- ÍtemAcceso AbiertoA simulation model to evaluate pharmaceutical supply chain costs in hospitals: the case of a Colombian hospital(2018) Franco Franco, Carlos AlbertoBackground: Healthcare costs is one of the most studied issue in our days because of increasing demand and the aging of population. Final costs of medicines is one of the most important issue in patient treatment and determine its real value is an important task within hospitals. Simulation models and in this case system dynamics allows to build representations of reality considering the interaction of the whole variables that affect the system where first a causal loop diagram allows to represent and identify the interaction between variables for develop a stock flow diagram to determine the final results. Objective: Develop a simulation model that allows decision makers in Hospitals and Governments to identify the variables that affect the final cost of medicines and to determine the legal reimbursement allowed by national agencies. Methods: This paper presents a conceptual modeling framework using a causal loop diagram and a dynamic simulation model in the real case of a hospital in Colombia to explore how different internal charges for medicines affect the behavior of the final unit-dose cost of medicines, considering the complexity of the pharmaceutical system. We developed a simulation model to represent and characterize the pharmaceutical supply chain in a hospital and by using real data we validate the results of the model and conclude about the supply chain of medicines in Colombia using the legal regulations as a main factor of analysis. Results and conclusions: We found that in some cases the maximum reimbursement value is less than the final cost of medicines within the hospital, which means that hospitals lose money on the administration of medicines to patients. The benefit of this model is that with the result the hospital can determine the real final monetary value of medicines, including the different processes starting from the reception of the medicines, ending with the administration to patients. © 2018, Springer Nature Switzerland AG.
- ÍtemSolo MetadatosADHD Endophenotypes in Caribbean Families(2018) Cervantes-Henríquez, M L; Acosta-López, J E; Martínez-Banfi, M L; Vélez, J I; Mejía-Segura, E; Lozano-Gutiérrez, S G; Sánchez-Rojas, M; Zurbarán, M A; Zurek, E E; Arcos-Burgos, M; Pineda, D A; Puentes-Rozo, P JObjective: The aim of this study is to contrast the genetics of neuropsychological tasks in individuals from nuclear families clustering ADHD in a Caribbean community. Method: We recruited and clinically characterized 408 individuals using an extensive battery of neuropsychological tasks. The genetic variance underpinning these tasks was estimated by heritability. A predictive framework for ADHD diagnosis was derived using these tasks. Results: We found that individuals with ADHD differed from controls in tasks of mental control, visuospatial ability, visuoverbal memory, phonological and verbal fluency, verbal and semantic fluency, cognitive flexibility, and cognitive ability. Among them, tasks of mental control, visuoverbal memory, phonological fluency, semantic verbal fluency, and intelligence had a significant heritability. A predictive model of ADHD diagnosis using these endophenotypes yields remarkable classification rate, sensitivity, specificity, and precision values (above 80%). Conclusion: We have dissected new cognitive endophenotypes in ADHD that can be suitable to assess the neurobiological and genetic basis of ADHD. © 2018, The Author(s) 2018.
- ÍtemSolo MetadatosBibliometric analysis of the scientific production found in Scopus and Web Of Science about physiological vocal rehabilitation(2018) Calvache-Mora C.A.; Ríos-Ramírez M.A.Introducción: El presente estudio analiza la producción científica internacional encontrada en Scopus y Web of Science relacionada con la Corriente de Rehabilitación Vocal Fisiológica en el periodo 2006 a 2015. Metodología: Estudio exploratorio-descriptivo con 3 fases de desarrollo: a) selección de palabras clave y criterios de búsqueda; b) búsqueda y sistematización de información; c) análisis de la información. Resultados: La mayor producción científica se identifica en la herramienta de búsqueda Scopus bajo el criterio de búsqueda «voice therapy». Se analizaron un total de 167 publicaciones, realizando correlaciones a partir de las variables año, criterios de búsqueda, autores, factor de impacto por cuartil y por citación. Conclusión: Existe alta correlación y grado de solapamiento entre ambas bases de datos. Sin embargo, Scopus comparada con Web of Science, con relación a las líneas de la Corriente de Rehabilitación Vocal Fisiológica y para el periodo estudiado, presentó un mayor porcentaje de producción científica. Las líneas con mayor impacto científico son «terapia Lee Silverman» y «ejercicios de la función vocal». «Ejercicios TVSO» y «terapia de voz resonante», con menos porcentaje de publicaciones, desde 2011 incrementan progresivamente factores de alto impacto. Introduction: The present study looks at the international scientific production found in Scopus and Web of