Sulfato de magnesio en dolor neuropático: revisión sistemática y meta-análisis

| dc.description | La eficacia de Sulfato de Magnesio en dolor neuropático aún resulta incierto, en revisiones sistemáticas previas ha demostrado su eficacia en la neuro modulación del dolor postoperatorio. Métodos: Se realizó una revisión sistemática de ensayos clínicos aleatorizados (ECA) desde 1990 hasta mayo de 2023 en cuatro bases de datos como Medline a través del motor de búsqueda PubMed, BVS-LILACS, Google Scholar así como una búsqueda de litera gris en Open Grey y en bola de nieve. Se calcularon diferencias de medias estandarizadas (DME) con un intervalo de confianza del 95% bajo el método de Mantel Haenszel de efectos aleatorios. Los ECAs fueron evaluados mediante la puntuación de Jadad y el riesgo de sesgo se realizó mediante la herramienta de la colaboración Cochrane. Finalmente se generó una evaluación de la certeza de la evidencia mediante la Clasificación de Recomendaciones, Valoración, Desarrollo y Evaluación (GRADE) para establecer la confianza en las estimaciones del efecto. Resultados: Se incluyeron 7 ensayos clínicos aleatorizados con 322 pacientes. Se encontró evidencia de calidad baja que mostró una eficacia a favor del uso del Sulfato de Magnesio frente a placebo con DME -0.99, IC 95%, -1.5 a -0.48 (heterogeneidad Tau2 0.03; Chi2 1.26 ; I2 del 20%). Además, una evidencia con muy baja calidad que no encontró diferencia entre el uso de Sulfato de Magnesio y Ketamina con DME de -0.39 (IC 95%: -0.84, 0.06) y una alta heterogeneidad (Chi2=4,07; I2: 75%). Discusión: El Sulfato de Magnesio mostró una eficacia en dolor neuropático, con una comparabilidad frente al uso de Ketamina, aunque de muy baja calidad. Futuras investigaciones podrían clarificar la eficacia y convertir al Sulfato de Magnesio como una herramienta del arsenal terapéutico para el tratamiento del dolor neuropático. | dc.description.abstract | Background:The effectiveness of Magnesium Sulfate in neuropathic pain is still uncertain; previous systematic reviews have demonstrated its effectiveness in the neuromodulation of postoperative pain. Methods: A systematic review of randomized clinical trials (RCTs) from 1990 to May 2023 was carried out in four databases such as Medline through the search engine PubMed, BVS-LILACS, Google Scholar as well as a gray litter search in Open Gray and in a snowball. Standardized mean differences (SMD) were calculated with a 95% confidence interval under the Mantel Haenszel random effects method. The RCTs were evaluated using the Jadad score and the risk of bias was assessed using the Cochrane collaboration tool. Finally, an assessment of the certainty of the evidence was generated using the Grading of Recommendations, Assessment, Development and Evaluation (GRADE) to establish confidence in the effect estimates. Results: 7 randomized clinical trials with 322 patients were included. Low-quality evidence was found that showed efficacy in favor of the use of Magnesium Sulfate versus placebo with SMD -0.99, 95% CI, -1.5 to -0.48 (Tau2 heterogeneity 0.03; Chi2 1.26; I2 of 20%). Furthermore, very low quality evidence that found no difference between the use of Magnesium Sulfate and Ketamine with SMD of -0.39 (95% CI: -0.84, 0.06) and high heterogeneity (Chi2=4.07; I2: 75 %). Discussion: Magnesium Sulfate showed efficacy in neuropathic pain, with comparability compared to the use of Ketamine, although of very low quality. Future research could clarify the effectiveness and convert Magnesium Sulfate as a tool in the therapeutic arsenal for the treatment of neuropathic pain.
