Recambio plasmático terapéutico con membrana. Experiencia de un solo centro: estudio de cohorte

dc.descriptionEl recambio plasmático terapéutico (TPE) es una técnica utilizada en el manejo de patologías de origen autoinmune, neurológico, hematológico, renal, entre otros. Se fundamenta en los principios de centrifugación o de filtración con membrana. Son pocos los estudios que han valorado el rendimiento y seguridad de esta técnica, mostrando resultados heterogéneos no comparables y, la información en Colombia es escasa. Por lo anterior, es importante describir la experiencia del recambio plasmático terapéutico con membrana (mTPE) y sus resultados clínicos en una cohorte de pacientes atendidos en La Cardio, en Bogotá, Colombia. Se plantea un estudio de tipo cohorte retrospectiva, donde se incluirán pacientes adultos con indicación de mTPE en el período de un año. Los análisis incluyen la descripción sociodemográfica y clínica de la población, laboratorio basales, caracterización del procedimiento, desenlaces clínicos e identificación de las complicaciones relacionadas con el procedimiento. Las principales indicaciones para el uso de la terapia fueron el rechazo humoral de trasplante y el síndrome de Guillain - Barré. Algunos aspectos en relación a la técnica son importantes de resaltar, especialmente en relación a la anticoagulación. Sin embargo en nuestro caso no se usó ninguna estrategia de anticoagulación, encontrando una tasa muy baja de problemas asociados. Además, la mayoría de las sesiones se realizaron en el área de hospitalización. Todas las terapias se realizaron con técnica de membrana encontrando una baja tasa de complicaciones. La información referida constituirá un aporte al conocimiento del uso de mTPE en el país y a investigaciones adicionales.
dc.description.abstractTherapeutic plasma exchange (TPE) is a technique used in the management of pathologies of autoimmune, neurological, hematological, renal origin, among others. It is based on the principles of centrifugation or membrane filtration. There are few studies that have assessed the performance and safety of this technique, showing heterogeneous, non-comparable results, and information in Colombia is scarce. Therefore, it is important to describe the experience of membrane therapeutic plasma exchange (mTPE) and its clinical results in a cohort of patients treated at La Cardio, in Bogotá, Colombia. A retrospective cohort study is proposed, where adult patients with an indication for mTPE will be included over a period of one year. The analyzes include the sociodemographic and clinical description of the population, baseline laboratories, characterization of the procedure, clinical outcomes and identification of complications related to the procedure. The main indications for the use of therapy were humoral transplant rejection and Guillain-Barré syndrome. Some aspects regarding the technique are important to highlight, especially in relation to anticoagulation. However, in our case no anticoagulation strategy was used, finding a very low rate of associated problems. Furthermore, most of the sessions were held in the hospitalization area. All therapies were performed with membrane technique, finding a low rate of complications. The information referred to will constitute a contribution to the knowledge of the use of mTPE in the country and to additional research.