Science, related to the Physiological Vocal Rehabilitation Current in the period 2006-2015. Methodology: Exploratory-descriptive study with three phases of development: A) selection of keywords and search criteria; B) search and systematization of information; C) analysis of information. Results: The highest scientific production is identified in the Scopus search tool under the “voice therapy” search criterion. A total of 167 publications were analyzed, making correlations from the year, search criteria, authors, impact factor by quartile and by citation variables. Conclusion: There is a high correlation and degree of overlap between the two databases. However, Scopus compared to Web of Science, in relation to the Physiological Vocal Rehabilitation Current lines and for the period studied, presented a higher percentage of scientific production. The lines with the greatest scientific impact are “Lee Silverman therapy” and “vocal function exercises”. “SOVT exercises” and “resonant voice therapy”, with less percentage of publications, since 2011 progressively increase high impact factors. © 2018 Elsevier España, S.L.U. y Asociación Española de Logopedia, Foniatría y Audiología e Iberoamericana de Fonoaudiología
- ÍtemSolo MetadatosConsiderations about the Neuropsychiatric Conditions of Quixote of La Mancha(2018) Velasquez-Torres, Alejandro; Palacios Sánchez, LeonardoDon Quixote of La Mancha, the picturesque character created by Miguel de Cervantes Saavedra in his immortal book The Ingenious Nobleman Sir Quixote of La Mancha, presents several neuropsychiatric conditions, including tremor, sleep disturbances, neuropsychiatric symptoms, syncope, perception disorders and traumatic brain injury. Throughout the masterpiece, there are episodes where the aforementioned disorders are evident. This paper makes a list of them and analyses them in the light of the current knowledge of those conditions. © 2018 Asociación Colombiana de Psiquiatría
- ÍtemSolo MetadatosInfluence of Calcium and Vitamin D Intakes on Body Composition in Children and Adolescents(2018) Correa-Rodríguez M.; Schmidt-RioValle J.; Ramírez-Vélez R.; Correa-Bautista J.E.; González-Jiménez E.; Rueda-Medina B.Identifying nutritional strategies to maintain a healthy body weight and reduce the comorbidities associated with obesity is extremely important. We aimed to investigate whether calcium and vitamin D intakes are associated with body composition measurements in a population of children and adolescents. A cross-sectional study was conducted involving 1,060 children and adolescents (65.8% females; 34.2% males) aged 9 to 19 years. Fat mass, percentage of fat mass, and fat-free mass were measured using a body composition analyzer (TANITA BC-418MA®). The mean dietary calcium and vitamin intakes were 829.66 ± 328.34 mg/day and 200.78 ± 400.91 IU/day. Linear regression analysis revealed a lack of significant association between daily calcium and vitamin D intakes and body composition measurements, after adjusting the model for age, sex, maturation status, and energy intake. Dietary calcium and vitamin D intakes do not appear to be associated with higher adiposity measurements in children and adolescents. © The Author(s) 2018.
- ÍtemSolo MetadatosKnowledge and use of clinical coordination mechanisms in healthcare networks in Latin America(2018) Miranda-Mendizábal, Andrea; Vargas, Ingrid; Mogollón Pérez, Amparo Susana; Eguiguren, Pamela; Samico, Isabella; López, Julieta; Bertolotto, Fernando; Amarilla, Delia; Vázquez, María-LuisaObjective: To analyze the level of knowledge and use, and the characteristics of use, of care coordination mechanisms in public healthcare networks of six Latin America countries. Method: Cross-sectional study based on a survey using the COORDENA® questionnaire with primary and secondary care doctors (348 doctors/country) of public healthcare networks in Argentina, Brazil, Chile, Colombia, Mexico and Uruguay (May-October 2015). Analyzed variables: degree of knowledge and use of information coordination (referral/reply letter, discharge report, phone, e-mail) and of clinical management coordination (shared clinical guidelines, joint meetings) mechanisms. Descriptive analyses were conducted. Results: Knowledge of clinical information coordination mechanisms was high in both care levels and analyzed networks as was the use of referral/reply letter. There was greater variability in the use of discharge reports (from 40.0% in Brazil to 79.4% in Mexico) and, except for Argentina, a low reception reported by primary care doctors stands out (12.3% in Colombia and 55.1% in Uruguay). In contrast, knowledge of clinical management coordination mechanisms was limited, especially among secondary care doctors. It is noteworthy, however, that adherence to clinical guidelines was high (from 83.1% in Mexico to 96.8% in Brazil), while participation in joint meetings varied widely (from 23.7% in Chile to 76.2% in Brazil). The difficulties reported in the use of the mechanisms are related to structural and organizational factors. Conclusions: The limited knowledge and use of coordination mechanisms shows insufficient diffusion and implementation. Strategies to increase its use are needed, including the related factors. © 2018 SESPAS
