Efectos de la terapia a gran altitud en pacientes con asma severa, una revisiòn sistemática de la literatura y metaanálisis

| dc.description | Antecedentes: el asma se ha convertido en una de las de las enfermedades respiratorias crónicas más comunes a nivel mundial, afectando entre el 1 al 29% de la población adulta y entre el 9-11 % de los niños (1-3), con un gran impacto en la morbimortalidad y costos económicos derivados de la pérdida de la productividad, el tratamiento en sus múltiples escenarios y el manejo de las complicaciones(4). Actualmente existen diversos y variados manejos escalonados que buscan un mejor control de la enfermedad; sin embargo, se estima que entre el 5% al 10% no logra un adecuado control de sus síntomas y se mantiene en un espectro de un asma severa (5). Esta población de pacientes precisa contar con nuevas alternativas de tratamiento costo-efectivas y de fácil implementación. Considerando esto, se plantea el tratamiento climático a gran altitud como una terapia emergente que ha demostrado resultados benéficos en el control de diferentes enfermedades pulmonares, incluyendo el asma (6). Métodos: se realizó una revisión sistemática de la literatura con la finalidad de identificar los efectos de la terapia climática a gran altura en pacientes con diagnóstico de asma severa. Se realizó la búsqueda en PubMed, Embase, Scielo, Lilacs y Google académico y CENTRAL, utilizando términos Emtree, términos MeSH, términos DeCS y términos en texto libre usando los siguientes operadores boleanos: ("Asthma"[Mesh]) AND ("Altitude"[Mesh] OR “Alpine"[Mesh]) y ('Asthma') AND ('Altitude’ OR 'Alpine') para todas las bases de datos descritas previamente. Se incluyeron estudios primarios realizados en seres humanos en población infantil y adulta, con diagnóstico de asma severa expuestos a altitudes mayores a 1500 msnm. Se excluyeron publicaciones duplicadas, reportes de casos y estudios de prevalencia. Se incluyeron los estudios que encontraban alguno de los siguientes como desenlace principal; la calidad de vida medida por SGRQ y/o AQLQ, el control de la enfermedad medida por ACQ y/o ACT, la función pulmonar medida por el valor de VEF1 y la relación VEF1/CVF y el marcador de alergia a partir del valor absoluto de IgE y/o el conteo absoluto de eosinófilos. Hallazgos: existe evidencia estadísticamente significativa con certeza de la evidencia baja que demuestra que la intervención con la terapia climática a gran altitud (mayor a 1500m.s.n.m) mejora la calidad de vida medida a partir del cuestionario AQLQ (DM -1.64 IC 95% [-1.77 a -1.5]) y mejora el control de la enfermedad medido por ACQ (DM 1.56 IC 95% [1.35 a 1.77]). No existe evidencia suficiente para considerar que exista un cambio significativo clínicamente en la función pulmonar medido por VEF1 con este tipo de intervención a pesar de arrojar un resultado estadísticamente significativo (DM -7.38 [-7,65 a – 7.11]). Se requieren más estudios para evaluar el efecto de la terapia climática a gran altitud en el conteo absoluto de eosinófilos y niveles de IgE. Interpretación: la terapia climática a gran altitud tiene evidencia estadísticamente significativa con una certeza de la evidencia baja en el control de síntomas y su impacto en la calidad de vida. No se encontró evidencia clínicamente significativa, pero si estadísticamente significativa para la función pulmonar y es necesario realizar más estudios para evaluar desenlaces como IgE y el conteo de eosinófilos. | |
| dc.description.abstract | Background: Asthma has become one of the most prevalent chronic respiratory diseases worldwide, affecting between 1% and 29% of the adult population and between 9% and 11% of children (1-3). It has a significant impact on morbidity and mortality, as well as economic costs derived from loss of productivity, treatment in its various forms, and management of complications (4). Currently, there are several and varied treatment strategies that seek to achieve better control of the disease. However, it is estimated that between 5% and 10% of patients do not achieve adequate control of their symptoms and remain in a spectrum of severe asthma. This patient population requires novel treatment options that are cost-effective and straightforward to implement. In light of this, high-altitude climate treatment is proposed as an emerging therapy that has demonstrated beneficial outcomes in the management of various pulmonary conditions, including asthma (6). Methods: A systematic review of the literature was conducted in order to identify the effects of high-altitude climate therapy in patients with a diagnosis of severe asthma. A comprehensive search was conducted in PubMed, Embase, Scielo, Lilacs, and Google Academic and CENTRAL databases. The search terms included Emtree, MeSH, DeCS, and free text terms. Boolean operators were used to combine search terms. A search was conducted in all the databases previously described using the following search terms: ("Asthma"[Mesh]) AND ("Altitude"[Mesh] OR "Alpine"[Mesh]) and ('Asthma') AND ('Altitude' OR 'Alpine'). The study included primary human studies in children and adults with a diagnosis of severe asthma exposed to altitudes higher than 1,500 meters above sea level (masl). Duplicate publications, case reports, and prevalence studies were excluded. Studies that found any of the following as the main outcome were included: quality of life as measured by SGRQ and/or AQLQ, disease control as measured by ACQ and/or ACT, lung function as measured by FEV1 value and FEV1/FVC ratio, and allergy markers as indicated by absolute IgE value and/or absolute eosinophil count. Results: The evidence is statistically significant and of low certainty, indicating that intervention with climate therapy at high altitude (greater than 1500 m.a.s.l.) improves quality of life as measured by the AQLQ questionnaire (MD -1.64, 95% CI [-1.77 to -1.5]). The results demonstrated a statistically significant improvement in quality of life as measured by the AQLQ questionnaire (MD -1.64, 95% CI [-1.77 to -1.5]), as well as a statistically significant improvement in disease control as measured by ACQ (MD 1.56, 95% CI [1.35 to 1.77]). The evidence is insufficient to conclude that this type of intervention has a clinically significant impact on lung function, as measured by FEV1, despite yielding a statistically significant result (MD -7.38 [-7.65 to -7.11]). Further studies are required to evaluate the effect of high-altitude climate therapy on absolute eosinophil counts and IgE levels. Interpretation: High altitude climate therapy has statistically significant evidence with low certainty of evidence on symptom control and its impact on quality of life. No clinically significant but statistically significant evidence was found for lung function and further studies are needed to evaluate outcomes such as IgE and eosinophil count. | |
