Supervivencia en pacientes adultos con diagnóstico de adenocarcinoma gástrico atendidos en el Hospital Militar Central

dc.descriptionEl cáncer gástrico representa un problema de salud pública debido a su alta incidencia. La supervivencia de todos los tipos de cáncer en forma global se calcula en 68.9% a 5 años, en contraste con el cáncer gástrico donde la supervivencia alcanza el 32%. A nivell local algunos estudios han encontrado sobrevida a 5 años del 10,6% en contraposición con países donde se encuentran tasas de sobrevida del 48,5%. La toma de dato se realizó de los registros del servicio de patología y en las historias clínicas del Hospital Militar Central del personal diagnosticado con adenocarcinoma gástrico en el periodo entre 2012 y 2015. Se obtuvieron 98 registros, la media de edad fue 59,3 años (59,3±18,12). Por sexo la distribución fue hombres 74.5% y mujeres 25.5%; por tipo histológico intestinal 52%, difuso 39.8% y mixto 8.2%; Por estadios, el IV fue el más frecuente con 45.9%. La tasa de supervivencia global fue de 19%. La mediana de supervivencia global fue de 18.2 meses (IC del 95% de 11.5-24.8) y la tasa de supervivencia para menores de 60 años se estimó en 25% y para mayores de 60 años fue 24%, analizadas a 4 años. La tasa global de sobrevida fue de 19%, inferior a la reportada en países desarrollados, aunque la sobrevida para el primer y segundo año son similares a éstos, lo que se explica por un tratamiento adecuado pero un diagnóstico tardío, ya que el 77.5% de los pacientes de nuestro estudio estaban en estadios III y IV.spa
dc.description.abstractGastric cancer represents a public health problem because of its high incidence. Survival of all types of cancer globally is estimated at 68.9% at 5 years, in contrast to gastric cancer where survival only reaches 32%. In our sample, some studies have found a 5-year survival rate of 10.6% (29) as opposed to countries where survival rates are 48.5%. Data collection was done by searching the records of the pathology service and the medical histories of the Central Military of personnel diagnosed with gastric adenocarcinoma in the Period between 2012 and 2015. The mean age was 59.3 years (59,3±18,12). The gender distribution was in men 74.5% and women 25.5%; by intestinal histological type 52%, diffuse 39.8% and mixed 8.2%; by stages, IV was the most frequent with 45.9%. The overall survival rate was 19%. The median overall survival was 18.2 months (95% CI of 11.5-24.8) and the survival rate for those younger than 60 years was estimated at 25% and 24% for those over 60 years old, analyzed at 4 years.The overall survival rate was 19% for the 48-month study period, lower than that reported for developed countries (The United States for 29%). The survival for the first and second year is similar to that for the study period. It´s can be explained by adequate treatment but a late diagnosis, since 77.5% of the patients in our study were in stages III and IV.eng
