BEFAST: Herramienta de tamizaje diagnóstico en triage para la identificación de accidente cerebrovascular en el servicio de Urgencias

dc.descriptionAntecedentes: El accidente cerebrovascular (ACV) es una patología aguda y grave, frecuente cuya morbimortalidad, carga de enfermedad y costos en salud son muy relevantes. El ACV isquémico y hemorrágico hace parte de su espectro, por lo que reconocer sus signos de alarma de forma temprana mediante herramientas de tamizaje se constituye en una estrategia que disminuye su impacto. Metodología: Mediante un estudio de prueba diagnóstica se evaluó el valor predictivo de la activación del código ACV de la escala BEFAST, para ACV en pacientes adultos atendidos en el triage en el servicio de Urgencias en la Fundación Santa Fe de Bogotá (FSFB) entre enero del 2022 a enero del 2023 comparado con el diagnóstico de Neurología determinado mediante angiografía, resonancia magnética nuclear o tomografía axial computarizada. Se excluyeron gestantes y pacientes con ACV o sospecha remitidos de otra institución con tratamiento antes del ingreso. El estudio fue aprobado por el comité de Ética de la FSFB. Resultados: Se identificó ACV mediante la escala en 182 pacientes (78,1%) entre quienes se activó el código ACV. El punto de corte para el ACV en la escala fue ≥1 punto (síntoma) y con este se presentó una sensibilidad de 0,91; especificidad de 0,31; valor predictivo positivo de 0,78 y valor predictivo negativo de 0,56 y un área bajo la curva ROC de 0,61 (IC 95%: 0,53-0,69; p<0,005). Conclusión: La escala BEFAST aplicada con exactitud puede ayudar a identificar desde el triage la población adulta que está presentando un ACV y permitir activar el código ACV. En el grupo analizado presento características operativas adecuadas, pero se requiere de mayor evidencia para poder comprobar este comportamiento.
dc.description.abstractBackground: Stroke is an acute and serious condition, common and with significant morbidity, mortality, disease burden, and healthcare costs. Ischemic and hemorrhagic strokes are part of its spectrum, making early recognition of their warning signs through screening tools a strategy to reduce their impact. Methodology: A diagnostic test was used to evaluate the predictive value of BEFAST stroke code activation for stroke in adult patients triaged in the Emergency Department of the Santa Fe de Bogotá Foundation (FSFB) between January 2022 and January 2023, compared to a neurological diagnosis determined by angiography, magnetic resonance imaging, or computed tomography. Pregnant women and patients with confirmed or suspected stroke referred from another institution and receiving treatment prior to admission were excluded. The study was approved by the FSFB Ethics Committee. Results: Stroke was identified using the scale in 182 patients (78.1%) among whom the stroke code was activated. The cutoff point for stroke on the scale was ≥1 point (symptom), and at this point, a sensitivity of 0.91, specificity of 0.31, positive predictive value of 0.78, and negative predictive value of 0.56 were observed, with an area under the ROC curve of 0.61 (95% CI: 0.53–0.69; p < 0.005). Conclusion: The BEFAST scale, when applied accurately, can help identify adults experiencing a stroke during triage and allow for the activation of the stroke code. In the group analyzed, it demonstrated adequate operational characteristics, but further evidence is needed to confirm this performance.
