Psicoeducación en trastornos del espectro de la esquizofrenia: una revisión sistemática de alcance

dc.descriptionIntroducción: La psicoeducación en pacientes con esquizofrenia, inclusive programas de corta duración, se ha asociado a disminución de la tasas de recaída, la no adherencia al tratamiento farmacológico y las tasas de reingreso hospitalario, así como un aumento en la satisfacción con el tratamiento, la funcionalidad y la calidad de vida. Objetivo: Describir la evidencia acerca de las distintas estrategias de psicoeducación para el manejo de pacientes y familiares de pacientes con trastornos del espectro de la esquizofrenia. Metodología: Se realizó la búsqueda en las plataformas LILACS, PUBMED y SCOPUS con los términos en inglés “Schizophrenia Spectrum and Other Psychotic Disorders” y “psychoeducation” así como sus variables. Se incluyeron artículos en inglés y español. Resultados: Se incluyeron 66 estudios (69 intervenciones) con 6.093 participantes, en su mayoría pacientes con esquizofrenia y sus cuidadores. Predominaron los ensayos clínicos aleatorizados y cuasi-experimentales, realizados en Asia y Europa, principalmente en formato grupal presencial. Las intervenciones abordaron principalmente conocimiento de la enfermedad, adherencia al tratamiento, afrontamiento y apoyo a cuidadores. Los desenlaces más evaluados fueron carga del cuidador, adherencia, síntomas, conocimiento y estigma. En general, se reportaron efectos positivos y significativos, aunque un 20 % de los estudios no encontró resultados concluyentes. Conclusiones: Esta revisión de alcance permitió identificar y caracterizar las diversas estrategias de psicoeducación dirigidas a pacientes con trastornos del espectro de la esquizofrenia y a sus familiares. Se evidenció una amplia variedad de formatos, componentes y modalidades, junto con una marcada heterogeneidad metodológica y una limitada representación de estudios latinoamericanos.
dc.description.abstractBackground: Psychoeducation for individuals with schizophrenia, including brief interventions, has been shown to reduce relapse rates, improve adherence to pharmacological treatment, and decrease hospital readmissions. It is also associated with greater treatment satisfaction, improved functioning, and enhanced quality of life. Objective: To review and summarize current evidence on psychoeducational strategies for patients and relatives of individuals with schizophrenia spectrum disorders. Methods: A systematic search was conducted in the LILACS, PubMed, and Scopus databases using the terms “Schizophrenia Spectrum and Other Psychotic Disorders” and “psychoeducation,” including their variations. Articles published in English and Spanish were included. Results: Sixty-six studies (69 interventions) were included, involving 6,093 participants—primarily patients with schizophrenia and their caregivers. Most studies were randomized controlled or quasi-experimental trials conducted in Asia and Europe, commonly delivered in face-to-face group formats. Interventions primarily addressed illness knowledge, treatment adherence, coping strategies, and caregiver support. The most frequently evaluated outcomes were caregiver burden, adherence, symptom severity, illness knowledge, and stigma. Overall, most studies demonstrated significant positive effects, although approximately 20% reported inconclusive findings. Conclusions: This scoping review identified and characterized the various psychoeducational strategies directed at patients with schizophrenia spectrum disorders and their families. A wide range of formats, components, and delivery modalities was observed, along with marked methodological heterogeneity and a limited representation of studies conducted in Latin America.
