Evaluación y seguimiento clínico de pacientes intervenidos quirúrgicamente por fracturas claviculares y luxaciones acromioclaviculares en una institución de cuarto nivel en Bogotá-Colombia.

| dc.description | Las fracturas de clavícula (FC) y las luxaciones acromioclaviculares (LAC) son lesiones frecuentes del hombro que, en muchos casos, requieren manejo quirúrgico. Objetivo: Describir los desenlaces clínicos y funcionales de pacientes intervenidos quirúrgicamente por FC en el tercio medio o distal y por LAC, tratados entre 2021 y 2023 en una institución de cuarto nivel en Bogotá, Colombia. Materiales y Métodos: Estudio observacional, descriptivo de corte transversal con selección de 35 pacientes hecho mediante muestreo por conveniencia. Se realizó seguimiento clínico a 18 meses mediante la escala Constant-Murley Shoulder Score (CMSS). Resultados: El 77,1% de los casos correspondió a FC, principalmente del tercio medio. El mecanismo de trauma más común fue la caída en bicicleta (54,3%). El puntaje CMSS promedio fue de 83,6 puntos (DE 16,7), indicando una recuperación biomecánica satisfactoria. Sin embargo, el 80% reportó limitaciones funcionales en actividades diarias y el 62,86% en actividades deportivas. El grupo tratado con placa gancho presentó mayor dolor, mayor tasa de retiro de material y menor tolerancia funcional. La tasa de complicaciones posoperatorias fue baja (2,9%). Conclusión: El seguimiento a mediano plazo mostró resultados clínicos positivos, pero se evidenció una discrepancia entre la funcionalidad objetiva y la percepción subjetiva del paciente. Se recomienda complementar la evaluación con instrumentos que integren aspectos psicosociales y ocupacionales. | |
| dc.description.abstract | Clavicle fractures (CF) and acromioclavicular dislocations (ACD) are common shoulder injuries that often require surgical management. Objective: To describe the clinical and functional outcomes of patients undergoing surgery for midshaft or distal CF and ACD, treated between 2021 and 2023 at a level IV institution in Bogotá, Colombia. Methods: Observational, descriptive, and retrospective study involving 35 patients selected through convenience sampling. Clinical follow-up was conducted up to 18 months using the Constant-Murley Shoulder Score (CMSS). Results: Clavicle fractures accounted for 77.1% of cases, mostly affecting the midshaft. Bicycle falls were the most frequent trauma mechanism (54.3%). The mean CMSS was 83.6 (SD 16.7), suggesting favorable biomechanical recovery. However, 80% reported significant limitations in daily activities and 62.86% in sports. Patients treated with hook plates experienced more pain, higher implant removal rates, and greater functional impairment. The overall postoperative complication rate was low (2.9%). Conclusion: Mid-term follow-up showed generally positive clinical outcomes, although a discrepancy was noted between objective functionality and patients’ subjective perception. The findings highlight the need for complementary assessment tools that incorporate psychosocial and occupational dimensions. | |
