Deseo y funcionamiento sexual en pacientes con diagnóstico de esquizofrenia en consulta externa. Clínica de Nuestra Señora de la Paz, 2020-2021

Resumen
Introducción: La naturaleza de la sexualidad y la satisfacción sexual de los pacientes con enfermedades mentales sigue siendo un área poco estudiada. Además, es ampliamente aceptado que las personas con enfermedades mentales experimentan altos niveles de disfunción sexual, dada la baja satisfacción o el bajo interés sexual en esta población de la comunidad. Métodos: Se realizó un estudio de tipo Descriptivo de corte Transversal, en donde se aplicaron dos escalas (una para los hombres, otra para las mujeres) que miden el deseo y el funcionamiento sexual (Sexual Functioning Questionnaire Short-Form (CSFQ-14), además se tipificó la severidad de la esquizofrenia con la escala PANSS (Positive and Negative Syndrome Scale), y el manejo farmacológico de los pacientes entre 18 y 55 años que fueron manejados por consulta externa de psiquiatría con un diagnóstico de esquizofrenia, en la Clínica de Nuestra Señora de La Paz, durante los años 2020-2021. Resultados: Se obtuvo una muestra de 86 pacientes con una mediana de la edad de 35 años, de los cuales 77,9% fueron hombres y 22,1% mujeres. En cuanto a la escala CSFQ-14 se obtuvo una mediana de 30 puntos y en la escala PANSS una mediana de 68 puntos. En cuanto a las correlaciones entre variables se encontró una asociación estadísticamente significativa e inversamente proporcional entre el puntaje de la PANSS de síntomas negativos y el puntaje de la CSFQ-14. Por otro lado, el uso de antidepresivos también tuvo una correlación inversa y estadísticamente significativa con el puntaje de la CSFQ-14. Discusión: Se encontraron asociaciones estadísticas importantes que detallan posibles asociaciones entre ciertas variables del estudio a pesar de no alcanzar el “n” propuesto de la muestra. La mayor severidad de los síntomas negativos se evidencia en este estudio como una variable que puede repercutir en el funcionamiento sexual, al igual que los medicamentos antidepresivos. Respecto a la asociación con estos medicamentos vale la pena preguntarse si su efecto deletéreo es independiente o se asocia al manejo antipsicótico que tomaba el 97,6% de la muestra. Vale la pena realizar mayores estudios en esta área con muestras representativas que permitan una adecuada interpretación de los hallazgos estadísticos.
Abstract
Introduction: The nature of sexuality and sexual satisfaction of patients with mental illness remains a poorly studied area. Furthermore, it is widely accepted that people with mental illness experience high levels of sexual dysfunction, given the low satisfaction or low sexual interest in this community population. Methods: A Cross-sectional Descriptive study was carried out, where two scales were applied (one for men, the other for women) that measure sexual desire and functioning (Sexual Functioning Questionnaire Short-Form CSFQ-14), In addition, pharmacological management and schizophrenia severity (Positive and Negative Symptom Scale PANSS) of patients between 18 and 55 years of age who were managed by an outpatient psychiatric clinic with a diagnosis of schizophrenia, at the Clinica de Nuestra Señora de La Paz, during the years of 2020-2021 was typified. Results: We obtained a sample of 86 patients with a median age of 35 years, of whom 77.9% were men and 22.1% were women. The CSFQ-14 scale showed a median of 30 points and the PANSS scale a median of 68 points. Regarding the correlations between variables, a statistically significant and inversely proportional association was found between the PANSS score of negative symptoms and the CSFQ-14 score. On the other hand, the use of antidepressants also had an inverse and statistically significant correlation with the CSFQ-14 score. Discussion: Important statistical associations were found thattidentify possible associations between certain variables of the study despite not reaching the proposed "n" of the sample. The greater severity of negative symptoms is evident in this study as a variable that can impact on sexual functioning, as well as antidepressant drugs. Regarding the association with these drugs, it is worth considering if their deleterious effect is independent or if it is associated with antipsychotic management that 97.6% of the sample took. Further studies in this area with representative samples that allow a proper interpretation of the statistical findings are worthwhile.
