Resumen
Introducción La deserción en los programas de cirugía es un fenómeno creciente en los últimos años a nivel mundial. Las investigaciones sobre deserción habitualmente son enfocadas hacia los estudiantes, sin embargo, a los docentes clínicos, quienes son el principal eslabón del estudiante con la institución, poco se les ha indagado acerca de su percepción y participación en este hecho. Esta investigación nos motiva a interpretar la deserción desde la perspectiva de los docentes y dar respuesta a cómo ellos experimentan la deserción. Métodos Se trata de un estudio de fenomenología hermenéutica, donde los autores realizaron 24 entrevistas en docentes clínicos, desertores y un docente investigador. Las entrevistas semiestructuradas se recolectaron en el segundo trimestre de 2023, se incluyeron actores del fenómeno en los últimos cinco años (2018-2023). Los datos fueron analizados desde la teoría de los mundos figurados. Resultados Los docentes clínicos del programa de cirugía general del PUJ tienen institucionalizado en su mundo figurado el discurso de lo que debe caracterizar a un buen cirujano. Aducen que se requiere de un nivel de abnegación y entrega hacia los pacientes que se anteponen a otros roles de su vida. Apalancados en este discurso, durante la formación de cirujanos, se ejercen agencias específicas que buscan detectar al estudiante que se alinea con el mundo figurado. Estas agencias incluyen trabajar bajo retos y presiones constantes. Aquellos residentes que no pasan los retos o que no se quieren alinear con el discurso terminan desertando. Conclusiones Los docentes en el mundo figurado del programa de cirugía de PUJ ejercen agencias encaminadas a detectar cuáles residentes cuentan con lo que se requiere para alinearse o no con el discurso de buen cirujano. Esto lleva a que se genere un ambiente de retos y presiones constantes que terminan influyendo en el crecimiento del fenómeno de la deserción. Los docentes de generaciones más recientes, si bien se alinean con el discurso, consideran que es necesario formarse para educar y contar con herramientas que fomenten que las nuevas generaciones de residentes deseen alinearse con dicho discurso.
Abstract
Attrition from surgery programs is a growing phenomenon in the recent years worldwide. Research on Attrition is usually focused on students, however, on clinical teachers, who are the main link of the student with the institution, little has been asked about them their perception and participation in this fact. This research motivates us to interpret attrition from the perspective of teachers and respond to how they experience desertion. methods It is a study of hermeneutic phenomenology, where the authors 24 interviews were conducted with clinical teachers, dropouts and a research teacher. Semi-structured interviews were collected in the second quarter of 2023, actors of the phenomenon were included in the last five years (2018-2023). The data They were analyzed from the theory of figurative worlds. Results The clinical professors of the PUJ general surgery program have institutionalized in his figurative world the discourse of what should characterize a good surgeon. They argue that a level of self-sacrifice and delivery is required towards the patients who take precedence over other roles in their lives. Leveraged in this speech, during the training of surgeons, specific agencies are exercised that seek detect the student who aligns with the figurative world. These agencies include work under constant challenges and pressures. Those residents who do not pass the challenges or who do not want to align with the discourse end up deserting. conclusions Faculty in the figurative world of the PUJ surgery program exercise agencies aimed at detecting which residents have what is required to align or not with the discourse of a good surgeon. This leads to the generation of a environment of constant challenges and pressures that end up influencing growth of the attrition phenomenon. The teachers of more recent generations, although are aligned with the discourse, consider that it is necessary to train to educate and have tools that encourage new generations of residents wish to align with said discourse.
