Segundo primario cáncer de tiroides en sobrevivientes de otro cáncer y asociación a radioterapia

| dc.description | Introducción: Las segundas neoplasias malignas, particularmente los tumores sólidos, son una causa importante de mortalidad y morbilidad grave entre los sobrevivientes de cáncer que se curaron con éxito de su primera neoplasia. Se sugiere que existe una asociación entre una primera neoplasia y el cáncer de tiroides, y se atribuye parte de ésta a la exposición a radioterapia en el tratamiento de la primera neoplasia. Objetivo: Determinar la relación de la radioterapia como factor de riesgo en la aparición de cáncer de tiroides en pacientes tratados previamente para la primera neoplasia. Metodología: Se realizó un estudio retrospectivo de casos y controles; de pacientes con diagnóstico de una primera neoplasia desde el año 2007 hasta el 2017. A partir de éste grupo de pacientes se definió como casos a aquellos que presentaron cáncer de tiroides como segunda neoplasia primaria y como controles a aquellos que no lo presentaron. El factor de riesgo a evaluar fue la exposición o no a radioterapia, además de otras variables como el sitio de radioterapia y la primera neoplasia. Resultados: Se encontró una asociación entre la radioterapia y el cáncer de tiroides OR 6,580 [95% CI 1,219-4,764], sexo femenino OR 6,188 [95% CI 1,232-7,907], seno como primera neoplasia OR 6,188 [95% CI 1,194-4,941] y como factores independientes radioterapia a nivel torácico OR 6,298 [95% CI 12,581-15,370] y la primera neoplasia de tejidos blandos OR 6,271 [95% CI 1,549-25,37]. La mediana de aparición de cáncer de tiroides como segundo primario fue de 63,80 meses y sin diferencias significativas entre en el tiempo entre exposicion o no de radioterapia p=0,899. | dc.description.abstract | Background: Second malignancies, particularly solid tumors, are an important cause of mortality and serious morbidity among cancer survivors who were successfully cured of their first malignancy. It is suggested that there is an association between a first neoplasm and thyroid cancer, and part of it is attributed to exposure to radiotherapy in the treatment of the first neoplasm. Objective: To determine the relationship of radiotherapy as a risk factor in the appearance of thyroid cancer in patients previously treated for the first neoplasm. Methodology: A retrospective study of cases and controls was carried out; of patients diagnosed with a first malignancy from 2007 to 2017. From this group of patients, those who presented thyroid cancer as the second primary malignancy were defined as cases and those who did not present as controls. The risk factor to evaluate was the exposure or not to radiotherapy, in addition to other variables such as the radiotherapy site and the first neoplasm.
