Resumen
El presente proyecto de grado se centra en la factibilidad de la validación de la escala de Rassi en pacientes con cardiomiopatía chagásica atendidos en un centro de referencia cardiovascular en Bogotá a través de la concordancia interobservador y la evaluación de la disponibilidad de datos, completitud y reproducibilidad para ser clasificados en los diferentes grupos de riesgo y de esta manera predecir el riesgo de mortalidad en dicha población.
Abstract
Trypanosoma Cruzi infection is an endemic parasitic infection in Latin American countries. Approximately 30 to 40% of those infected develop some form of organ involvement, with Chagas cardiomyopathy being the most significant. The Rassi score evaluates the severity of the disease and predicts the risk of death through various clinical and paraclinical criteria. However, no studies have validated this score in our setting. Methodology: A feasibility study was conducted to validate the Rassi score. We established the availability of data, its completeness, its reproducibility through interobserver concordance, and the distribution in the risk groups established by Rassi. Results: Interobserver concordance for functional class criteria was good, with an index of 0.611 95% CI (0.461-0.729), and it was moderate for global/segmental contractility disorders, cardiomegaly, and NSVT with indices of 0.576 (0.428-0.698), 0.470 (0.301-0.611), and 0.449 (0.230-0.626) respectively. There was no concordance for low voltage [-0.043 (-0.167-0.082)]. Fifty-nine (54.6%) participants met the complete criteria, and of these, 18 (30.5%) were classified in the low-risk group, 22 (37.2%) in the intermediate group, and 19 (32.2%) in the high-risk group. Conclusion: In this study, half of the participants had complete information. The concordance between observers for measuring the criteria was moderate in most cases, and the classification according to the risk score was distributed with a greater weight in the high and intermediate risk groups. This highlights the importance of conducting the study in centers of different complexity and of developing a strategy to standardize data collection.
