Alteraciones en la estructura retiniana en pacientes con esquizofrenia temprana. Estudio piloto

| dc.description | Introducción: Aunque estudios previos sugieren que la estructura neurovascular de la retina está afectada en los trastornos del espectro de la esquizofrenia (SSD), esto no ha sido previamente explorado en los pacientes colombianos. Este estudio pretende evaluar si la estructura y la microvasculatura retiniana difiere en los SSD en comparación con los controles sanos (HC), y si se correlaciona con las variables clínicas y cognitivas. Métodos: Este estudio transversal incluyó a los pacientes con esquizofrenia (n = 20) o trastorno esquizoafectivo (n = 2) y HC pareados por edad/género (n = 21). Se empleó tomografía de coherencia óptica-angiografía. Los análisis estadísticos incluyeron modelos lineales generales ajustados por edad, sexo, equivalente esférico y tabaquismo. Se exploraron las asociaciones entre las mediciones retinianas y las variables clínicas o cognitivas mediante análisis de correlación y regresión lineal. Resultados: Las medidas retinianas fueron menores en los SSD que en los HC, con diferencias estadísticamente significativas solo en densidad vascular (DV) macular profunda inferior y promedio del ojo derecho. La gravedad de los síntomas se encontró inversamente correlacionada con el grosor del complejo de células ganglionares y la DV macular/papilar. El ajuste premórbido se asoció con medidas vasculares, pero no con medidas neurales, mientras que el tiempo de psicosis no tratada no se relacionó con ninguna medida retiniana. Un mayor tiempo desde la última hospitalización y un mejor rendimiento cognitivo se asociaron con una estructura retiniana más gruesa. Conclusiones: El estudio de la retina podría brindar información valiosa y rápida sobre la salud cerebral en los SSD, aunque aún se requieren estudios longitudinales multimodales en pródromos y en los SSD incipiente. | |
| dc.description.abstract | Introduction: Previous studies have suggested that the neurovascular structure of the retina is affected in schizophrenia spectrum disorders (SSD). However, no research has investi gated this in Colombian patients. This cross-sectional study explores whether individuals diagnosed with early-course SSD exhibit distinct retinal structure and microvasculature compared to healthy controls (HCs) and whether these differences correlate with clinical, course-related, and cognitive variables. Methods: Patients diagnosed with schizophrenia (n = 20) or schizoaffective disorder (n = 2) and age/gender-matched HCs (n = 21) were included. Retinal structure was assessed using Optical Coherence Tomography – Angiography. Statistical analyses included general linear models adjusted for covariates such as age, gender, spherical equivalent, and smoking sta tus. Associations between retinal measures and clinical or cognitive variables were explored using linear regression and correlation analyses. Results: Retinal measures were generally lower in patients than in HCs, with statistically significant differences observed only in the right eye’s inferior and average macular deep vessel density (VD). Symptom severity was inversely correlated with Ganglion Cell Complex thickness and macular/papillary VD. The premorbid adjustment was associated with vas cular measures but not neural tissue measures, while the duration of untreated psychosis was unrelated to any retinal measures. Longer time since the last hospitalization and better cognitive performance were linked to a thicker retinal structure. Conclusions: Retina could provide valuable and rapid information in real-world settings about brain health in SSD, so longitudinal studies on neurovascular retinal structure, including metabolic, inflammatory, and neuroimaging variables, in prodromal and early-course SSD are needed. | |
