sistema de evaluación a partir de la clasificación de KL que incluye las variables de espacio articular, osteofitos, línea subcondral, esponjosa subcondral. Se realiza la evaluación individual de los compartimentos femorotibial interno (FTI), externo (FTE) y patelofemoral (PF). El resultado de la concordancia interobservador en la primera evaluación fue 0.48, 0.46 y 0.51, para compartimentos FTI, FTE y PF, en la segunda evaluación fue de 0.54, 0.56 y 0.61, respectivamente. Se observa mejoría en la concordancia interobservador en la segunda evaluación. La concordancia intraobservador fue de moderada a substancial y para algunos evaluadores casi perfecta con rangos de 0.48-0.8 , 0.55-0.82 y 0.44-0.83, para compartimentos FTI, FTE y PF. Conclusión: La concordancia de KL modificada es de moderada a substancial y consideramos que la propuesta presentada es útil y específica para la evaluación radiológica de la artrosis de rodilla.spa
dc.description.abstractIntroduction: Kellgren and Lawrence (KL) scale is used to evaluate osteoarthritis of the knee, there is no consensus in the literature of the characteristics to be evaluated in each category, nor the option of evaluating compartments. The modification of KL scale is proposed for the individual analysis of the compartments of the knee. Methods: The modification to the scale was made by informal consensus. Intra-observer and interobserver concordances were evaluated using Krippendorff's alpha coefficient. 210 knee radiographs were evaluated by 6 observers, in two instances with 4 weeks of difference assigning them a score. Results: Based on the KL scale evaluation system was constructed that includes the variables of joint space, osteophytes, subchondral line, spongy subchondral. The individual evaluation of the three compartments of the knee (Medial, lateral and patellofemoral) was performed. The result of the interobserver concordance in the first evaluation was 0.48, 0.46 and 0.51, for medial, lateral and patellofemoral compartments, in the second evaluation it was 0.54, 0.56 and 0.61, respectively. An improvement in interobserver concordance was observed in the second evaluation. The intra-observer concordance was moderate to substantial and for some evaluators was almost perfect with ranks between 0.48-0.8, 0.55-0.82 and 0.44-0.83, for medial, lateral and patellofemoral compartments. Conclusion: The agreement of modified KL scale is moderate to substantial and we consider that the presented proposal is useful and specific for the radiological evaluation of knee osteoarthritis.spa
