Control metabólico en pacientes diabéticos y relación con procesos infecciosos durante julio 2020 y marzo de 2021 en Clínica del Occidente de Bogotá

dc.description | OBJETIVO: Determinar la asociación entre los niveles de hemoglobina glicosilada y la prevalencia de microorganismos resistentes en infecciones del tracto urinario o de tejidos blandos en pacientes diabéticos admitidos en urgencias de la clínica del Occidente durante julio de 2020 y marzo de 2021. MATERIALES Y MÉTODOS: Estudio observacional transversal analítico de una cohorte de pacientes diabéticos admitidos en urgencias por procesos infecciosos de vías urinarias y tejidos blandos durante un periodo de 8 meses para posterior análisis estadístico bivariado. RESULTADOS: Se obtuvo una muestra de 72 pacientes con un promedio de edad de 65 años, se encontró resistencia bacteriana en 18 pacientes (25%) de los cuales el 55% (n=10) corresponden al sexo femenino, 76% (n=13) corresponden a estrato 1 y el 94% (n=17) tenían comorbilidades, mayor prevalencia la infección de vías urinarias (83%) y mayor proporción de infecciones en pacientes con HbA1c fuera de metas sin embargo, sin hallazgos estadísticamente significativos con respecto a la resistencia bacteriana (p=0,129), la mediana de SOFA es de 2 puntos con mayor severidad en infecciones de tejidos blandos (p=0,014), la duración máxima de terapia antibiótica para infección urinaria fue 10 días y para infección de tejidos blandos 21 días (p=0,000). CONCLUSIONES: Los pacientes diabéticos con hemoglobina glicosilada fuera de metas presentan mayor riesgo de infección de vías urinarias y de tejidos blandos por lo cual se debe priorizar el control metabólico desde la atención primaria en salud con el fin de mitigar el impacto en la salud pública. | spa |
dc.description.abstract | OBJECTIVE: To determine the association between glycosylated hemoglobin levels and the prevalence of resistant microorganisms in urinary tract or soft tissue infections in diabetic patients admitted to Clinica del Occidente emergencies during July 2020 and March 2021. MATERIALS AND METHODS: Analytical cross-sectional observational study of a cohort of diabetic patients admitted to the emergency department due to infectious processes of the urinary tract and soft tissues with recording of the variables of interest in a database for a period of 8 months for subsequent bivariate statistical analysis. RESULTS: A sample of 72 patients with an average age of 65 years was obtained, bacterial resistance was found in 18 patients [25%] of which 55% (n=10) correspond to the female sex, 76% (n=13) correspond to stratum 1 and 94% (n=17) had comorbidities, higher prevalence urinary tract infection (83%) and higher proportion of infections in patients with HbA1c outside of targets however without statistically significant findings regarding bacterial resistance (p=0.129), the median SOFA is 2 points with greater severity in soft tissue infections (p=0.014), the maximum duration of antibiotic therapy for urinary tract infection was 10 days and for soft tissue infection 21 days (p=0.000). CONCLUSIONS: Diabetic patients with glycosylated hemoglobin outside of goals present an increased risk of urinary tract and soft tissue infection, so metabolic control should be prioritized from primary health care to mitigate the impact on public health. | spa |
