Caracterización fibrilación auricular y nuevos anticoagulantes orales en urgencias - Fundación Santa Fe de Bogotá

dc.description | Introducción: En los últimos años se dispone de nuevos anticoagulantes orales para la fibrilación auricular. En Colombia su uso es más frecuente desde el año 2013. Se pretende caracterizar a los pacientes con fibrilación auricular no valvular en la era de los nuevos anticoagulantes orales. Metodología: Se revisaron historias clínicas electrónicas de los pacientes que consultaron a urgencias del Hospital Universitario Fundación Santa Fe de Bogotá, con diagnóstico principal o secundario de fibrilación auricular entre enero de 2013 y marzo de 2015. Resultados: La prevalencia de la Fibrilación Auricular fue del 0,29%, se analizaron 229 pacientes de los cuales el 54,59% fueron hombres. El 75,1% tenían más de 65 años y los mayores de 80 años representan el 41%. La warfarina fue el anticoagulante oral más usado seguido por el dabigatrán, rivaroxabán y el apixabán. Solo 4 casos de sangrado mayor en pacientes anticoagulados dos con warfarina y 2 con dabigatrán. 4 pacientes anticoagulados tuvieron ACV isquémico nuevo, 1 con warfarina, 1 con dabigatrán y 2 con rivaroxabán. Discusión: En Colombia es limitado el uso de nuevos anticoagulantes debido a las leyes relacionadas a la atención en salud. Se requieren más estudios similares o concurrentes en Colombia para poder tener datos de peso que respalden el uso de estos medicamentos en nuestro país y no tener necesidad de extrapolar datos de otros países. | spa |
dc.description.abstract | Introduction: In recent years new oral anticoagulants are available for atrial fibrillation. In Colombia, the usage has become more frequent since 2013. This paper aims to characterize patients with nonvalvular atrial fibrillation in the era of new oral anticoagulantsMethodology: The electronic medical records were reviewed for patients who consulted the emergency department of the Santa Fe University Hospital in Bogotá. Specifically, patients with a primary or secondary diagnosis of atrial fibrillation between January 2013 and March 2015. Results: The prevalence of atrial fibrillation was 0. 29%. 229 patients were analyzed of which 54. 59% were men. 75. 1% were over 65 and those over 80 represented 41% of the population size. Warfarin was the most commonly used oral anticoagulant followed by dabigatran, rivaroxaban and apixaban. There were only 4 cases of major bleeding in anticoagulated patients; 2 with warfarin and 2 with dabigatran. 4 anticoagulated patients had a new ischemic stroke, 1 with warfarin, 1 with dabigatran and 2 with rivaroxaban. Discussion: In Colombia, the use of new anticoagulants is limited due to health care regulations. More similar or concurrent studies are required to be able to have enough data to support the use of these drugs in the country, without the need to extrapolate data from other sources. | eng |
