Comparación de la eficacia y seguridad de la nefrostomía percutánea y el catéter doble JJ en el tratamiento de la uropatía obstructiva benigna: revisión sistemática-metaanálisis

| dc.description | Introducción: La uropatía obstructiva benigna, es una consulta frecuente en urología y puede manifestarse desde pacientes asintomáticos hasta insuficiencia renal aguda o sepsis. En estos casos, requiere una derivación urgente o drenaje de la vía urinaria mediante nefrostomía o catéter doble j, sin embargo, la evidencia disponible sobre la superioridad de una técnica frente a la otra continúa siendo controvertida. Objetivo: Comparar la eficacia y seguridad de la nefrostomía percutánea y el catéter doble j en el tratamiento de la uropatía obstructiva benigna, una revisión sistemática y metanálisis. Métodos: Se realizó una revisión sistemática y metaanálisis, siguiendo recomendaciones PRISMA en base de datos biomédicas hasta el 2023. La selección de estudios y la extracción de datos se efectuaron siguiendo criterios inclusión y exclusión, el análisis estadístico se realizó en Review Manager (RevMan®) versión 5.4 y Stata versión 1.4. Se determinaron diferentes variables dicotómicas mediante riesgo relativo (RR), con intervalos de confianza del 95%. Resultados: Se encontraron 728 registros, se incluyeron 20 estudios, cuatro fueron ensayos clínicos aleatorizados y dieciséis estudios a estudios observacionales, ambos procedimientos demostraron ser seguros para el manejo de la uropatía obstructiva, se identificaron diferencias en seguridad, el CDJ mostró ventajas en términos de menor riesgo de desplazamiento del dispositivo (RR 0,43) y menor necesidad de recambio (RR 0,49)menor tiempo de hospitalización. En conjunto, los hallazgos sugieren que, aunque ambas técnicas son efectivas, el catéter doble J podría ser preferible en escenarios donde se priorice menor tasa de complicaciones, menor estancia hospitalaria, mientras que la nefrostomía percutánea podría reservarse para casos seleccionados según características clínicas específicas. Conclusión: La evidencia disponible permite fortalecer que la nefrostomía percutánea y catéter doble j son métodos eficaces y seguras para el manejo de uropatía obstructiva, aunque ambas técnicas son efectivas, el catéter doble J mostró menor estancia hospitalaria y mejor tolerancia, la selección de la derivación se debe individualizar según las características de los pacientes. Mientras que la Nefrostomías Percutánea podría ofrecer mayores tasas de éxito en obstrucciones complejas. Se requieren estudios adicionales, ensayos clínicos aleatorizados, para fortalecer la evidencia clínica disponible. | |
| dc.description.abstract | Introduction: Benign obstructive uropathy is a common urological presentation, ranging from asymptomatic cases to acute renal failure or sepsis. These cases require urgent urinary tract diversion or drainage via nephrostomy or a double-J stent; however, evidence regarding the superiority of one technique over the other remains controversial. Objective: To compare the efficacy and safety of percutaneous nephrostomy and double-J stents in the treatment of benign obstructive uropathy through a systematic review and meta-analysis. Methods: A systematic review and meta-analysis were conducted following PRISMA guidelines, searching biomedical databases up to 2023. Study selection and data extraction adhered to predefined inclusion and exclusion criteria. Statistical analysis was performed using Review Manager (RevMan®) version 5.4 and Stata version 1.4. Dichotomous variables were assessed using relative risk (RR) with 95% confidence intervals. Results: A total of 728 records were identified, and 20 studies were included (four randomized clinical trials and sixteen observational studies). Both procedures proved safe for managing obstructive uropathy; however, differences in safety profiles were observed. The double-J stent showed advantages regarding a lower risk of device displacement (RR 0.43), a reduced need for replacement (RR 0.49), and shorter hospital stays. Overall, the findings suggest that while both techniques are effective, the double-J stent may be preferable in scenarios prioritizing lower complication rates and shorter hospital stays, whereas percutaneous nephrostomy might be reserved for selected cases based on specific clinical characteristics. Conclusion: Available evidence supports the finding that percutaneous nephrostomy and double-J stents are effective and safe methods for managing obstructive uropathy. Although both techniques are effective, the double-J stent was associated with a shorter hospital stay and better tolerability; the choice of diversion method should be individualized based on patient characteristics, whereas percutaneous nephrostomy may offer higher success rates in complex obstructions. Further studies—specifically randomized clinical trials—are needed to strengthen the available clinical evidence. | |
