| dc.description | Antecedentes: la manera en que se transmiten las malas noticias en el ámbito médico puede fortalecer o dañar la relación médico-paciente. Para los médicos, esta tarea es altamente estresante, impactando su bienestar emocional y desempeño laboral. Este desafío es especialmente significativo para los residentes de pediatría, quienes a menudo carecen de formación estructurada, lo que aumenta su estrés y perciben una sensación de falta de preparación. Por ello, conocer las experiencias de los residentes de esta especialidad durante su formación de posgrado permitirá para establecer estrategias efectivas que aporten al aprendizaje de comunicación de malas noticias. Propósito: explorar como ha sido la experiencia de los residentes de pediatría de dos universidades privadas acreditadas en Colombia, en el aprendizaje de comunicar malas noticias a pacientes y familiares durante el proceso de formación de posgrado. Métodos: se realizó un estudio de investigación cualitativa, de tipo fenomenológico hermenéutico basado en la arquitectura de las prácticas. Se realizaron entrevistas semiestructuradas a 14 residentes de dos instituciones privadas acreditadas. La recolección de datos y el análisis se realizó de manera iterativa. Se realizó la codificación y el análisis de los datos en el programa Quirkos. Resultados: bajo el lente de la arquitectura de las prácticas, la estructura del fenómeno de aprendizaje de dar malas noticias se puede describir a través de tres significados: discursos que modifican la experiencia de dar malas noticias (discursivo-cultural), rotaciones y actividades que influyen en el aprendizaje de esta competencia (material-económicos) y las relaciones vividas en la transmisión de malas noticias (socio-político). Conclusiones: La observación crítica al docente es la principal práctica del aprendizaje de transmisión de malas noticias la cual es enriquecida y modificada por las rotaciones clínicas y las relaciones de poder entre el profesor y el residente. La competencia se fortalece con un grupo de Cuidados Paliativos Pediátricos establecido en los sitios de práctica y con actividades de enseñanza que utilicen la reflexión y/o la simulación. | |
| dc.description.abstract | Background: The way in which bad news is conveyed in the medical setting can either strengthen or damage the patient-physician relationship. For physicians, this task is highly stressful, impacting their emotional well-being and job performance. This challenge is especially significant for pediatric residents, who often lack structured training, which increases their stress and they perceive a sense of unpreparedness. Therefore, learning about the experiences of residents in this specialty during their postgraduate training will allow for the establishment of effective strategies that contribute to learning how to communicate bad news. Purpose: To explore the experience of pediatric residents of two accredited private universities in Colombia in learning to communicate bad news to patients and families during the postgraduate training process. Methods A qualitative research study was conducted, based on hermeneutic phenomenology and the architecture of practices. Semi-structured interviews were conducted with 14 residents from two accredited private institutions. Data collection and analysis was conducted iteratively. Data coding and analysis were performed in the Quirkos program. Results: Under the lens of the architecture of practices, the structure of the phenomenon of learning to deliver bad news can be described through three meanings: discourses that modify the experience of delivering bad news (discursive-cultural), rotations and activities that influence the learning of this competence (material-economic) and the relationships experienced in the transmission of bad news (socio-political). Conclusions: Critical observation of the teacher is the main practice of learning the transmission of bad news, which is enriched and modified by clinical rotations and power relations between the teacher and the resident. Competency is strengthened by an established Pediatric Palliative Care group at practice sites and teaching activities that utilize reflection and/or simulation. | |
