Interacciones en las rotaciones clínicas de un grupo de residentes de pediatría: una mirada desde la teoría del cruce de fronteras

dc.descriptionIntroducción: El aprendizaje en el lugar de trabajo es un componente esencial en la formación del personal de salud. No obstante, se trata de un proceso dinámico y complejo que puede presentar múltiples desafíos. Entre los factores que influyen en este aprendizaje, la interacción entre estudiantes de distintas especialidades desempeña un papel clave. Este estudio explora cómo las interacciones entre residentes de pediatría y residentes de otras especialidades médicas durante las rotaciones clínicas contribuyen a su aprendizaje. Métodos: Se llevó a cabo un estudio cualitativo basado en la teoría fundamentada constructivista, enmarcado en la teoría del cruce de fronteras. Se realizaron entrevistas semiestructuradas a nueve residentes de pediatría que rotaban en la Fundación HOMI Hospital Pediátrico la Misericordia. La recolección y el análisis de los datos se realizaron de manera iterativa, permitiendo que los hallazgos emergieran a través del proceso de comparación constante entre casos y categorías. Asimismo, se utilizó el muestreo teórico para seleccionar a los participantes con base en la evolución del análisis, lo que facilitó una comprensión progresivamente más rica y matizada del fenómeno estudiado. Resultados: Se identificaron tres procesos clave en las interacciones entre residentes de pediatría y residentes de otras especialidades durante las rotaciones clínicas: (1) la coordinación, a través de mecanismos como la rutinización, la conexión comunicativa y los esfuerzos de traducción; (2) la identificación, mediante la legitimación de la coexistencia; y (3) la reflexión, promovida por la construcción y adopción de nuevas perspectivas. Los residentes valoraron estas interacciones como altamente útiles para su formación. Conclusiones: Las interacciones entre residentes de distintas especialidades favorecen el aprendizaje en el lugar de trabajo, promueven el intercambio de conocimientos entre pares y fortalecen la comprensión del rol del pediatra dentro de un equipo multidisciplinario. Asimismo, facilitan una mejor comprensión del rol del otro y fomentan la empatía hacia perspectivas diversas dentro del ámbito clínico.
dc.description.abstractIntroduction: Workplace learning is an essential component of healthcare professional training. However, it is a dynamic and complex process that can present multiple challenges. Among the factors that influence this learning, interaction between students from different specialties plays a key role. This study explores how interactions between pediatric residents and residents from other medical specialties during clinical rotations contribute to their learning. Methods: A qualitative study based on constructivist grounded theory was conducted, framed within the theory of crossing boundaries. Semi-structured interviews were conducted with nine pediatric residents rotating at the Fundación HOMI Hospital Pediátrico la Misericordia. Data collection and analysis were performed iteratively, allowing findings to emerge through the process of constant comparison between cases and categories. Theoretical sampling was also used to select participants based on the evolution of the analysis, which facilitated a progressively richer and more nuanced understanding of the phenomenon under study. Results: Three key processes were identified in the interactions between pediatric residents and residents from other specialties during clinical rotations: (1) coordination, through mechanisms such as routinization, communicative connection, and translation efforts; (2) identification, through the legitimization of coexistence; and (3) reflection, promoted by the construction and adoption of new perspectives. Residents rated these interactions as highly useful for their training. Conclusions: Interactions between residents from different specialties foster learning in the workplace, promote knowledge exchange among peers, and strengthen understanding of the role of the pediatrician within a multidisciplinary team. They also facilitate a better understanding of each other's roles and foster empathy toward diverse perspectives within the clinical setting.
