Resumen
La afectación hepática en pacientes críticamente enfermos con COVID-19 es frecuente, aunque generalmente transitoria y autolimitada. Sin embargo, algunos pacientes presentan alteraciones persistentes en las pruebas hepáticas. Se describe el caso de un paciente de 29 años con hallazgos clínicos e histológicos compatibles con colangiopatía post-COVID. A pesar del tratamiento con ácido ursodesoxicólico y colestiramina, los niveles de bilirrubina y transaminasas permanecieron elevados. Este reporte de caso resalta la dificultad en el manejo de complicaciones hepáticas asociadas al COVID-19.
Abstract
Liver involvement in critically ill patients with COVID-19 is frequent, although usually transient and self-limited. However, some patients present persistent liver test abnormalities. This case describes a 29-year-old patient with clinical and histological findings consistent with post-COVID cholangiopathy. Despite treatment with ursodeoxycholic acid and cholestyramine, bilirubin and transaminase levels remained elevated. This case report highlights the challenge of managing liver complications in patients with COVID-19.