- ÍtemAcceso AbiertoMuscle strength cut-offs for the detection of metabolic syndrome in a nonrepresentative sample of collegiate students from Colombia(2018) Garcia-Hermoso, Antonio; Tordecilla Sanders, María Alejandra; Correa Bautista, Jorge Enrique; Peterson, Mark D.; Izquierdo, Mikel; Quino-Ávila, Aura Cristina; Sandoval-Cuellar, Carolina; González-Ruíz, Katherine; Ramírez-Vélez, RobinsonBackground: Evidence shows an association between grip strength and health; however, grip strength cut-offs for the detection of metabolic syndrome (MetS) in Latin American populations are scarce. The purpose of this study was to determine cut-offs of normalized grip strength (NGS) for the detection of MetS in a large nonrepresentative sample of a collegiate student population from Colombia. Methods: A total of 1795 volunteers (61.4% female, age = 20.68 ± 3.10 years, mean ± SD), ranging between 18 and 30 years of age participated in the study. Strength was estimated using a handheld dynamometer and normalized to body mass (handgrip strength (kg)/body mass (kg)). Anthropometrics, serum lipids indices, blood pressure, and fasting plasma glucose were measured. Body composition was measured by bioelectrical impedance analysis. MetS was defined as including ?3 of the 5 metabolic abnormalities according to the International Diabetes Federation definition. A metabolic risk score was computed from the following components: waist circumference, triglycerides, high-density lipoprotein cholesterol, glucose, and systolic and diastolic blood pressure. Results: Receiver operating curve analysis showed significant discriminatory accuracy of NGS in identifying the thresholds and risk categories. Lower strength was associated with increased prevalence of MetS. In males, weak, intermediate, and strong NGS values at these points were less than 0.466, 0.466–0.615, >0.615, respectively. In females, these cut-off points were less than 0.332, 0.332–0.437, >0.437, respectively. Conclusion: Our sex-specific cut-offs of NGS could be incorporated into a clinical setting for identifying college students at cardiometabolic disease risk. © 2018
- ÍtemSolo MetadatosSpeculating on tentacular infrastructures(2018) Bocarejo Suescún, DianaTrains, water pipes and many other infrastructures whisper, and sometimes even roar. There is the sound of metal gripping water tanks, the echoes of deep wells, the soothing sounds of rivers stifled in irrigation ditches, and the sudden rumble of the train. The visual and audible life of infrastructures reverberates through the landscape of the immense green banana and palm oil plantations of the Zona Bananera, Colombia. Walking what was before a banana emporium built by the United Fruit Company involves knowing how to move within infrastructures. This ethnographic photo-essay is a reflection on the everyday becoming of both infrastructure and peasant zoneros. ‘The Octopus’, as the United Fruit Company is still called by locals, expanded its tentacles through a wide variety of materials. As much as tentacles can violently suffocate they can also be tamed and tricked. By repurposing infrastructures speculative and creative lives are lived, felt and imagined. © The Author(s) 2018.

Revisión sistemática y metaanálisis de proporciones.(2019) Vega-Lopez, Johanna; Sanchez-Moreno, David; Duran-Torres, FelipeOBJECTIVE: Female urinary incontinence has a prevalence of up to 40%. Stress urinary incontinence is the most frequent type, about 50%. When primary management fails, it is unclear what the behavior should be. This study aims to show the cure rate of the second-line management options described in the literature.MATERIAL AND METHOD: Systematic review of the literature with meta-analysis of proportions. Observational and interventional studies were included in which the different types of management of recurrent/persistent urinary incontinence were evaluated in women who had a mid-urethral tape as primary management.RESULTS: 161 studies were identified and screened, including 29 in the qualitative synthesis and 27 in the quantitative synthesis. Given the high clinical and methodological heterogeneity, an estimation of the cure rate for each type of management was performed, with the following results regarding the total combined cure rate: in the implantation of adjustable tapes of 86% (IC95%: 76.4% -92.1%), the implantation of a second mid urethral tape was 75.1% (95% CI: 68% -81.1%), in the shortening of the previous mid urethral tape was 62.3% (95% CI: 49.1% -73.9%) and finally the implantation of bulking agents was 55.4% (95% CI: 43.2% -67%). We did not find enough studies to perform a quantitative synthesis with respect to: pubovaginal sling, colposuspension and pelvic floor therapy, as well as secondary outcomes.CONCLUSION: The evidence found shows that there may be superiority of the adjustable tapes versus the mid urethral tapes and other included treatments. However, a comparison to statistically corroborate this difference could not be made. These results should be confirmed with multicenter collaborative randomized clinical trials.
