Estado crítico de cirrosis hepática como predictor de mortalidad en síndrome hepatoadrenal. Revisión sistemática de la literatura

dc.description | Introducción: El síndrome hepatoadrenal es frecuente en los pacientes con cirrosis descompensada y críticamente enfermos. Buscamos identificar la evidencia sobre el impacto del síndrome hepatoadrenal en el desenlace de mortalidad de pacientes con cirrosis hepática que cumplen criterios para este síndrome. Metodología: Realizamos una revisión sistemática de la literatura usando las bases de datos Ovid-MEDLINE, PUBMED, EMBASE, The Cochrane Library, LILACS y web of Science (WOS) para identificar los estudios relevantes hasta el año 2020. Resultados: De 336 estudios identificados, 28 artículos fueron definidos potencialmente relevantes, 27 fueron excluidos considerando que, 3 correspondían a editoriales, 24 artículos con análisis de desenlaces no pertinentes a la pregunta de investigación de nuestro estudio; por tanto 1 estudio cumplía con las características de síntesis cualitativa demostrándose la escasa evidencia científica a la fecha. Los pacientes cirróticos con hemorragia por várices tienen concentraciones basales medias más altas de cortisol (15,4 μg/dL) que los pacientes cirróticos estables (8,7 μg/dL, P = 0,001) ó los voluntarios sanos (10,1 μg/dL, P = 0,01). Los pacientes cirróticos críticamente enfermos tienen concentraciones máximas medianas de cortisol más altas que los pacientes cirróticos estables (resultados de SST de 32,7 frente a 21 μg/dL, p = 0,001; resultados de LDSST de 9,3 frente a 8,1 μg/dL; no significativos), sin diferencias en delta máximo (Δmax) en cualquiera de las dos pruebas. Estas diferencias fueron mayores con el sangrado por várices que en pacientes cirróticos estables con insuficiencia adrenal. Discusión: A pesar de las limitaciones del estudio, esta revisión sistemática se suma a la intención de crecimiento mundial en el conocimiento de esta enfermedad, la existencia del síndrome hepatoadrenal es un determinante de mal pronóstico y mortalidad en pacientes con cirrosis críticamente enfermos. Son necesarios más estudios de buena calidad que permitan analizar y esclarecer este problema clínico. | es |
dc.description.abstract | Introduction: Hepatoadrenal syndrome is common in critically ill patients with decompensated cirrhosis. We seek to identify the evidence on the impact of hepatoadrenal syndrome on the mortality outcome of patients with liver cirrhosis who meet the criteria for this syndrome. Methodology: We conducted a systematic review of the literature using the databases Ovid- MEDLINE, PUBMED, EMBASE, The Cochrane Library, LILACS, and the web of Science (WOS) to identify relevant studies up to the year 2020. Results: Of 336 studies identified, 28 articles were defined as potentially relevant, 27 were excluded considering that 3 corresponded to editorials, 24 articles with analysis of outcomes not relevant to the research question of our study; therefore 1 study fulfilled the characteristics of qualitative synthesis, showing the scant scientific evidence to date. Cirrhotic patients with variceal hemorrhage have higher mean baseline cortisol concentrations (15.4 μg / dL) than stable cirrhotic patients (8.7 μg / dL, P = 0.001) or healthy volunteers (10.1 μg / dL). dL, P = 0.01). Critically ill cirrhotic patients have higher median peak cortisol levels than stable cirrhotic patients (SST results of 32.7 vs 21 μg / dL, P = 0.001; LDSST results of 9.3 vs 8.1 μg / dL; not significant), with no differences in maximum delta (Δmax) in either of the two tests. These differences were greater with variceal bleeding than in stable cirrhotic patients with adrenal insufficiency. Discussion: Despite the limitations of this study, this systematic review adds to the intention of worldwide growth in the knowledge of this disease, the existence of hepatoadrenal syndrome is a determinant of poor prognosis and mortality in critically ill patients with cirrhosis. More good quality studies are needed to analyze and clarify this clinical problem. | es |
